About this file

This document is a submission requirements checklist for a federal contract proposal using SF 330 forms.

Respondents must submit a cover letter (1 page) containing point of contact information, CAGE code, and business size for relevant NAICS codes, along with the completed checklist. The proposal must include a table of contents with separated sections using tabs and company name visibility. SF 330 Part I requires Contract Specific Qualifications (3 pages maximum) listing CAGE codes for all prime and subcontractors, followed by an Organization Chart of the Proposed Team (1 page attachment). Key Personnel resumes are limited to one page per resume with one resume per key discipline. Example Projects are capped at 10 projects maximum, including subcontractor and joint venture projects (10 pages), with Key Personnel Participation documentation (1 page) and Past Performance Information (10 reference pages). SF 330 Part II General Qualifications forms are required for each office performing services, whether prime or subcontractor entities (1 page each). Additionally, Past Performance Questionnaires submitted by customers must be included (1 per project), with a checkbox to indicate if any customers provided questionnaires. All materials should be organized sequentially behind the cover letter with clear tabs identifying each section.
