1.0 INTRODUCTION
This Storm Water Pollution Prevention Plan (SWP3) has been prepared to comply with the Ohio EPA Construction General Storm Water Permit No. OHC000003 (General Permit). A copy of the General Permit and Notice of Intent (NOI) form are attached in Appendix A. A copy of the Notice of Termination (NOT) form is also included in Appendix A. The NOT must be submitted to the Ohio EPA within 45 days of completing all land disturbance activities. The purpose of this SWP3 is to identify the potential sources of storm water pollution from the project and to establish measures and procedures for preventing storm water pollution. The project will include the demolition of facility 30089 and the grading and stabilization of a new lawn area at Wright-Patterson Air Force Base (WPAFB), northeast of Dayton, Ohio. Site location and layout maps are attached in Appendix B and details of the construction is shown on the project plans.
Activities during the project will include building demolition, grading, utility excavation, and/or filling activities which will result in the disturbance of more than one acre of land area. The project area is developed. All required preventative measures to provide erosion and sediment control and storm water management will be utilized throughout the project.
1.1 SWP3 Coordinator and Duties
The construction site SWP3 coordinator for the facility has not been selected due to the contractor not being selected. Their duties will include the following:
• Implement the SWP3 with the aid of the SWP3 team
• Oversee maintenance practices identified as Best Management Practices (BMPs) in the SWP3
• Conduct or provide for inspection and monitoring activities
• Identify other potential pollutant sources and make sure they are added to the plan
• Identify any deficiencies in the SWP3 and make sure they are corrected
• Ensure that any changes in construction plans are addressed in the SWP3.
To aid in the implementation of the SWP3, the members of the SWP3 team are the construction Superintendent(s) and construction Supervisors who will ensure that all housekeeping and monitoring procedures are implemented and the integrity of the BMPs is maintained.
2.0 MANAGEMENT CERTIFICATION AND PLAN AVAILABILITY
2.1 Management Certification
I certify under penalty of law that this document and all attachments were prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gathered and evaluated the information submitted. Based on my inquiry of the person or persons who manage the system or those persons directly responsible for gathering the information, the information submitted is, to the best of my knowledge and belief, true, accurate and complete. I am aware that there are significant penalties for submitting false information, including the possibility of fine and imprisonment for knowing violations.

2.2 Plan Availability
The Plan will be made available immediately upon request of the Ohio EPA during working hours. A copy of the NOI and letter granting permit coverage under the Construction General Storm Water Permit will also be made available at the site. A copy will be provided within 7 days of written request from the Ohio EPA, a local agency approving sediment and erosion plans, grading plans or storm water management plans, or to the public in accordance with the Ohio Public Records law.
3.0 PLAN REVISION AND AMENDMENT
3.1 Plan Revision
The SWP3 will be amended within 10 days after notification from the Ohio EPA that the SWP3 does not meet one or more of the minimum requirements of the General Permit. If requested, the revised SWP3 or a written certification that the requested changes have been made will be submitted to the Ohio EPA.
3.2 Plan Amendment
The SWP3 will be amended whenever there is a change in design, construction, operation or maintenance, which has a significant effect on the potential for the discharge of pollutants to surface waters of the state or if the SWP3 proves to be ineffective in achieving the general objectives of controlling pollutants in storm water discharges associated with construction activity.
4.0 DUTY TO INFORM CONTRACTORS AND SUBCONTRACTORS
All contractors and subcontractors who will be involved in the implementation of this SWP3 will be informed of the terms and conditions of the Construction General Permit. Prior to beginning their work on-site, all contractors and subcontractors are required to sign the acknowledgement form in Appendix C that certifies they have reviewed and understand the conditions and responsibilities of the SWP3.
5.0 SITE DESCRIPTION
5.1 Site Description/Construction Activity
As shown on the maps in Appendix B and the project plans, the project includes building demolition and site grading and removal of the buildings utility connections at Wright-Patterson AFB northeast of Dayton, Ohio. The project is shown on drawings in Appendix B and on the project plan sheets. The Task #6 project will be the complete demolition of the 30089 facility structure of approximately 150,200 square feet (sf) and the removal of approximately 7,665 sf of associated impervious area.
The area where soil will be disturbed is currently the Building 30089 facility. The site is bounded by paved streets with gutters on all four sides with Swann Pearson Road to the north and Ritchie Road to the south. The soil disturbing activities will include the excavation of site utilities and building foundations and mass grading.
Disturbed surface areas will be stabilized at the completion of the project. The site will be graded so that the storm water runoff will flow to existing storm drainage utilities.
5.2 Threatened/Endangered Species and Historic Preservation Review

As a primarily mowed lawn habitat in a developed area, no threatened or endangered species have been identified or are likely to occur in the project area. The proposed construction location is not located adjacent to any historic buildings nor located within a historic district. Due to the high level of past soil disturbance, there is a low likelihood of the presence of intact historic artifacts. A copy of the documentation regarding the review of threatened/endangered species and historic preservation is attached in Appendix I.
5.3 Disturbed Area
Disturbed area is defined as the area of the site that is subjected to grubbing, clearing, excavating, filling or grading.
The anticipated total project site area will be approximately 4.49 acres, as shown on the drawing in Appendix B and in the design plan set. The project will result in new pervious areas. Following demolition, the project area will have a total impervious area of 0.20 acres, or 4.5 percent of the project area. The disturbed area will be less than the entire project site.

The project involves demolition of the current facility and converting the area to open space. The runoff coefficient for the pre-demolition site conditions is 0.78 (Open Space and Impervious building & paved areas) and the post-demolition land use is open space with a runoff coefficient of 0.22. The pre- and post-demolition weighted runoff coefficient is calculated as follows:

The soil type mapped in the project area is Warsaw urban land complex, nearly level. A copy of the soil information from the NRCS Soil Survey is attached in Appendix D.
5.5 Prior Land Uses
The area that will be disturbed is currently a mostly unoccupied building with associated open space areas.
5.6 Implementation Schedule
Erosion and sediment control measures will be installed prior to soil disturbing activities. The Construction Manager will monitor regional weather forecasts and direct installation of coverings or other stabilization methods when a 24-hour rainfall event is expected to exceed 1 inch. The contractor and their various subcontractors will be on site from approximately 7:00 am until 5:00 pm, seven days per week. The tentative schedule for the erosion and sediment control activities is attached in Appendix F.
5.7 Receiving Streams/Surface Water
The project is located east of the Mad River. Storm water runoff of the area currently discharges to the WPAFB Municipal Separate Storm Sewer System (MS4).
The project will not disturb any wetlands or special aquatic sites.
6.0 CONTROLS
6.1 Contractor Responsibilities
The Contractor for the project work will be responsible for complying with the provisions in this SWP3 and in the Construction General Permit. The contractor will be responsible for installing all necessary erosion and sediment controls described herein and conducting the inspections and maintenance of the controls throughout the project.
6.2 Control Measures
6.2.1 Non-structural preservation methods
Existing natural vegetation areas (mostly grass) will be preserved to the extent possible. Only shrubs and other vegetation necessary for the work are expected to be disturbed as part of the construction activities.
A construction fence may be used to prevent inadvertent disturbance of areas beyond that needed for the project. Due to the existing building, there is very little existing vegetation at the site besides mowed grass lawn and a few trees and shrubs. The majority of the site will be regraded as part of the project.
6.2.2 Erosion control
To prevent soil from washing into the streams at the site or into undisturbed areas of the site, the following BMPs will be implemented:
• Minimize the areas of disturbance, minimize the amount of area disturbed at one time by utilizing phased construction, and prevent runoff from undisturbed areas, as appropriate.
• Stabilizing areas after final grading will be performed through appropriate management techniques such as temporary and permanent seeding, use of mulches, or geotextile fabric, in accordance with Ohio’s “Rainwater and Land Development” guidance manual.

• Disturbed areas will be permanently stabilized within 7 days of reaching final grade.
Temporary stabilization may be used in the event disturbed areas will be dormant for more than 21 days or idle over the winter. The temporary stabilization will be placed within 7 days of the most recent disturbance.
• Soil piles will be surrounded with silt fencing or covered with a tarp. Topsoil stockpiles stored for more than 21 days will be temporarily seeded or covered with a tarp to prevent erosion and compaction.
Stabilization methods shall be in accordance with Ohio’s “Rainwater and Land Development” guidance manual (portions are attached in Appendix H). Additional erosion and sediment control practices not already specified in this plan may be necessary due to unforeseen environmental conditions and/or changes in drainage patterns caused by earth-moving activity.
6.2.3 Runoff control
Runoff control at the project area will be managed by minimizing disturbance of the surface to only that needed for demolition of the facilities and site grading, and redirecting off-site runoff away from the disturbed areas where possible. Disturbed areas will be stabilized as soon as possible. Stabilization (temporary) will be initiated immediately following completion of disturbance and the areas will be permanently stabilized in accordance with Ohio’s “Rainwater and Land Development” guidance manual and/or the project specifications.
6.2.4 Sediment control
Sediment controls will be provided in accordance with best management practices, regulatory guidance, manufacturer’s specifications, and good engineering practices. Sediment controls will be implemented to meet the intent of the standards in the ODNR Rainwater and Land Development Manual, 2006. The sediment control practices that will be utilized at the site include:
• Minimize the areas of disturbance
• Stabilize areas after work has completed and establish permanent vegetative cover for all disturbed areas not intended for other cover, such as buildings and parking areas.
• Grade the work areas to minimize the runoff from disturbed areas and divert off-site areas from contributing runoff to the disturbed area.
• Install sediment barriers (including silt fences and storm drain protection) as necessary to isolate potential areas of erosion and minimize sediment transport
• Maintain stabilized construction entrances to minimize the transport of sediment from the disturbed areas.
• Surround soil stockpiles with sediment control barriers or cover stockpiles
Throughout the project, the contractor will inspect and evaluate sediment controls. The contractor will also provide additional sediment control measures to meet the intent of the standards as conditions warrant during work at the site. The contractor will, prior to and immediately following the extreme weather event, inspect all sediment control measures and evaluate implementation of additional controls to minimize mobilization of sediment into the surface waters.
Sheet flow runoff from the disturbed areas will be intercepted by erosion eels and inlet protection devices to prevent sediment transported via sheet flow. Erosion eels will be placed on a level contour.

Additional erosion and sediment control practices not already specified in this plan may be necessary due to unforeseen environmental conditions and/or changes in drainage patterns caused by earth-moving activity.
6.2.5 Inlet Protection
Storm drain inlets located along the roadways and the new storm drains installed as part of the redevelopment project will be protected utilizing inlet protection devices such as geotextile inlet protection, geotextile-stone protection, geotextile-stone curb inlet protection, gravel curb inlet protection, or a manufactured inlet protection device (such as “Dandy Bags”). All inlet protections must be inspected regularly and cleaned regularly to prevent clogging and subsequent flooding conditions. Sediment will be removed when accumulation is one-half the height of the trap.
6.2.6 Stream Protection
The proposed project will not disturb any surface waters of the State.
6.2.7 Modifying Controls
If periodic inspections or other information indicates a sediment and erosion control has been used inappropriately or incorrectly, the control will be replaced or modified for site conditions.
6.2.8 Post-construction storm water control
As required by the General Permit, sites that have been previously developed shall either ensure a 20 percent net reduction of the site impervious area, provide for treatment of at least 20 percent of the Water Quality Volume (WQV), or a combination of the two. A reduction of imperviousness is applicable to this site, with an 80% reduction of the post-construction impervious area.
6.2.9 Other controls
6.2.9.1 Non-sediment pollutant controls
The following BMPs will be implemented at the site to minimize the impact of non-sediment pollutants at the site:
• All waste materials will be collected and removed from the site. All trash and demolition debris from the site will be stored in covered containers and removed from the site. No demolition debris will be disposed of on site. All personnel will be instructed regarding the correct procedure for waste disposal.
• Good housekeeping and spill control practices will be followed during construction to minimize storm water contamination from petroleum products and other products used during the work. Equipment fueling and maintenance must be performed away from ditches or storm drains, in an area designated for that purpose.
• All construction vehicles on site will be monitored for leaks and receive regular preventive maintenance to reduce the chance of leakage. Tanks must be provided with secondary containment.
• Petroleum products will be stored in tightly sealed containers which are clearly labeled.
• No fuel storage areas will be allowed on the construction site.
• Spill kits will be included with all fueling sources and maintenance activities.

• All spills will be cleaned up immediately upon discovery. Spills of one gallon or more in a waterway must be reported immediately to WPAFB Fire Dispatch. Report spills immediately IAW the hazardous material site specific spill plan.
• Dump trucks hauling material from the demolition site will be covered with a tarpaulin.
• Concrete wash water will not be allowed to flow to storm drains or other water conveyance. A concrete truck washout pit with no potential for discharge will be constructed to contain concrete wash water.
6.2.9.2 Off-site traffic
Off-site vehicle tracking of sediments and dust generation will be minimized by limiting traffic in the construction area to only those vehicles necessary for the construction.
6.2.9.3 Compliance with other requirements
This SWP3 reflects the requirements for storm water management and erosion and sediment control, as required in the Ohio EPA Construction General Storm Water Permit. All State and local regulations regarding waste disposal, including prohibitions on open burning shall be followed.
6.2.9.4 Trench and groundwater control
Dewatering activities are not anticipated for this project. However, in the event that dewatering is required, the discharge will pass through a sediment control device such as a filter bag, sump pit, sediment pond, or other effective device prior to discharging to a vegetated area.
6.2.9.5 Maintenance/ Inspections
Visual inspections of all cleared and graded areas of the construction site will be performed at a minimum of every 7 days and within 24 hours following the end of a storm with rainfall amounts greater than 0.5 inches per 24 hour period. The inspection will be conducted by the SWP3 coordinator or his designated storm water team members. The inspection will verify that the structural BMPs described in the SWP3 are in good condition and are minimizing erosion. The inspection will also verify that the procedures used to prevent storm water contamination from construction materials and petroleum products are effective. The following inspection and maintenance practices will be used to maintain erosion and sediment controls:
• Erosion eels and silt fence will be inspected for depth of sediment and damage and to see that they are firmly attached to the ground.
• Built up sediment will be removed from erosion eels when it has reached one-third the height of the eel/fence.
• Inlet protection will be inspected for integrity and maintained by removing accumulated sediment when half full.
• Temporary and permanent seeding will be inspected for bare spots, washouts, and healthy growth.
• The roads around the demolition site will be inspected for sediment tracked on the roadway.
If an inspection reveals that a control practice is in need of repair or maintenance, it must be repaired or maintained within three days of the inspection. If an inspection reveals that a control
POLLUTION PREVENTION PLAN Page 8 of 8 practice fails to perform its intended function and that another, more appropriate control practice is required, the SWP3 must be amended and the new control practice must be installed within 10 days of the inspection. If an inspection reveals that a control practice has not been implemented in accordance with the SWP3 and/or the General Permit, the control practice must be implemented within 10 days from the date of the inspection. If the inspection reveals that the planned control practice is not needed, the inspection report must contain a statement of explanation as to why the control practice is not needed and the SWP3 will be updated.
The maintenance inspection report will be completed after each inspection. A copy of the report form to be completed by the SWP3 coordinator is provided in Appendix F of this SWP3.
Completed forms will be maintained on-site during the entire construction project. Following construction, the completed forms will be retained by the owner for a minimum of three years. If construction activities or design modifications are made to the site plan which could impact storm water, this SWP3 will be amended appropriately. The amended SWP3 will have a description of the new activities that contribute to the increased pollutant loading and the planned source control activities.

Appendix B: Sample Inspection Report
Instructions
This sample inspection report has been developed as a helpful tool to aid you in completing your site inspections. This sample inspection report was created consistent with EPA’s Developing Your Stormwater Pollution Prevention Plan. You can find both the guide and the sample inspection report (formatted in Microsoft Word) at www.epa.gov/npdes/swpppguide
This inspection report is provided in Microsoft Word format to allow you to easily customize it for your use and the conditions at your site. You should also customize this form to help you meet the requirements in your construction general permit related to inspections. If your permitting authority provides you with an inspection report, please use that form.
For more information on inspections, please see Developing Your Stormwater Pollution Plan Chapters 6 and 8.
Using the Inspection Report This inspection report is designed to be customized according to the BMPs and conditions at your site. For ease of use, you should take a copy of your site plan and number all of the stormwater BMPs and areas of your site that will be inspected. A brief description of the BMP or area should then be listed in the site-specific section of the inspection report. For example, specific structural BMPs such as construction site entrances, sediment ponds, or specific areas with silt fence (e.g., silt fence along Main Street; silt fence along slope in NW corner, etc.)
should be numbered and listed. You should also number specific non-structural BMPs or areas that will be inspected (such as trash areas, material storage areas, temporary sanitary waste areas, etc).
You can complete the items in the “General Information” section that will remain constant, such as the project name, NPDES tracking number, and inspector (if you only use one inspector).
Print out multiple copies of this customized inspection report to use during your inspections.
When conducting the inspection, walk the site by following your site map and numbered BMPs/areas for inspection. Also note whether the overall site issues have been addressed (customize this list according to the conditions at your site). Note any required corrective actions and the date and responsible person for the correction in the Corrective Action Log.
