About this file
This document is a Front Desk Screening Questionnaire from the Department of Mental Health (DMH) that has been revised as of 8-26-21. The purpose of this questionnaire is to screen employees for COVID-19 symptoms and potential exposure prior to entering the facility. The questionnaire asks employees about symptoms, travel history, close contacts with COVID-19 positive individuals, and recent positive COVID-19 tests. Based on the employee's responses, the questionnaire outlines the appropriate actions to be taken, such as sending the employee home or allowing them to continue working with additional testing and masking requirements.
The document does not provide any details about a state or local contract opportunity. It is focused solely on COVID-19 screening protocols for DMH employees.
