About this file

This is a bread delivery schedule for the John J. Madden Mental Health Center (MHC) in Illinois, establishing a small purchase contract for weekly bread and bun deliveries. The facility, located at 1200 S. First Avenue in Hines, Illinois, requires weekly deliveries comprising 27 loaves of 1.5 lb white Pullman bread, 27 loaves of 1.5 lb whole wheat Pullman bread, 24 dozen dinner rolls, 18 dozen hamburger buns, 9 packages of hot dog buns, and 9 dozen fresh rolls or equal items. The contract term extends through June 30, 2027, with items ordered on an as-needed basis. The quantities specified are estimates and do not represent guaranteed purchase amounts during the contract period.

No specific pricing terms, set-aside provisions, incumbent contractor information, or funding sources are detailed in this delivery schedule. The document serves as an operational specification for a small purchase contract, with deliveries to be coordinated through the facility's procurement process rather than through formal bidding procedures typical of larger state contracts.
