SCHEDULE OF CHANGES
FA9101-10-B-0007 0002
The purpose of this Amendment is to:
1) Provide additional information:
Add note to drawing VYT34152.E5: The contractor shall install a single duplex electrical receptacle (120 volt power; circuit L3-9) to the communications cabinet located in storage room 220.
Q: How do we address the case of crowded LCDs?
A: Please include 12 extra 2-axis arms
Q: A-1 Note 6: How many pieces of furniture do we have to move?
A: 13 pieces plus 3 wall mounted white boards 1 book shelf.
Q: What wage rate do we use for asbestos abatement since it is not included in posted wage determination?
A: There is no wage classification for asbestos worker and DOL will not issue wage conformance statements until after contract award and contractor request of conformance on
SF 1444. The prevailing wage rate for asbestos worker included in TN080014 has a rate of
$21.07 and fringe benefit rate of $10.22. This rate should be used when preparing your bid.
If it is later determined by DOL that a higher rate should be used, guidance will be provided by the Contracting Officer.
2) Provide the following questions and answers for clarification purposes:
Q: Please identify placement of CPU Caddies that cannot be seen in drawings for Project C (2 of 3 not visible).
A: Include one CPU caddy on the left side of the left work surface and include one CPU caddy on the right side of the far right work surface.
Q: Project C shows some rear panels, does this use the same instruction for this area as was given for Project A?
A: Correct
Q: Which roof company handles the roof warranty for Building 676?
A: The roof is under warranty with the manufacturer, Duro-Last Roofing, Inc
Q: Drawing A6, Detail - Sink Casework for Toilet Room: The detail calls for P. Lam. tops and solid surface backsplash. Is this correct?
A: The break room, kitchenette, and toilet vanity are all plastic laminate with laminate four inch high side and backsplashes. The kitchen and kitchenette have glass tile side and backsplashes above the laminate side and backsplashes to the underside of the upper cabinets. We do not have any corian on this job.
Q: Does the base have a fire hydrant flow test closest to the tie in point so we can conduct temporary calculations on the new system. I realize if awarded the project we would schedule a flow test for our design documents.
A: 56 psi is max pressure
Q: Addendum #1 is not clear on who supplies the LCD mounts? Are the mounts owner supplied contractor installed?
A: the contractor provides the mounts with the furniture.
Q: Page C4 Shows pavement repair detail with 8'' of concrete under 1 1/2" pavement. Does existing pavement have concrete under it?
A: Yes. See note, "EXISTING CONCRETE OVERLAIN WITH ASPHALT", on Sheet C2.
Q: Page M6, What does note at top of plans (High Bay 51'- 0" above grade) mean?
A: This note is for information only to advise how tall the high bay area is above the roof to be worked on. No work takes place on the high bay roof although, only on the second floor roof as shown.
Q: Who is the Obligee you want on the Bid Bond?
A: Please use SF24 for bid bonds, the United States of America (Government) is the obligee
Q: Traditionally, on military bases we have not had to pull a typical building permit from the city/county because work is on govt property. But in the specification there is a reference to a
"Master Work Permit" that we are responsible for. Is this similar to a building permit? Is there a contact there on base that I should be dealing with?
A: You will work with ATA the base support contractor to obtain these permits, no cost
Q: Regarding Utilities... We have to bring water into this building. Are water lines on base property of Arnold AFB, or should I be dealing with an outside utility company?
A: Water lines are property of Arnold AFB
Q: Voluntary Protection Program (VPP) - if I understand this right we are to enroll (or apply for) this project to be in the VPP program. This appears to be similar to a OSHA Voluntary
Compliance Program, which is a pretty straightforward program . I have reviewed the required information that is to be submitted, but am not familiar with this particular program. Can you confirm that this is something that is an administrative procedure only, and that there is no additional cost to the contractors for the VPP program.
A: The AF goal is to make VPP applicable to all contractors performing work on AF installations. Contractors working on AF installations are not required to obtain VPP certification, however, they will be held to the higher safety and health of the AF installation.
IAW SHE Standard A6 Safety plans and/or GC-1792, AEDC Contractor/Subcontractor Pre-Job
Checklist will be required prior to beginning work.
Q: Is there a Fireline Tap Fee? If so, who do we talk to about that?
A: No
Q: The Door Schedule and the notes on that page of the drawings overlap and are not clear on the new and existing doors/frames. Can you clarify what the architect would like in this matter?
A: All the existing door frames to remain are listed in note #8, Door Notes on sheet A5. The door schedule shows which doors are to remain and which are to be new (not all new doors get new frames, some use existing.
Q: Can you provide us with a detail on how to penetrate the concrete decking for the HVAC ducts?
A: The roof decking is approximately 4" thick concrete and is to be penetrated per specification section 01 73 29. "CUTTING AND PATCHING" (specifically paragraph 3.3). Also, see Note
#2, sheet M6, which states "Provide new openings in roof and new roof curbs per AC unit manufacturer's installation instructions."
Q: Who is the Duro-Last roofing contractor for the ABC Operations Center Building?
A: See Note #1, sheet M6, which states "Installation of rooftop units and openings to be performed by an authorized Duro-Last contractor."
Q: Is there a special Bid Form that we have to use when we turn our bid in?
A: Please see page 2 of the solicitation.
