About this file

This is a sample First Article Test (FAT) Waiver Worksheet template used by contractors to request waivers or partial waivers from first article testing requirements in federal contracts.

The worksheet captures current acquisition information including the contracting officer's name, solicitation/contract number, contractor name and CAGE code, NSN, part number, noun description, top drawing number, and revision level. Contractors may request either a partial waiver (specific tests only) or a full waiver (all FAT requirements). The form requires detailed justification for the waiver request, with five primary justification categories: (A) successful completion of FAT on a recent contract for the same item using identical facilities and processes; (B) successful FAT on a similar item tested to the same requirements; (C) material in continuous production with documentation of the last successful FAT approval date; (D) successful FAT on the next higher assembly to eliminate duplicate testing on lower assemblies; and (E) other justifications with specific supporting documentation. The worksheet also requires disclosure of any quality issues, PQDRs, corrective actions, manufacturing data changes, subcontractor changes, or Material Review Board actions affecting the item. All requests must be submitted on company letterhead, signed by a company agent, with attached supporting documentation as specified in the solicitation or contract. The form includes fields for contractor contact information and certification that statements are true and correct.
