About this file

This is a Federal Tax Liability and Recent Felony Conviction certification form required for the City of Gainesville's Design and Construction Manager Services contract for electric charging stations in Florida. The certification document mandates that all contractors and subcontractors submit a signed attestation confirming they have no unpaid federal tax liabilities that have been assessed and for which all judicial and administrative remedies have been exhausted or have lapsed, and that they were not convicted of any felony criminal violation under federal law within the preceding 24 months. Failure to complete, sign, and upload this form with the bid submittal will result in automatic disqualification from the contract opportunity.

The contractor must ensure compliance flows down through all tiers of subcontractors regardless of subcontract value. The form requires completion of the contractor's company name, authorized representative title, signature, and date of certification. This certification is a mandatory prerequisite document for participation in the procurement process and must accompany all bid submissions to demonstrate the bidder's eligibility and good standing with federal tax authorities and the criminal justice system.
