About this file
This is a First Article Test (FAT) Waiver Worksheet template designed for contractors to request waivers or partial waivers from first article testing requirements in federal contracts. The form must be submitted on company letterhead and signed by a company agent, with all submissions directed to the contracting officer.
The worksheet requires contractors to identify the current acquisition using specific data fields including the contracting officer's name, solicitation/contract number, contractor name and CAGE code, NSN, part number, nomenclature, top drawing number, and revision level. Contractors must specify whether they are requesting a full waiver (eliminating all first article testing) or a partial waiver (waiving only specific tests), with detailed reference to the Technical Data Package (TDP) specifications and drawing notes for tests to be waived. The justification section provides five primary grounds for waiver requests: (A) successful completion of a FAT on a recent contract for the same item using identical facilities, equipment, processes, and subcontractors; (B) successful FAT on a similar item tested to the same requirements with documented differences; (C) material in continuous production with documented production run length; (D) successful FAT on a next higher assembly to eliminate duplicative testing; and (E) other justifications with specific supporting documentation. Additional sections require disclosure of quality issues including Product Quality Deficiency Reports (PQDRs) and corrective actions, changes to manufacturing data or subcontractors, and any Material Review Board (MRB) actions or waiver/deviation requests. The submission section requires the signatory to provide printed name, title, company name, address, and phone number, with certification that all statements are true and correct.
