About this file

This is a Financial System Risk Assessment form from the U.S. Department of Labor Employment and Training Administration (ETA) used to evaluate applicant organizations' financial management capabilities for federal grant administration.

The form requires applicants to provide comprehensive organizational information including legal name, incorporation date, principal officer details, EIN, employee count, and total sales/revenues. Organizations must demonstrate compliance with 2 CFR 200 and 2 CFR 2900 standards by documenting adequate accounting systems that: identify receipt and expenditure of funds by grant, record expenditures by project component and budget cost category, maintain time distribution records for employee effort allocation, segregate direct and indirect expenses if proposing overhead rates, and implement budgetary controls to prevent obligations exceeding available funds. The assessment also requires disclosure of any prior government reviews of accounting system adequacy, details regarding approved indirect cost rates or cost allocation plans, internal control structures, and the organization's financial stability including any legal matters or ongoing financial concerns. Applicants must provide evidence of independent CPA examination of financial statements, current CPA reports and management letters, and existing ETA Payment Management System account information. All items marked "No" or "Not Sure" in Section C require detailed written explanation on organizational letterhead signed by a principal officer to demonstrate organizational expertise in those areas.
