This is a Statement of Work for a flooring replacement project at the VA Nebraska-Western Iowa Healthcare System's Grand Island Community Based Outpatient Clinic (Project Number 636-26-502). The contractor must remove existing carpeting and rubber cove base from specified areas on the 2nd and 3rd floors of Building 1 (located at 2201 N. Broadwell Ave, Grand Island, NE 68803) and replace them with Luxury Vinyl Plank (LVP) flooring with a woodgrain pattern and new rubber cove base (4-inch nominal height). The project scope encompasses approximately 9,300 square feet of flooring and 2,500 linear feet of cove base across third floor corridors, nurse stations, alcoves, and waiting areas, plus second floor areas including the main waiting room, Respiratory Clinic, Radiology office, and Physical Therapy areas. The contractor must provide all labor, materials, and management to complete the work within 150 calendar days following Notice to Proceed issuance. Upon project completion, the contractor must supply attic stock materials including 500 square feet of LVP with appropriate flooring adhesive and one roll of rubber cove base (minimum 120 linear feet) with cove base adhesive.
The SOW establishes extensive preconstruction, construction, and closeout requirements including submission of a Logistics Plan, Construction Security Plan, Construction Phasing Plan, Infection Control Plan, Quality Control Plan, Environmental Protection Plan, and Activity Hazard Analyses. Work must comply with VA Technical Information Library standards and includes strict Infection Control Barrier (ICB) specifications to prevent dust and debris migration in the active healthcare facility. The contractor must maintain an OSHA-certified site superintendent on-site during all work, attend biweekly construction meetings, submit weekly field reports, and obtain COR approval for all shop drawings and submittals seven days prior to construction start. Work hours are 0800-1630 Monday-Friday excluding federal holidays, with the east corridor on the third floor requiring completion between 1900-2400 hours. Third floor work in north and south corridors may be phased separately, while the Respiratory Clinic on the second floor must be completed as quickly as possible for return to full service.
