L-1 PROPOSAL PREPARATION INSTRUCTIONS

A. GENERAL INFORMATION

1. This acquisition is being conducted as a 100% 8(a) Small Business set-aside under the North

American Industry Classification System (NAICS) 541511. The size standard for this NAICS is

$27.5 Million.

2. Points of Contact (POC). The Contracting Officer (CO) and/or Contract Specialist (CS) are the only POCs for questions or concerns regarding this acquisition. Address any questions or concerns to the CO specified in Blocks 10a through 10c of the SF 33.

3. Debriefings. The CO will promptly notify offerors of any decision to exclude them from the competitive range; whereupon, they may request and receive a debriefing in accordance with

(IAW) FAR 15.505. If a competitive range is established, offerors excluded from the competitive range may request a pre-award debriefing or they may choose to wait until after the source selection decision to request a post-award debriefing. However, offerors excluded from the competitive range are entitled to no more than one debriefing. The CO will notify unsuccessful offerors in the competitive range of the source selection decision IAW FAR 15.503.

Upon such notification, unsuccessful offerors that requested a post-award debriefing may request and receive a debriefing. Offerors desiring a debriefing must make their request IAW the requirements of FAR 15.505 or 15.506, as applicable.

4. Discrepancies. If an offeror believes the requirements in these instructions contain an error, omission, or are otherwise unsound, the offeror shall immediately notify the CO and CS in writing with supporting rationale as well as the remedies the offeror is asking the CO to consider as related to the omission or error. The offeror is reminded that the Government reserves the right to award this effort based on the initial proposal, as received, without discussion so it is the responsibility of the offeror to ensure the initial proposal conforms to the solicitation requirements.

5. Electronic Reference Documents. All referenced documents for this solicitation are available on the Federal Business Opportunities (FedBizOpps) web site at https://www.fbo.gov/.

Potential offerors are encouraged to subscribe for real-time e-mail notifications when information has been posted to the website for this solicitation.

6. Communications. The CO will control exchanges of source selection information between

Government and offerors. The Government may use email to transmit such information to offerors and will include “Source Selection Information – See FAR 2.101 & FAR 3.104” in the

Subject line.

7. Questions: All questions related to this solicitation must be submitted to Ms. Kirsten

Hawley, CO (kirsten.hawley.1@us.af.mil) and Mr. Jack Murray, CS (jackie.murray@us.af.mil), no later than 10 calendar days after the solicitation is released. Late requests may not be considered.

8. Reading Library Instructions (Attachment 12) - PENDING (Will be addressed prior to release of the official RFP). A Reading Library containing a list of Exhibits has been established at Joint

Base San Antonio (JBSA) - Lackland. The single POC for access is Ms. Kirsten Hawley, CO

((210) 977-5682 or kirsten.hawley.1@us.af.mil). Reservations shall be made in advance not later than 3:00 PM (CST) on DD MMM YY. The Library will be open DD MMM YY – DD MMM YY from 8:00 AM (CST) to 12:00 PM (CST) and 1:00 PM (CST) to 4:00 PM (CST).

B. GENERAL INSTRUCTIONS

1. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, technical requirements, and Contract Security

Classification Specification, in addition to those identified as evaluation factors or subfactors.

2. This section of the RFP provides general guidance for preparing proposals as well as specific instructions on the format and content of the proposal. The offeror's proposal must include all data and information requested by the RFP and must be submitted IAW these instructions. The offer shall be compliant with the requirements as stated in the Performance

Work Statement (PWS) and RFP. Non-conformance with the instructions provided in the RFP may result in an unfavorable proposal evaluation. Alternate proposals will not be accepted.

3. Offeror’s proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of stated claims. The proposal should not simply rephrase or restate the Government's requirements but rather shall provide convincing rationale to address how the offeror intends to meet these requirements. Offerors shall assume that the

Government has no prior knowledge of their facilities and experience and will base its technical evaluation on the information presented solely in the offeror's proposal.

4. Elaborate brochures or documentation, binding, detailed art work, or other embellishments are unnecessary and are not desired.

5. The proposal acceptance period is specified in SF 33, Section A, Block 12 of the solicitation.

6. IAW FAR Subpart 4.8 (Government Contract Files), the Government will retain one copy of all unsuccessful proposals. After contract award, the Government will destroy extra copies of any unsuccessful proposals.

C. ORGANIZATION.

1. Number of Copies/Page Limitations. The offeror shall prepare the proposal as set forth in the Proposal and Binder Organization Tables (Tables 1 and 2 below). The titles and content of each part, all of which shall be within the required page limits, are defined in Table 1, and the number of copies is described in Table 2. Page limitations shall be treated as maximums. If exceeded, the excess pages will not be read or considered in the evaluation of the proposal.

Proposal change pages are customarily subject to the same page limitations stated in Table 1 below; however, deviated page limitations may be required on responses to Evaluation Notices

(ENs). Each page shall be counted except cover pages, table of contents, OCO / DCO & RTO&I

SHELTER, labor matrix, position descriptions and glossary. The cover page shall include

Offeror name, DUNS, Cage Code, POC’s name, phone number, email, and physical address.

2. The response shall consist of four (4) separate parts: Part I – Technical Proposal, Part II –

Present/Past Performance Information (PPI), Part III – Price, and Part IV – Contract

Documentation. The content of each proposal binder is described in detail as follows:

*Original and Copy 1 binders contain the complete proposal package. Reproduction of the

Government RFP is unnecessary (and unwanted). Original proposal package shall include two (2) recordable, virus-free data compact disk (CD) which contains all parts (Parts I, II, III, and IV). CD must be secured inside CD/DVD binder storage pages (3-hole type) and secured into binder rings at the front of the Original” binder.

**The OCO / DCO & RTO&I SHELTER Labor Matrix (Attachment 8) will not be included in the page count and shall be submitted via a CD (2 each). This should be the only information contained on this CD.

3. Page Size and Format.

a. A page is defined as each face of a sheet of paper containing information. When both sides of a sheet display printed material, it shall be counted as two pages. Page size shall be

8.5 x 11 inches, not including foldouts. Pages may be single-spaced but shall have at least double spacing between paragraphs and factors or sub-factors. Except for the reproduced sections of the solicitation document, Task Order Pricing Table (Attachment 11), and items specified in paragraph (b) below, the text size shall be no less than 12 point, Times New Roman, Arial or Courier font type. Tracking, kerning, and leading values shall not be changed from the default values of the word processing or page layout software. Use at least one-inch margins on all sides. Pages shall be numbered sequentially by part. These limitations shall apply to both electronic and hard copy proposals.

b. Legible tables, charts, graphs and figures may be used wherever practical to depict organizations, systems and layout, implementation schedules, plans, etc. These displays shall be uncomplicated, legible, and shall not exceed 11 by 17 inches in size. Foldout pages shall fold entirely within the part, and shall count as two pages. Foldout pages may only be used for large tables, charts, graphs, diagrams and schematics and not for pages of text. For tables, charts, graphs and figures, the text shall be no smaller than eight-point. These limitations shall apply to both electronic and hard copy proposals. For the OCO / DCO & RTO&I SHELTER Labor

Matrix refer to the instructions in paragraph C.2 above.

4. Cross Referencing. The offeror shall provide a cross-reference matrix, which does not count against the page count, at the beginning of Part I - technical volume indicating the PWS paragraph number and the corresponding proposal paragraph. To the greatest extent possible, each part shall be written on a stand-alone basis so that its contents may be evaluated with a minimum of cross referencing to other parts of the proposal. Cross referencing within a proposal part is permitted where its use would conserve space without impairing clarity. Information required for proposal evaluation which is not found in its designated part will be assumed to have been omitted from the proposal.

5. Indexing. Tab indexing shall be used to identify parts.

6. Glossary of Abbreviations and Acronyms. Each part shall contain a glossary of all abbreviations and acronyms used, and with an explanation for each. Glossaries do not count against the page limitations for their respective parts.

7. Binding and Labeling. The proposal shall be bound in three-ring, loose leaf binders permitting the binders to lie flat when open. Staples shall not be used. A cover sheet shall be bound in each binder, clearly marked with the title, copy number, solicitation identification, and the offeror’s name. The same identifying data shall be placed on the spine of each binder.

8. Electronic Documents. The content and page size of electronic documents must be identical to the hard copies. Label the CD with the RFP number, title, and offeror’s name. Within the

CD, use a separate electronic file for each part with associated exhibit, annexes, and attachments to permit rapid search and location. Files shall not be compressed. Submit two (2) electronic proposal documents in a format readable by Microsoft (MS) Office Word 2010, MS Office Excel

2010, and MS Office Power Point 2010, as applicable. In the event of discrepancy(s) between the hard copies and electronic copies of a proposal, the hard copies will be used for evaluation.

D. PART I – TECHNICAL PROPOSAL.

1. The Technical Part should be specific and complete. Legibility, clarity, and coherence are very important. Offeror responses will be evaluated against the Technical subfactors defined in

M-1 Evaluation of Offers, Factor 1-Technical Acceptability (Attachment 4). Using the instructions provided below, provide the actual methodology the offeror will use to accomplish/satisfy these subfactors. All the requirements specified in the solicitation are mandatory. By submitting its proposal, the offeror represents that its firm will perform all the requirements specified in the solicitation. It is not necessary or desirable for the offeror to tell the Government so in its proposal. Do not merely reiterate the objectives or reformulate the requirements specified in the solicitation.

2. Identify submitted material by corresponding factor, subfactor, and paragraph/element herein.

3. Format and Specific Content. Offeror shall address its proposed approach to meet the minimum performance or capability requirements of each Technical Proposal subfactor as follows:

a. Subfactor 1: Vulnerability Identification. The offeror shall provide its approach to the following scenario that demonstrates vulnerability identification that reflects an ability to meet

PWS sub-paragraphs 3.2.1, 3.3.1, 3.4.1, 3.5.1 requirements:

Scenario 1: A team of developers has been tasked with analysis of a piece of malware discovered on an unknown Air Force information system. During the time of collection, there were a number of observed anomalies on the affected network: some users were unable to deliver IP network traffic to their default IP gateways, other users were able to log in, but unable to access shared drives, and yet other users were able to use all network services, but web browsing was occurring at an unusually slow rate with no sites being blocked. The team is to determine possible implications associated with the malware being on an operational system, its behavior if left unchecked, and a method to remove the malware.

b. Subfactor 2: Software Development. The offeror shall provide in its response to the following scenarios its approach to supporting software development that meets PWS sub-paragraphs 3.1.1, 3.6.1, 3.9.1 requirements.

Scenario 2: The development team is tasked with creating a distributed signage application. The public affairs office needs to drive a number of small displays outside of buildings with text displayed on LCD displays. Text is entered at a web-based central control point and then distributed via an Air Force network to displays powered each by a raspberry pi. In order to increase complexity for this scenario, it is necessary to use the GPIO pins on the raspberry pi to drive an I C LCD screen and the code running the LCD screen must be written in assembly as a loadable kernel module.

c. Subfactor 3: Staffing/Management. The offeror shall present a staffing and management approach that explains how it will meet the PWS requirements and Appendices. The approach shall not include employee names or resumes. The approach shall provide the following:

(1). Completed OCO / DCO & RTO&I SHELTER Labor Matrix identifying proposed labor categories, education / training, skill types / levels, technical certifications, quantity of personnel by task, security clearance by labor category. The labor matrix shall correlate directly with PWS tasks / requirements and position descriptions.

(2). Position descriptions for each labor category. The position descriptions shall correlate directly with the labor matrix and PWS tasks / requirements. No more than one page each and will not be included in the technical proposal page limitations.

(3). Methods of recruiting, accession, retention of qualified personnel, training, and obtaining and maintaining personnel clearances and certifications.

(4). Approach for ensuring continuity of services during personnel absences due to sickness, leave and vacancies such that impact to the Government is minimal.

(5). Managing processes to ensure personnel maintain ability to operate existing and evolving network resources, topology, processes and procedures.

(6). Organizational Staffing Chart for managing and staffing (including program management, on-site leads and off-site leads) the overall contract and each individual Task Order (TO).

Organizational staffing chart shall delineate tasks performed by other than the offeror’s (i.e.

subcontractor or teaming partners).

(7). Management processes for operating in classified environments.

E. PART II –PAST PERFORMANCE INFORMATION.

1. General. Each offeror shall submit Past Performance Information no later than 1:00 PM

(CST) DD MMM YY IAW the format contained in the Past Performance Questionnaires (PPQ)

(Attachment 6) of the RFP. This information is required on the offeror and all subcontractors, teaming partners, and/or joint venture partners. It shall include a cross-reference matrix identifying the PWS references and the team member(s) proposed to perform that function. Offerors are cautioned the Government will use data provided by each offeror in this volume and data obtained from other sources in the evaluation of past performance. Past performance of either party in a joint venture counts for the past performance of the entity.

2. New Corporate Entities. New corporate entities may submit data on prior contracts involving its officers, employees, and key personnel who have relevant experience and/or data of efforts that will perform major or critical aspects of the requirement. Discuss the roles and responsibilities of personnel involved in the prior contracts cited.

3. Reference Data. The Government reserves the right to contact and use information provided by the references and any other sources. The Government may also obtain and consider information about other contracts not mentioned in the offeror’s proposal, but which the

Government determines to be similar to the proposed effort. It is the offeror’s responsibility to ensure all reference data is current and correct so that the Government can properly evaluate PPI.

4. Recency/Relevancy. Offeror shall submit PPI on no more than six recent contracts the offeror considers most relevant in demonstrating its ability to perform the proposed effort. The total includes any subcontractors or teaming partners; however, no more than three of the six recent contracts shall be submitted on the teaming partners and/or significant subcontractors that the offeror considers most relevant in demonstrating their ability to perform the proposed effort. Do not duplicate a contract reference for prime and subcontractor, but rather explain the role each played within the contract reference itself. For a description of the characteristics or aspects the

Government will consider in determining how contracts are considered recent and relevant, see

Shelter Relevancy Assessment (Attachment 5).

5. Quality. Past Performance Information Retrieval System (PPIRS), Contractor Performance

Assessment Reporting System (CPARS) information and PPQs will be used in evaluation of recent and relevant past performance. Offeror may provide any additional information to explain corrective actions taken in the past, if any, for substandard performance and any current performance problems such as cost overruns, extended performance periods, numerous warranty calls, etc.

6. Past Performance Questionnaire.

a. The offeror shall send out the Past Performance Questionnaires to each of the offeror’s subcontractors’, teaming contractors’ and/or joint venture partners’ (i.e., each entity’s). The responsibility to send out and track the receipt and submission of the PPQs rests solely with the offeror. For each PPQ, prepare, complete and sign a PPQ Cover Letter containing substantially the same information as stated below. Cover letters should be printed on company letterhead.

The offeror shall exert its best effort to ensure that a current POC, per relevant contract, submit a completed PPQ directly to the Government not later than the due date for the past performance proposal volume. Each of the offeror’s POCs shall e-mail the completed PPQ directly to Ms. Kirsten Hawley, CO (kirsten.hawley.1@us.af.mil) and Mr. Jack Murray, CS

(jackie.murray@us.af.mil). Mailing the PPQs is an acceptable alternative method of transmission.

b. Once the PPQs are completed by the POC, the information contained therein shall be considered source selection sensitive and shall not be released to the offeror. Questionnaires shall be sent to directly to the Government from at least one of the following:

Program / Project Manager or equivalent, CO / Contract Administrator (CA) or equivalent, or CO Representative (COR).

7. Organizational Structure Change History. Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a "roadmap" describing all such changes in the organization of the company. As part of this explanation, show how these changes impact the relevance of any efforts the offeror identifies for past performance evaluation/performance confidence assessment. Since the

Government intends to consider PPI provided by other sources as well as that provided by the offeror(s), the "roadmap" should be both specifically applicable to the efforts identified and general enough to apply to efforts on which the Government receives information from other sources.

8. Consent Letters. The offeror shall submit a consent letter, executed by any potential subcontractors, teaming partners, and/or joint venture partners. The consent letter authorizes release of adverse PPI to the offeror so the offeror can, in turn, respond to such information. A sample letter follows:

9. Client Authorization Letters. PPI concerning private sector contractors, subcontractors and joint venture partners cannot be disclosed to the Government without their consent. Client authorization letters are required for each identified effort for a commercial customer. This letter will authorize release to the Government of requested information on the offeror’s performance.

The Government will need the consent before contacting commercial customers to assess the offeror’s past performance. In an effort to assist the Source Selection Evaluation Board (SSEB) in assessing offeror’s past performance relevancy and confidence, the Government requests the following client authorization letter be completed for any commercial customers identified in offeror’s proposal. Offeror shall include the completed client authorization letter(s) within the

Past Performance Part II.

In their acquisitions, the Government is placing increased emphasis on past performance as a source selection factor. The Government requires entities responding to their solicitation to identify clients and requests client participation in the evaluation process. F. PART III – PRICE PROPOSAL.

1. Volume Organization. Price Proposal shall consist of the following:

Section 1:

a. Table of Contents

b. Pricing Notes/Methodology: Provide data Other Than Certified Cost or Pricing information and supporting data for fully burdened labor rates, to include estimating methodology. Offerors are expected to use their customary formats and calculations consistent with their normal accounting practices to develop the fully burdened labor rates. To support reasonableness, describe the basis of estimate for the proposed direct and indirect rates and factors and provide narrative support for all elements. The Professional Compensation Plan is to be included here.

c. Pricing Assumptions and Exceptions: Offerors shall identify any assumptions and exceptions taken to the pricing requirements stated in this solicitation. Provide complete rationale for each assumptions and exception. Do not cross reference to/from Part IV, Contracting

Documentation. Annotate “none,” if no assumptions or exceptions apply. Information provided for exceptions and assumptions will assist the price evaluation team in determining price reasonableness, realism, balance, completeness and affordability.

Section 2:

a. Labor Rate Tables (Attachment 9) and Pricing Model (Attachment 10): The Labor Rate

Tables and Pricing Model shall be submitted in Microsoft Excel format (no PDF). Do not password protect any Microsoft Excel spreadsheets.

Note: Offerors shall follow the “Instructions for filling in the Labor Rate Tables and Pricing

Model worksheets” as provided in the Tab denoted as “Instructions” in the Pricing Model.

b. Task Order Pricing Table: Offerors shall follow the instructions at paragraph F.5 below.

2. GENERAL INSTRUCTIONS:

a. This section is to assist the Offeror in submitting data other than certified cost or pricing data that is required to evaluate the reasonableness, realism, balance, completeness and affordability of the Offeror’s proposed cost/price. Compliance with these instructions is mandatory and failure to comply may render your proposal ineligible for award. Offers shall be sufficiently detailed to demonstrate price reasonableness, price realism, balance, completeness, and affordability. Offerors shall also provide supporting data for the basis of proposed prices.

The burden of proof for credibility of proposed prices rests with the Offeror at the time of proposal or during subsequent discussions, if any are held. An Offeror’s Total Evaluated Price

(TEP) is defined as the pricing model’s TEP plus the total proposed price for the Task Order

(TO) (Task Order Pricing Table).

Offers must use pricing from the Labor Rate Tables when developing the Task Order Pricing Table for the TO.

b. Cost/Price Data: Data beyond that required by this instruction shall not be submitted, unless you consider it essential to document or support your cost/price position. All data relating to the proposed price, including all required supporting documentation, must be included in the section of the proposal designated as the Cost/Price volume. Under no circumstances shall this data and documentation be included elsewhere in the proposal.

c. The Government has determined there is a high probability of adequate price competition in this acquisition. However, in accordance with FAR 15.403-1(b) and 15.403-3(a), data other than certified cost or pricing data may be required to support a determination of price reasonableness and realism. If, after receipt of proposals, the CO determines that there is insufficient data available to determine price reasonableness and realism and none of the exceptions in FAR 15.403-1 apply, the Offeror shall be required to submit additional cost or pricing data.

d. Rounding: All dollar amounts (unit and extended prices and other price factors) provided shall be rounded to the nearest penny (two decimal places only). This includes base period and out- year (option) prices. Rounding errors caused by Government formulas will be corrected during evaluation to round to the nearest penny.

3. Labor Rate Table.

a. The Labor Rate Tables are provided for use in proposing fully burdened Firm Fixed Price

(FFP) direct labor rates. The Offeror shall complete and submit Labor Rate Tables for the Prime and all teaming partners for the entire contract periods using the excel spreadsheets.

b. Spreadsheet columns that require data inputs are blue color coded. A blank column has been included in the template to accommodate any labor category titles that are different than specified in the RFP. This is only a title difference; the personnel proposed must still meet the qualifications requirements as specified per the Government’s Personnel Qualifications and

Experience Requirements.

c. Failure to propose rates for all labor categories will render the proposal incomplete, and the entire proposal will be subject to rejection.

d. For evaluation purposes and establishment of Fully Burdened FFP Labor Rates, Offerors are required to submit only FFP pricing. Cost Plus Fixed Fee (CPFF) Contract Line Item

Numbers (CLINs) will be established in the basic contract and cost/price analysis will be conducted as necessary at the TO level.

FULLY BURDENED FFP LABOR RATES:

The following provides guidelines for the major cost/price elements normally associated with the buildup of fully burdened labor rates and required support documentation:

Base Labor Rates: Show base labor rates associated with each labor category. Base rates are the hourly rates that the employees will be paid for performance under the basic contract. The most common basis for a proposed base rate is the actual rate the employee is currently being paid. If an individual in a particular labor category is currently not employed, a verifiable method used to establish the reasonableness of proposed rate must be provided, e.g. Payrolls, Salary.com, quotes, etc.

Base Labor Rate Escalation: Current rates will require adjustment for annual cost of living increases. Base rates for contract out-years will require escalation adjustments to reflect what employees will receive for those periods.

Indirect Rates: Common indirect rates are Labor Overhead, Fringe Benefits, General &

Administrative (G&A), and Facilities Capital Cost of Money (FCCOM). To support reasonableness, describe the basis of estimate for the proposed direct and indirect rates and factors and provide narrative support for all elements.

4. Pricing Model.

To complete the Pricing Model use the instructions on the first page of the Pricing Model. If the prime contractor has a teaming partner and elects to propose only one set of rates for the entire team, this must be stated in the Volume IV narrative. If the Offeror’s labor category definitions do not directly correspond to the Government’s labor category definitions/qualifications, the

Offeror may propose its alternative; however, the Offeror also must provide a cross-reference and explain the changes as an exception.

These fully burdened labor rates shall be submitted for each of the fiscal years contained in the

Pricing Model. Each Fiscal Year (FY) is the same as the Government’s Fiscal Year: 1 October through 30 September.

The labor categories identified in the Labor Rate Tables and corresponding Pricing Model are anticipated to be all inclusive of the labor categories expected to be used throughout the life of the contract. All labor categories identified in the Pricing Model shall be covered and proposed collectively by the prime and/or teaming partners for Fully Burdened FFP labor Home Office and Field rates. These rates may be proposed by the prime, teaming partner(s), or a blend. The

Offeror shall ensure that all labor categories are covered but NOT all labor categories must be covered by any one business entity. If an Offeror’s labor category nomenclature differs from the

Government’s, the Offeror must provide a cross-reference in support of meeting the

Government’s minimum labor qualification for that specific category, i.e. describe qualification for the skill level applicable to the labor category in terms of education level, experience and typical skills/capabilities/job functions.

The rates as proposed in the Pricing Model shall be contractually binding rates should

Offeror be awarded a contract. The rates shall be utilized for future task orders and task order modifications; however, discounted or lower rates may be proposed.

5. Task Order Pricing Table.

a. The Offeror shall comply with the following requirements and instructions:

(1) Offerors shall use proposed pricing from the Labor Rate Tables in the TO.

(2) Price all CLINs. Limit unit prices to two decimal places. The abbreviations Not

Separately Priced (NSP) and No Charge (N/C) are not acceptable entries. If the Offeror is providing a CLIN to the Government at no additional charge, “$0.00” must be entered for that item. The dollar values of the Not to Exceed Cost CLINs: 0008, 0009, 0108, 0109, 0208, 0209, 0308, 0309, 0408, 0409, 0508 and 0509, are established by the Government and shall not be altered by the Offeror. Option to Extend Services, CLINs 0502-0509 will be automatically populated with established formulas on the Task Order Pricing Table. The number of FTEs is for informational purposes only; monthly price is calculated by multiplying the number of months by the unit price. Unit price is the total price for the applicable mission area.

(3) Electronic Media Submission of Task Order Pricing Table. The CLINs in the Task

Order Pricing Table of this solicitation are an electronic file, presented as Excel spreadsheet/ workbook format. Submit both a hard copy and an electronic copy using the exact file provided by the Government without changing the format of the files.

b. Description of Files:

(1) Summary Sheet: Contains cells for the entry of unit prices for CLINS 0001 –

0009, 0102 – 0109, 0202 – 0209, 0302 – 0309, and 0402 – 0409, and calculates extended prices for the five (5) year period and the Option to Extend Services (CLINS 0502 – 0509).

(2) Option to Extend Services, CLINs 0502-0509, will be automatically populated

IAW clause 52.217-8. While it is not the Government’s intent to utilize these option CLINs, they are included to facilitate the extension of services if it is deemed necessary.

6. Affordability: The Government will evaluate whether each offeror’s price proposal is affordable by comparing the total proposed to the budgetary information included below.

The evaluation shall be made on the basis of a separate comparison for each fiscal year of the contract as well as a comparison between the total price and the total budgetary information included in the solicitation.

7. Professional Compensation Plan: As part of the price proposal, Offerors shall provide a total compensation plan setting forth salaries and fringe benefits proposed for the professional employees who will work under the contract FAR 52.222-46 -- Evaluation of

Compensation for Professional Employees.

G. PART IV - CONTRACT DOCUMENT SECTION

1. Contract Documentation. The purpose of this part is to provide information to the

Government for preparing the contract document and supporting file. The offeror's proposal shall include a signed copy of the SF 33 and SF 30 for any subsequent amendments issued. The offeror shall complete and submit “ONLY” the contract document sections as follows:

Section A – Solicitation/Contract Form: Complete Blocks 12 through 18 (including original signature and date) of the SF 33. Signature by the offeror on the SF 33 constitutes an offer, of which the Government may accept. The "original" copy should be clearly marked under separate cover and should be provided without any punched holes. Instead, secure the original signed SF 33 in a document protector bound in the “original proposal” binder and one copy in the “Copy 1” binder.

Offeror shall include a statement within this part that the proposal is valid through the proposal acceptance period specified in Section A.

Section B – Supplies or Services and Costs/Prices: Do not include. This section will be populated by the Government at contract award from data provided on the Task Order Pricing

Table.

Section C – Description / Specifications / Work Statement (Not Applicable).

Section D – Packaging and Marking (Not Applicable).

Section E – Inspection and Acceptance: Do not include. This section is not modifiable by the offeror and is redundant to include with the proposal submission.

Section F – Deliveries or Performance: Do not include. This section is not modifiable by the offeror and is redundant to include with the proposal submission.

Section G – Paragraph G-1. Contract Administrative Data: Offeror shall provide a “remit-to” address if different from the mailing address in Block 9 on the SF 33. The Remit-to address will be included in Section G of any resultant contract at the time of award. Do not include

Section G if “remit-to” address does not apply.

Section H – Special Contract Requirements (Not Applicable).

Section I – Contract Clauses. Submit pages for only clauses which require offeror fill-ins:

52.219-28(g). Ensure the submissions are from the RFP itself, which includes the RFP and page numbers. Other pages in this section shall not be included with the submission.

Section J – List of Documents, Exhibits and Other Attachments:

Attachment 2 – DoD Contract Security Classification Specification (DD Form 254). To be considered for award, offerors must comply with all security requirements specified in the DD

Form 254, Contract Security Classification Specification included in this solicitation, complete

Blocks 6.a., b. and c. thereof, and return the completed DD Form 254, Contract Security

Classification Specification.

Attachment 6 – Past Performance Questionnaire. To be considered for award, offerors must comply with this requirement IAW L-1 Proposal Preparation Instruction and M-1

Evaluation of Offers.

Attachment 8 – Labor Matrix. To be considered for award, offerors must comply with this requirement IAW L-1 Proposal Preparation Instruction and M-1 Evaluation of Offers.

Attachment 9 – Labor Rate Tables. To be considered for award, offerors must comply with this requirement IAW L-1 Proposal Preparation Instruction and M-1 Evaluation of Offers.

Attachment 10 – Pricing Model. To be considered for award, offerors must comply with this requirement IAW L-1 Proposal Preparation Instruction and M-1 Evaluation of Offers.

Attachment 11 – Task Order Pricing Table. To be considered for award, offerors must comply with this requirement IAW L-1 Proposal Preparation Instruction and M-1 Evaluation of

Offers.

Attachment 12 – Professional Compensation Plan. As part of its price proposal, offeror shall provide its total compensation plan setting forth salaries and fringe benefits proposed for the professional employees who will work under the contract IAW Section L, FAR 52.222-46 --

Evaluation of Compensation for Professional Employees (Feb 1993). Offeror’s shall provide supporting data and source information for compensation rates as a part of its price proposal.

a. Professional Employee Compensation. In accordance with FAR Subpart 22.11 and

FAR Provision 52.222-46, Evaluation of Compensation for Professional Employees, the offeror and subcontractors/teaming partners/joint venture partners shall submit a total compensation plan setting forth salaries and fringe benefit package proposed for the prime’s professional employees. The prime Offeror should instruct its subcontractors to submit their total compensation plan directly to the contracting officer. The prime Offeror shall also submit a list of the subcontractors to include points of contact and phone numbers who will be submitting professional employee compensation packages to the Government. The total compensation plan submitted in the proposal will not become part of the awarded contract. The plan shall set forth salaries and fringe benefits proposed for professional employees as prescribed in the instructions below:

(1) Total Compensation Plan. Provide the planned compensation for all professional employee labor categories proposed to perform the effort. Delineate which employees are considered professional employees and which employees are covered under the Service Contract

Labor Act. Describe the job skills and qualification requirements associated with each identified labor category. Identify the Standard Occupational Classification (SOC) code for each labor category using the Bureau of Labor Statistics SOC system http://www.bls.gov/soc/. The labor categories shall be identified by geographic location of contract performance. Provide the corresponding salary and quantified fringe benefits for each labor category and skill level. The fringe benefit data shall also include a description of the benefits offered, and to which professional employee category(ies) the fringe benefits would apply.

(2) Surveys and Studies. Provide the recent surveys and studies from professional, public, and private organizations (e.g., Watson & Wyatt, Economic Research Institute, Bureau of

Labor Statistics, etc.) used to support the proposed salaries and fringes. Identify the source with the provided surveys and studies. If not using one of the sources listed above, explain the credibility of the source providing the surveys and studies."

Exhibit A – Contract Data Requirements List (CDRLs), DD Form 1423-1, which is to be returned with Block 18 completed. If the CDRLS are not separately priced, use NSP to complete the block. The Task Order Pricing Table is to be submitted with Volume III and Labor Matrix is to be provided with Volume I.

Section K – Solicitation Provisions. Submit pages for only provisions which require offeror fill-ins: FAR 52.204-17, 52.209-2, 52.209-7, 252.209-7992 and DFARS 252.227-7017.

a. System for Award Management (SAM), Representations, Certifications, and other Statements of Offerors. Completed SAM representations, certifications, acknowledgments and statements shall be included with submission. Ensure SAM is current, complete, and accurate at the time of proposal submission. SAM representations and certifications must include those specified within Section K of this RFP. SAM can be accessed at https://www.sam.gov/portal/public/SAM/#1. Offeror shall be eligible for award under the specified NAICS code, 541511, and the specified set-aside at the time of submittal of their proposal and remain eligible when verified again prior to award of the contract.

Offerors are required to maintain an active registration in the SAM database at https://www.sam.gov/portal/public/SAM/ (see FAR 52.204-7).

b. Joint Venture Agreements. Joint Ventures are allowable if offeror(s) meets the criteria established in FAR 19.1403(c).

Section L – Instructions, Conditions and Notices to Offerors. Do not include. This section is not modifiable by the offeror and is redundant to include with the proposal submission with exception of DFARS 252.237-7024, Notice of Continuation of Essential Contractor Services.

Offeror shall include, in Part IV, documentation required by DFARS 252.237-7024. There is no page limitation for this document and it is not included in the overall page limitation.

Additionally, IAW Section I, DFARS 252.237-7023, Continuation of Essential Contractor

Services the submitted document will be incorporated into the contract by reference in Section J.

Section M – Evaluation Factors for Award. Do not include. This section is not modifiable by the offeror and is redundant to include with the proposal submission.

2. Offerors shall list any assumptions that affect the offered price, such as Government

Furnished Property/Government Furnished Equipment (GFP/GFE), and base support. Should the offeror have no assumptions, then the proposal shall so state “no assumptions made for the

RFP proposal submission.” Moreover, failure to include assumptions in this area will be deemed as acceptance of all terms and conditions of the solicitation (as amended) and resulting award document.

3. In the event offeror proposes to use classified or other workspace provided by a vendor outside of a subcontracting or teaming arrangement, the offeror shall submit an Associate

Contractor Agreement (ACA) IAW AFFARS IG 5317.9000 as part of its proposal.

4. OTHER INFORMATION REQUIRED. Offerors shall also provide a separate sheet at the end of this part with the following information:

a. Authorized Offeror Personnel. Provide the name, title, e-mail address, telephone numbers, street and mailing address (if a PO Box is used) of the primary and alternate company/division

POC regarding decisions made with respect to your proposal and who can obligate your company contractually. Also, identify those individuals authorized to negotiate with the

Government.

b. Company/Division Address, Identifying Codes, and Applicable Designations. Provide company/division's street address, county and facility code; CAGE code; DUNS code; TIN; size of business (large or small); and labor surplus area designation.

4. Accounting System. Provide a summary of your accounting system or methods. Provide your current Cost Accounting System (CAS) Disclosure Statement and discuss any pending revisions applicable to this solicitation. Indicate whether you have Government (DCMA) approval of your accounting system and if so, provide evidence of such approval. Also, identify any deviations from your standard procedures in preparing this proposal.
