About this file
This document is a Foreign Country of Concern Attestation form (PUR 1355) for governmental entities in Florida, specifically designed to verify the ownership and business status of entities seeking to enter into, renew, or extend contracts that would provide access to personal identifying information. The form requires an authorized representative to declare that their organization is not owned by, organized under the laws of, principally based in, or controlled by a foreign government designated as a "Country of Concern".
The attestation is a compliance document that mandates transparency regarding an entity's international business connections, particularly focusing on potential national security or data privacy risks. By requiring signatories to affirm their organization's independence from foreign government influence under penalties of perjury, the form serves as a protective mechanism for governmental contract processes, ensuring that entities with potential foreign governmental ties are identified and potentially scrutinized before being granted access to sensitive personal information.
