About this file

This is a Reference Questionnaire Form issued by Collier County's Procurement Services Division for use in evaluating vendor proposals submitted in response to the Invitation to Negotiate (ITN 26-8652) for Trap-Neuter-Vaccinate-Release (TNVR) program services. The form requires proposers to submit one completed questionnaire for each required reference, with references to be returned as part of the bid/proposal submittal package. The TNVR program represents Collier County's first formal feral cat management initiative and will involve comprehensive services including humane trapping operations, animal transportation, temporary housing coordination, veterinary surgery coordination, post-surgical monitoring, medical record retention, data reporting, and emergency disaster preparedness for free-roaming and feral cats throughout Collier County. Proposals are due March 17, 2026, at 3:00 P.M., submitted electronically through the Collier County Procurement Services Division Online Bidding System. The proposed contract term is three years with one two-year renewal option.

The Reference Questionnaire evaluates vendors across ten criteria on a scale of 1 to 10, including ability to manage project costs and schedule, quality of work, quality of consultative advice, professionalism and personnel management, project administration, communication and documentation, risk management, contract compliance, and overall customer satisfaction. Evaluators must provide a total score across all items, with blank responses scored as zero. The form requires completion of project description, completion date, project budget, and project duration fields. Reimbursement for TNVR services will be provided on a per-cat basis with tiered rates: $25.00 for healthy, sterilized, vaccinated community cats; $10.00 for cats requiring extended medical treatment or adoptable cats; and $5.00 for cats requiring humane euthanasia. No veterinary costs are borne by the vendor, as all surgical and medical services are provided at no cost by Collier County or County-approved veterinary partners.
