Attachment 4 – Past Performance Questionnaire
Please return this completed questionnaire directly to acqsubmission@ftc.gov by the date and time the proposals are due to the FTC. Please include the following subject line for the email: “[Offeror Name] – Past Performance Questionnaire.”
Past performance questionnaires submitted as a part of the proposal, by the offeror or anyone other than the person identified in the past performance section of the proposal will not be considered.

SECTION 3: EVALUATION RATINGS
The rating scale is defined as follows:
Rating Description
Exceptional E Performance met all contractual requirements and exceeded many requirements. The contractual performance of the element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
Very Good V Performance met contractual requirements and exceeded some requirements. The contractual performance of the element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
Acceptable A Performance met contractual requirements. The contractual performance of the element being assessed contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
Marginal M Performance did not meet some contractual requirements. The contractual performance of the element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor's proposed actions appear only marginally effective or were not fully implemented.
Unacceptable U Performance did not meet most contractual requirements. The contractual performance of the element being assessed contains serious problem(s) for which the contractor's corrective actions appear or were ineffective.
Not Applicable N Unable to provide a score. Performance in this area is not applicable to effort assessed.
SECTION 4: EVALUATION FACTORS
