About this file

This is a Federal Certifications Form required for state and local contracts utilizing federal funds, applicable to the Susanville Unified School District (SUSD) in California for a bread and bakery products procurement. The document outlines the certifications and compliance requirements that vendors must acknowledge when bidding for this contract. SUSD is seeking a qualified bread vendor to provide fresh, whole grain bread products to district schools on at least a weekly basis. All products must meet Child Nutrition Program requirements and comply fully with USDA regulations. The vendor must demonstrate consistency, strong food safety practices, and the ability to meet the scale and distribution needs of a large school district. The form does not specify quantities, response dates, due dates, site visits, bidder meetings, award dates, or contract terms.

The Federal Certifications Form requires vendor compliance with multiple federal regulations when federal funds are expended, including provisions for contracts exceeding the simplified acquisition threshold of $150,000 regarding remedies for breach of contract, termination for cause and convenience for contracts exceeding $10,000, compliance with the Clean Air Act and Federal Water Pollution Control Act for contracts exceeding $150,000, debarment and suspension requirements under Executive Orders 12549 and 12689, and the Byrd Anti-Lobbying Amendment for contracts exceeding $100,000. The vendor must certify non-use of federal funds for lobbying activities and disclose any non-federal lobbying activities. The form requires the authorized representative's initials affirming agreement to all stated federal requirements and provisions.
