About this file

This document is a General Information and Questions and Answers file for Request for Proposal #PE-12800-NONST-2026-000000018, issued by the Fulton County Board of Health in partnership with the Centers for Disease Control and Prevention (CDC) for HIV Prevention Services under CDC-RFA-PS24-0047. The RFP seeks proposals for High Impact HIV Prevention and Surveillance services from health departments and qualified organizations. The submission deadline is November 14, 2025, at 11:59 PM, with contract implementation commencing June 1, 2025. The proposal process requires applicants to submit comprehensive applications including executive summaries, organizational histories, officer and director information, financial stability documentation, cover letters signed by authorized representatives, and evidence of relevant professional licenses or certifications such as CLIA certificates or business licenses. Applicants applying for Category A (Non-Healthcare Setting HIV Testing) may propose services to multiple priority populations but must submit separate proposals, workplans, and budgets for each population.

Funding for this opportunity is provided through CDC Cooperative Agreement PS24-0047, with the Catalog of Federal Domestic Assistance (CFDA) number 93.940 (HIV Prevention Activities - Health Department Based). The RFP allocates $40,000 for PrEP/PEP expansion activities and $10,000 for National Black HIV/AIDS Awareness Day activities; these allocations cannot be divided across multiple subpopulations and must be assigned to specific priority population proposals. Applicants must demonstrate financial stability through audited financial statements or acceptable alternatives such as compiled statements, IRS Form 990s, or bank statements. HIV testers certified by the Georgia Department of Public Health in September 2024 and remaining in good standing do not require recertification in 2025. Applicants must also disclose any business relationships with Fulton County Board of Health within the past five years. The RFP was developed in accordance with Fulton County Board of Health and Georgia Department of Public Health procurement standards and will be reviewed for alignment with applicable federal executive orders, with any updates posted to the Georgia Procurement Registry.

REQUEST FOR PROPOSAL # PE-12800-NONST-2026-000000018

2024-HIV Prevention Services CDC-RFA-PS24- 0047 High Impact HIV Prevention and Surveillance for Health Departments

General Information and Questions and Answers

1. Should we list names and business addresses of each of the Offeror’s firm’s officers and directors?

Yes — this refers to your organization’s leadership, such as your Board of Directors (if applicable) and executive leadership team. Include the full name, title, and business mailing address for each.

2. General development of the Offeror’s business

This question is asking for a brief history of your organization over the last five years (or since inception if newer). You should highlight key milestones, growth, changes in services offered, major partnerships, and other relevant developments that demonstrate organizational stability and capacity to deliver the proposed services.

3. Disclosure of relationships with Fulton County Board of Health

This question asks if any employees, agents, or representatives directly involved in the proposed project have had — within the last five years — any type of business relationship with Fulton County Board of Health beyond this RFP process. This includes, but is not limited to, contracts, consulting work, revenue earned from Fulton County property, or other business arrangements. Service agreement contracts specifically related to previous or current FCBOH work should be disclosed as part of this answer.

4. Professional License requirements

Exhibit D-Business License and CLIA Certification are required for clinical testing. If your organization is not a medical clinic, you would not provide a CLIA waiver.

Instead, you should provide your current Business License or any applicable professional certifications relevant to your services. If the offeror does not have a CLIA Certificate or Waiver, the offeror can provide evidence that the CLIA Certificate of Waiver has been applied for.

5. Financial stability documentation Audited financial statements are the preferred documentation to demonstrate financial stability. If audited statements are not available, you may submit alternative documents such as compiled or reviewed financial statements from an accountant, recent IRS Form 990 (if a nonprofit), or bank statements showing financial solvency. This information should be included in the “Financial Information” section of your proposal, as outlined in the RFP.

6. Cover letter requirements

The cover letter should be signed by an authorized representative of your organization and include:

• The name of your organization

• The RFP number and title

• A brief statement of your organization’s interest and capacity to fulfill the scope of work

• Contact information for the authorized representatives

• Signature and date

7. HIV tester certification requirement

If your testers were certified by DPH in September 2024 and remain in good standing, they do not need to be recertified in 2025 unless otherwise directed by DPH. Please ensure that certifications are current and valid at the time-of-service delivery under the contract.

8. For budgeting purposes, is it ok to spread the $40,000 allocated to Prevent PrEP/PEP expansion (under component 3 on page 7) between our four proposed subpopulations? If yes, is it ok to include this line item within the subpopulation budget?

No. According to the RFP, agencies applying for Category A (Non-Healthcare Setting HIV Testing) may apply for multiple priority populations; however, separate proposals, workplans, and budgets must be submitted for each priority population (see page 7 of the RFP). Therefore, the $40,000 allocated for PrEP/PEP expansion under Component 3 cannot be split across multiple subpopulations. It must be assigned to the specific priority population proposal to which the associated activities and outcomes will apply. Each budget should independently reflect the full scope and cost of activities proposed for that priority population.

9. For budgeting purposes, is it ok to spread the $10,000 allocated for our chosen National Black HIV/AIDS Awareness Day (under component 4 on page 7) between our two subpopulations that serve Black/ African American individuals? If yes, is it ok to include this line item within the subpopulation budget?

No. As with Component 3, each priority population requires a separate proposal, workplan, and budget. Therefore, the $10,000 allocated for National Black HIV/AIDS Awareness Days cannot be split between two subpopulations. The awareness day funding must be associated with one priority population proposal only. Any related activities and expenditure should be reflected solely within the budget for that specific priority population.

10. Please provide the original Funding Opportunity Number from the appropriate federal agency, as well as the CFDA Number for this opportunity.

The funding supporting this RFP is aligned with CDC Cooperative Agreements PS24- 0047. The Catalog of Federal Domestic Assistance (CFDA) number associated with these awards is 93.940 – HIV Prevention Activities – Health Department Based.

11. On page 5 of the RFP in section 1, 1.1 Project description, funds are issued by the Center for Disease Control and Prevention (CDC) which is a Federal agency.

Will the Health Department be issuing a revised RFP that aligns with the language outlined in Executive Orders 14148, 14168, 14236?

The RFP was developed in accordance with both Fulton County Board of Health and Georgia Department of Public Health procurement standards. We will review the referenced Executive Orders and make any necessary updates to ensure alignment with applicable federal requirements. Any revisions or updates will be posted to the Georgia Procurement Registry’s purchasing portal where the RFP is listed.

12. QUESTION: On page 38 of the RFP, section 1-Executive Summary, Item 4 asks applicants to “Include a brief statement of approach to the work, understanding of the project’s goals and objectives and demonstrated understanding of the project’s potential problems and concerns”. However, there is no designated place in section 1 of the RFP Proposer Application Template. Should the applicant add it at the bottom of Section 1 of the template?

Yes, applicants should include this information at the end of Section 1 in the proposal application template. We encourage all proposers to ensure their narrative addresses the guidance as outlined in both the RFP instructions and the template, even if slight formatting adjustments are needed.

14. For budgeting purposes is it ok to spread the $10,000 allocated for our chosen National Black HIV/AIDS Awareness Day (under component 4 on page 7) between our two subpopulations that serve Black/ African American individuals? If yes, is it ok to include this line item within the subpopulation budget?

No. As with Component 3, each priority population requires a separate proposal, workplan, and budget. Therefore, the $10,000 allocated for National Black HIV/AIDS Awareness Days cannot be split between two subpopulations. The awareness day funding must be associated with one priority population proposal only. Any related activities and expenditures should be reflected solely within the budget for that specific priority population.

15. We are perplexed as to why we are given few working days to respond to such a complex RFP. Would the Health Department please consider extending the due date (given the contract does not begin until June 1, 2026) to give applicants enough time to submit a solid application without rushing and scrambling to meet the extremely short deadline?

We appreciate the concern regarding the timeline. The implementation start date is June 1, 2025. The procurement timeline was designed to allow sufficient time for evaluation, contracting, and onboarding. At this time, November 14, 2025, 11:59 PM is the submission deadline and this remains firm. Your request for an extension has been noted and will be reviewed internally, but no changes are anticipated.
