About this file
This is an Incident Report Form 4000 issued by the Michigan Department of Treasury, Office of Privacy and Security. The form serves as a standardized template for reporting security incidents involving sensitive information, including Treasury data, federal tax information, personal information, and confidential materials. The document is not a contract opportunity but rather an internal reporting mechanism designed to capture and track security breaches, data compromises, and related incidents within the state treasury system.
The form is divided into multiple sections requiring reporting entities to document contact information for both the reporting party and affected party, incident details such as the type of compromise (including unauthorized access, data theft, system hacking, or physical loss of documents), the date and time of occurrence and discovery, affected individuals count, and whether personal identifiable information was involved. Part 3 addresses incident resolution, requiring documentation of notifications issued to affected individuals, breach notification methods, incident costs, and post-incident recommendations. The form mandates cost tracking, including manhours, postage, credit monitoring services, and other expenses, with the requirement that incidents costing $250 or more receive detailed cost summaries. Final reports must include administrator approval and signature, with all submissions directed to the Office of Privacy and Security per Procedure PT-03253.
