About this file

NPS Form 10-354 is a Business Organization Information form used for documenting details about a corporation, limited liability company, partnership, or joint venture proposing to do business with the National Park Service. The form requires comprehensive organizational details including entity name, contact information, tax ID, state of formation, ownership structure, percentage of ownership interests, current investment value, and a list of officers, directors, or managing members.

The form is part of the National Park Service's leasing program documentation, designed to collect information to ensure business activities comply with federal laws and regulations. It supports tracking applicants and lease holders conducting business within national park units. The form is voluntary but failure to provide requested information may impede the NPS's ability to manage the leasing program, monitor resources, and provide public information about businesses operating in national parks. The estimated time to complete the form ranges from 5 to 10 hours depending on the complexity of the business organization.
