MEMORANDUM

Date: 19 November 2008 Organization Mail Stop

To: Mark Gifford, Project Manager ID20 1400

From: Bill Sizemore, SHG SH11 7000

Subject: Asbestos Survey: APTU High Bay Lighting System Wiring

File No: SH-09-029 Ref: CDRL OT-2003-30089

At the request of Mark Gifford, Bill Sizemore of the Industrial Hygiene department performed an asbestos survey on the wiring supplying the lighting in the high bay area of APTU, Building 579 on 19 November 2008.

Incandescent and mercury vapor light fixtures, transformers, conduits, and associated junction boxes were visually checked during the survey.

Findings/Recommendations:

1. The incandescent and mercury vapor light fixtures contained only vinyl insulated wiring.

2. Conduit and junction boxes contained only vinyl insulated wiring.

3. The transformers have vinyl insulated wiring entering and exiting the units, but the interior contains several white fibrous coated wires which are to be treated as asbestos. Many, but not all of the transformers have been bypassed to convert the mercury vapor fixtures to incandescent use. If the transformers are to be removed as a complete unit, the wiring may be cut on either side and contact with the asbestos wiring will not be necessary.

The above components were randomly inspected to provide a representative sample of the composition of the lighting system. If, during actual replacement of the lighting system, suspect materials are discovered, please contact Bill Sizemore at Ext. 5538 for additional support.
