About this file
This document is a sample First Article Test (FAT) Waiver Worksheet template designed for contractors to request waivers from First Article Testing requirements in federal contracts. The form must be submitted on company letterhead and signed by an authorized company agent, with the Contracting Officer available for questions regarding submission requirements.
The worksheet is structured to capture current acquisition information including the contractor name, CAGE code, part number, NSN, solicitation/contract number, and drawing references. Contractors may request either a partial waiver (for specific tests only) or a full waiver (for all FAT requirements). The justification section allows contractors to support their waiver request through five primary pathways: successful completion of FAT on a recent contract for the same item using identical facilities and processes; successful FAT on a similar item tested to the same requirements; material in continuous production with documented approval dates; successful FAT on a next higher assembly to eliminate duplicate testing on lower assemblies; or other justifications with specific supporting documentation. The form requires contractors to disclose any quality issues, manufacturing data changes, or Material Review Board actions related to the item. Supporting documentation must be attached, and the authorized company representative must certify the accuracy of all statements provided, including printed name, title, company address, and phone number.
