About this file
This is a forms guidance document for non-POS (non-Blanket Purchase Order) solicitations issued by MassAbility, a Massachusetts state agency. The guidance provides bidders with instructions for completing and submitting required forms associated with contract opportunities, specifically addressing the submission requirements for the Medical Items & Prosthetics for MassAbility Participants procurement. Bidders must submit the Bidder Response Worksheet and Cover Page, Bidder Response Checklist, Supplier Diversity Plan for Goods and Services, and a Department of Revenue Certificate of Good Standing by the proposal submission deadline. The Bidder Response Worksheet serves as the primary proposal evaluation document and must include accurate contact information for the designated contact person who will receive award or non-award notifications. All required forms must be complete and accurate; missing or incomplete documentation may render a proposal non-responsive and ineligible for evaluation.
The Supplier Diversity Plan represents 25 percent of the total proposal score and requires bidders to report prior year organizational revenue, spending with certified diverse businesses, projected current year revenue, and planned spending with certified diverse businesses. Bidders must use only vendors certified by the Massachusetts Supplier Diversity Office and should verify certification through the SDO Directory prior to submission. The Department of Revenue Certificate of Good Standing, distinct from the Secretary of the Commonwealth certificate, must be obtained early as processing typically requires several weeks. Successful bidders will be required to submit additional forms after award notification, including a Substitute W-9, Electronic Funds Transfer Form, Certified Authorized Signatory Listing, Certificate of Insurance, CORI Assurance Form, and Prompt Pay Discount Form, but these should not be included with the initial proposal submission.

Guide to Required Forms Submission
This guide is intended to help bidders complete and submit the required forms associated with Non-POS solicitations. Bidders are responsible for reviewing all RFR requirements and submitting complete and accurate documentation by the submission due date.
Missing required forms, submitting incomplete forms, or submitting incorrect forms may result in a proposal being deemed non-responsive. Non-responsive proposals cannot be evaluated or awarded.
Before You Begin
Before completing the required forms, bidders should:
• Review the Bidder’s Checklist which includes a list of all required forms.
• Identify the individual(s) who will prepare the required forms. This person could be different than the person who will write the program proposal.
• Gather information needed to complete the Supplier Diversity Plan (SDP for Goods and
Services), including any existing relationships with certified diverse businesses.
• Request a Department of Revenue Certificate of Good Standing as early as possible, as processing may take several weeks.
Forms Required with Proposal Submission
1. Bidder Response Worksheet and Cover Page
Purpose: The Bidder Response Worksheet is the primary proposal document and is used by the Proposal Review Team to evaluate responses.
Tips:
• Follow all page limits and instructions.
• Respond directly to each question.
• The contact person identified on the cover page will receive notices of award / non-award from the Procurement Unit. Please be sure their contact information is accurate.
2. Bidder Response Checklist
Purpose: Confirms that all required forms have been included in your submission.
Tips:
• Use the checklist as a final review tool before submitting.
• Missing or incomplete forms may result in a proposal being deemed non-responsive.
Non-responsive proposals will not be considered by the Review Team.
3. Supplier Diversity Plan (SDP for Goods and Services)
Purpose: The Supplier Diversity Program encourages participation by businesses certified through the Commonwealth's Supplier Diversity Office (SDO).
Why This Matters: The Supplier Diversity Plan represents 25% of the total proposal score.
Important: Only businesses certified by the Massachusetts Supplier Diversity Office count toward SDP participation.
The SDP Form asks bidders to report:
• Prior year total organizational revenue
• Prior year spending with certified diverse businesses
• Projected organizational revenue for the current year
• Planned spending with certified diverse businesses for the current year
This is one of the most common areas of confusion for bidders. The SDP Form for Goods and Services is based on organizational revenue and spending, not anticipated contract revenue.
Tips:
• Gather prior fiscal year financial information before starting the form.
• If your organization already purchases goods or services from certified diverse businesses, include those expenditures.
• If you do not currently work with certified diverse businesses, identify opportunities to do so before submitting your proposal.
• Consider common purchasing categories such as office supplies, printing, marketing, technology services, consulting services, training, facilities, and professional services.
Common SDP Mistakes
• Listing vendors that are not SDO-certified. MassAbility’s Procurement staff do check to verify that vendors included on a SDP are in fact certified. Please click HERE to access the SDO Directory of Certified Businesses.
• Leaving sections blank.
• Using estimates that cannot be supported.
• Confusing organizational spending with anticipated contract spending.
• Waiting until the last minute to identify SDP partners.

4. Department of Revenue Certificate of Good Standing
Purpose: Demonstrates that the organization is current with its Massachusetts tax obligations.
Tip:
• This certificate is ordered through the Massachusetts Department of Revenue website.
• It is not the same as a Certificate of Good Standing issued by the Secretary of the Commonwealth. MassAbility cannot accept a Certificate of Good Standing from the Secretary of the Commonwealth in place of the Department of Revenue Certificate.
• Request this document as early as possible, as processing may take several weeks.
