PAST PERFORMANCE COVER LETTER

SUBJECT: AFRL IT Support Services (AFRL-ITSS) Present/Past Performance Assessment

1. The Air Force Research Laboratory at Wright Patterson AFB, Ohio is in the process of selecting a contractor for award of the AFRL-ITSS contract. The AFRL-ITSS contract includes Computer Support (Specialized Helpdesk), Software Application Development, IT Planning, Network Implementation, Systems Integration, Systems Support, Communication Support, Network Media Devices, Asset Management, Cybersecurity Management, Video Teleconference Capabilities and meeting support, and Computer- Communication Maintenance for the Air Force Research Laboratory, Wright-Patterson

AFB, OH.

2. In the selection process, the Government intends to evaluate the past performance of each offer through verification of the Offeror’s past and present performance on other contracts. For this, we must depend on information received from agencies such as yours, which have had experience with an Offeror. We would appreciate if you complete the attached questionnaire and include any comments qualifying your ratings and answers. If you have any questions or difficulty completing the questionnaire, please contact the undersigned.

3. Please return your completed questionnaire to Ms. Jamie L. Parks via e-mail Jamie.Parks@us.af.mil no later than 10 working days from date of receipt. As an alternative, you can mail the completed questionnaire to:
