About this file

This is a Privacy Act Statement form used by the Air Force Research Laboratory (AFRL) for collecting information from covered individuals involved in federal grants, cooperative agreements, contracts, and other research-related agreements. The form outlines the legal authorities, purpose, and routine uses of collected information, which is used to ensure Department of Air Force awards are given to responsible parties and to maintain records relevant to research agreements. The form requires signatures from both the covered individual and the institution's authorized representative.

The form is an appendix related to security risk review for Project Kaiju, a closed Broad Agency Announcement (BAA) from AFRL. It is a fillable template that includes fields for proposal title, covered individual information, institutional representative details, and dates, but contains no specific requirements or scope information about the actual contract opportunity.
