About this file
Summary
This is a Request for Bid (RFB-237-19) document from the Commonwealth of Kentucky, Finance and Administration Cabinet, Department for Facilities and Support Services, Division of Engineering and Contract Administration, in conjunction with the Division of Abandoned Mine Lands. The solicitation seeks sealed bids for furnishing all labor, equipment, materials, and services necessary to complete two High Priority Abandoned Mine Land (AML) reclamation projects in Pike County, Kentucky: the Mike May Slide project (approximately 1.6 acres) and the Rick Edmiston Slide project (approximately 4.9 acres, with 1.9 acres of construction area). The Mike May project involves removal of slide material, relocation of concrete barriers, subdrain installation, revegetation, and roadway repairs to protect a residential structure. The Rick Edmiston project includes excavation of slide material to bedrock, gradework on subsidence slopes, subdrain and culvert installation, structure removal and replacement, and revegetation. A mandatory pre-bid conference was scheduled for May 6, 2019, at 11:00 A.M. Eastern Time at Little Caesars Pizza in Belfry, Kentucky, which was the only time a site tour would be conducted. Sealed bids were due by May 13, 2019, at 10:30 A.M. Eastern Time at the Division of Engineering and Contract Administration, 403 Wapping Street, 2nd Floor, Frankfort, Kentucky. All work must be completed within 90 calendar days from the effective date of the award of contract. The bid process requires submission of a comprehensive bid package including qualifications, equipment lists, subcontractor schedules, material listings, bid guaranties of not less than 5 percent of the lump sum base bid, and various affidavits and certifications.
The projects are awarded on a lump sum basis to the lowest responsive bid from a responsible bidder. A 100 percent Performance Bond and separate 100 percent Payment Bond must be furnished by the successful bidder within five calendar days of receipt of the intent to award. Bidders are required to list all vendor violations issued within the last five years related to taxes, wages and hours, occupational safety, workers' compensation, and unemployment insurance, with verification through the Kentucky Labor Cabinet. Minority Business Enterprise (MBE) participation is encouraged, with bidders required to meet a 5 percent MBE goal or provide documented good faith efforts to meet this goal. The projects are federally funded under Title IV of the Surface Mining Reclamation and Control Act of 1977 (Public Law 95-87), administered by the U.S. Department of the Interior, Office of Surface Mining. Buy American Act requirements apply to all construction materials. Bidders must be registered with the Kentucky Secretary of State as being in good standing, registered with the state's eProcurement system (eMars), and compliant with all Kentucky statutes regarding campaign finance, sales and use taxes, income taxes, workers' compensation, and unemployment insurance. The contract documents include detailed specifications, standard AML technical specifications, best management practices for erosion and sediment control, and comprehensive general conditions governing construction procedures, payment, safety, insurance, bonds, completion standards, and dispute resolution.

PART I
ADVERTISEMENT FOR BIDS
1. INVITATION:
Sealed bid documents for the following work will be received by the Division of Engineering and Contract Administration, 2nd Floor Bush Building, 403 Wapping Street, Commonwealth of Kentucky, Frankfort, KY 40601, in the manner and on the date hereinafter specified for the furnishing of all labor, materials, supplies, tools, appliances, equipment, services, etc., necessary for Mike May Slide & Rick Edmiston Slide, High Priority, AML Project, Pike County, Kentucky, as set forth in the specifications and as shown on the drawings prepared by and approved by the Department for Facilities and Support Services of the Commonwealth of Kentucky and under the terms and conditions to this Request for Bid.
2. PROJECT DESCRIPTION:
Mike May Slide :
The Mike May Slide High Priority Phase II AML Reclamation Project, approximately 1.6 acres, consists of the removal of slide material and the relocation of concrete barriers that have been placed to protect the May residence. The project site is located within the Belfry Quadrangle at 37° 35’ 56' N, 82° 17’ 40' W. The May residence is located at 237 Charles Deskins Street, McAndrews, KY.
Slide material has moved and impacted the rear of the May residence. Drainage is flowing through the slide and around the residence.
The project consists of the removal of the slide material and the relocation of precast concrete barrier blocks placed to protect the home. Excess material generated during construction will be transported to an off-site waster area that has been previously used during multiple AML projects. The waste area is located at 37° 35’ 56' N, 82° 17’ 38' W. Other debris will be disposed of in accordance with AML Technical Specifications.
Construction disturbances will be kept to a minimum by the implementation of standard AML Best Management practices (BMP’s), which will prevent significant amounts of sediment related to project construction activities from entering any streams. No trees 3' in diameter or more at breast height will require removal outside of any unstable exempt slopes. Prompt re-vegetation will be implemented by using agricultural limestone, fertilizer, seed, netting and/or mulch, as needed, for the areas disturbed by the project, in accordance with standard AML design specifications.
Access to the site is via county roads and a residentially maintained driveway and yard. A construction entrance will be made from the road, avoiding the septic system to access the rear planned work area. This will be regraded and revegetated at the completion of the project. The relocated concrete barriers will remain to protect from possible future movement. Approximately one hundred and fifty (150) feet of subdrain to reach the county road and additional subdrain from there depending upon site condition. There are two options shown. OPTION A has the subdrain being directed towards an existing dropbox to the northwest approximately 180 linear feet.
Option B is to the east alongside the road and diverting to the creekbank some 380 linear feet. The ENGINEER will choose which option after determining the depth of the one subdrain after removal of material behind the residence. We expect to haul approximately 10,000 yards of material from the slide to a waste area approximately four (4) miles away on 119. We include bituminous repair to address roadway resurfacing and roadway stone to help reduce mud/debris on the road near entrance.
This project will include pre- and/or post- project maintenance and repair of existing previously constructed public county roads utilized during the construction of this project, which are administered by the county or local road authority. This construction activity will be conducted under the authority and supervision of the local public road authority and will consist of standard road construction practices appropriate to mitigate impacts to the local community from potential degraded road conditions in order to maintain public traffic safety.
Rick Edmiston Slide The project is located on Okey Smith Road in Pike County, near the Pinsonfork post office on KY 199. The project is the result of subsidence on the slope above the Edmiston home and a slide on the slope below the home. In heavy rainfall events, drainage exits through highwall behind house.
The project area is 4.9 acres and the construction area is 1.9 acres. The project consists of excavation of the slide to the underlying rock and gradework of the subsidence slope above the home. A combination rock ditch subdrain will be installed behind the house, with a culvert with headwalls carrying water to the existing road ditch.
Silt control measures at the site shall be a silt fence, installed and remaining in effect until construction is completed at the site, as well as a temporary debris barrier above the home and existing structures. Trees will be cut as part of this project in the area of the gradework, approximately 1 acre.
An onsite waste area to accommodate up to 3000 cubic yards of waste includes removal of five structures, two of which will be replaced.
Concrete and bituminous repair, including roadway stone, has been factored into the contract. Revegetation of the site, including agricultural limestone, fertilizer, mulch and seed will be done according to standard AML design specifications.
3. METHOD OF BIDDING:
Bids will be received from Prime Contractors on a Lump Sum Bid Basis for the total project. All phases of work shall be bid to and through the Prime Contracting Firms.
