The Offeror shall submit Performance Information Sheets in accordance with the format contained in Enclosure 3. This information is required on the Offeror and subcontractors, teaming partners, and/or joint venture partners proposed to perform aspects of the effort the Offeror considers critical to overall successful performance. Offerors are cautioned that the USG will use data provided by each Offeror in this volume and data obtained from other sources in the evaluation of past performance. “Prime contractor” means the legal entity with which the USG creates contractual privity by entering into a contract. “Subcontractor,” “teaming partner,” and “joint venture partners” mean any supplier, distributor, vendor, or firm that furnishes supplies or services to or for a prime contractor or another subcontractor, teaming partner, or joint venture partner. The Offeror shall submit up to eight (8) total Performance Information Sheets identifying active or completed contracts or task orders, either Government or commercial. Each relevant contract/task order shall have been performed during the past 8 years from the date of issuance of this solicitation and continuing up to date of award. Of the maximum of eight (8) PPI citations an
Offeror may submit, it may choose to include up to five (5) PPI citations on key teaming partners or subcontractors that it considers most relevant in demonstrating its ability to perform the requirements within the solicitation. Performance Information Sheets, up to eight, are limited to a maximum of 40 pages total. Offerors are cautioned that the Government will use data provided by each Offeror in this volume and data obtained from other sources in the evaluation of past and present performance
5.3.1 Relevant Contracts
For a description of the characteristics or aspects the USG will consider in determining relevance, see section M, 4.0 Factor 3: Past Performance. In addition to the submission of information on contracts considered relevant, include rationale supporting your assertion of relevance. Explain what aspects of the contracts are deemed relevant to the proposed effort and clearly link the PPI to the Technical factor. Indicate whether your performance on the contract was as a Prime, subcontractor or teaming partner. Offerors are cautioned that the USG will use data provided by each Offeror in this volume and data obtained from other sources in the evaluation of performance.
5.3.2 Recent Contracts
The USG will only consider contracts to be recent if they were performed during the past five (5) years from the release date of this solicitation.
5.3.3 Organizational Structure Change History
Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a "roadmap" describing all such changes in the organization of your company. A pamphlet or other commercial document describing such reorganizations may suffice. As part of this explanation, show how these changes impact the relevance of any efforts you identify for past performance evaluation/performance confidence assessment. Since the Government intends to consider past performance information provided by other sources as well as that provided by the Offeror(s), your "roadmap" should be both specifically applicable to the efforts you identify and general enough to apply to efforts on which the
Government receives information from other sources.
5.4 Evidence of Transmittal of Questionnaires
The Offeror shall coordinate Past Performance Questionnaires (Enclosure 2 to this Attachment) with each of the POCs identified in the Past Performance Volume, and provide evidence of transmission to those POCs. The Offeror may add its own transmittal letter to provide an Offeror representative name and telephone number; in case questions arise from the POC receiving the questionnaires. In your transmittal letter, direct the POCs to submit completed questionnaires directly to the USG POCs. The USG POCs are listed in the above paragraph 2.1.1 Points of
Contact. Once the Past Performance Questionnaires are completed by the POCs, the information contained therein shall be considered sensitive and shall not be released to you, the Offeror.
5.5 Subcontractor Consent
The Offeror shall submit, along with the information required in this Volume, a consent letter executed by each subcontractor, teaming partner, and/or joint venture partner, authorizing release of adverse past performance information to the Offeror so the Offeror can respond to such information. For each identified effort for a commercial customer, the Offeror shall also submit a client authorization letter, authorizing release to the Government of requested information on the
Offeror’s performance.
6.0 Volume IV - Cost/Price
6.1 Introduction
These instructions are to assist you in submitting information other than cost or pricing data that is required to evaluate the reasonableness and realism of your proposed cost/price. Compliance with these instructions is mandatory and failure to comply may result in rejection of your proposal.
For award purposes, evaluation will be in accordance with Section M.
6.2 General Instructions
6.2.1 Cost/Price Reasonableness and Realism
Note that unrealistically low or unreasonably high proposed costs or prices, initially or subsequently, may be grounds for eliminating a proposal from competition either on the basis that the Offeror does not understand the requirement or has made an unrealistic proposal. Offers should be sufficiently detailed to demonstrate their reasonableness and realism. The burden of proof for credibility of proposed costs/prices rests with the Offeror. All cost information included in the proposal must be consistent with the Cost/Price Volume.
6.2.2 Ground Rules and Assumptions
The Offeror shall identify all ground rules and assumptions (GR&A) used in developing the pricing documentation, as well as GR&A’s used in developing its proposal.
6.3 Cost/Price Data
Data beyond that required by this instruction shall not be submitted, unless you consider it essential to document or support your cost/price position. All data relating to the proposed price, including all required supporting documentation, must be included in the section of the proposal designated as the Cost/Price volume. Under no circumstances shall this data and documentation be included elsewhere in the proposal.
6.3.1 Cost or Pricing Data Requirements
The PCO has made a preliminary determination of “Adequate Price Competition”. Therefore, a
Certificate of Current Cost or Pricing Data per FAR 15.403-4 will not be required. In the event that adequate competition does not emerge in response to the solicitation, the PCO may require current cost and pricing data in accordance with FAR 15.4.
In accordance with FAR 15.403-1(b) and 15.403-3(a), information other than cost or pricing data may be required to support cost or price reasonableness and cost realism. Information shall be provided in accordance with FAR 15.403-5. If, after receipt of proposals, the PCO determines that there is insufficient information available to determine price reasonableness and none of the exceptions in FAR 15.403-1 apply, the Offeror shall be required to submit cost or pricing data.
6.3.2 Subcontracts
This worksheet tab shall identify the offeror’s subcontractors. For this cost type CLIN, identify each separate subcontract costs to be incurred by either the prime, an interdivisional transfer, or another subcontractor. Each line item, shall identify the applicable source (i.e. prime, division, or subcontractor).

These targets will be incorporated into and become a part of the contract.
The successful Offeror will be required to provide reports on SDB subcontractor participation in accordance with FAR clause 52.219-25 in Section I of the contract. Failure to submit such a plan will render the Offeror ineligible for award. In the event the Offeror has negotiated a
