About this file

This document is a Florida Trench Safety Act Certification and Disclosure Statement form related to a utilities contract opportunity with Pinellas County Utilities Department. The form requires bidders to acknowledge and comply with Florida's trench safety standards, which are based on federal excavation safety standards (29 C.F.R. Section 1926.650 Subpart P). Contractors must disclose their proposed trench safety measures, including specific safety techniques, units of measure, quantities, unit costs, and extended costs for trench excavation.

The form emphasizes that the disclosed costs do not fully represent the contractor's total obligation to comply with trench safety requirements. Contractors remain solely responsible for selecting appropriate safety data and implementing necessary safety systems, even if the County accepts their bid. The document allows bidders to use additional sheets to detail trench safety measures and warns that failure to complete the form may result in the bid being declared non-responsive. Bidders must provide company details, including name, contact information, and federal employee identification number when submitting the form.

The Contractor acknowledges sole responsibilities for the selection of the data on which it relies in designing the safety system, as well as for the system itself.

A. The amount disclosed as the cost of compliance with the applicable trench safety requirements does not constitute the extent of the Contractor’s obligation to comply with said standards.
