The Contractor shall furnish this notice within 60 days after the increase or decrease, or within any additional period that the Contracting Officer may approve in writing, but not later than the date of final payment under this contract. The notice shall include the contractor’s proposal for an adjustment in the contract unit prices to be negotiated under paragraph (b) of this clause, and shall include, in the form required by the Contracting Officer, supporting data explaining the cause, effective date, and amount of the increase or decrease and the amount of the Contractor’s adjustment proposal.

j. Clause H-909 Performance Incentive Clause, referenced in RFP FA8536-11-R-30055 is hereby superseded by the following:
H-909 - PERFORMANCE INCENTIVE CLAUSE
This clause establishes an incentive structure and process under which the Government will provide an incentive to motivate excellence and continuous improvement in contractor performance in supply management through a combination of the opportunities to earn the Performance Incentive. The clause also establishes a remedy structure and process which allows the Government fair compensation should the contractor fail to meet the annual performance requirement for a defined key performance objective. Under this structure the contractor shall be evaluated at the end of each 12 month performance period, to determine if he will receive the Performance Incentive or be subjected to the Performance Remedy. This Performance Incentive/Remedy is tied to the average annual performance of the contractor in meeting the objectives of the monthly performance threshold function as outlined below. The incentive and remedy values are set at $300,000 per ordering period.
Award of Performance Incentive ($300,000): The Contractor may bill for the Performance Incentive immediately upon receipt of the Contracting Officer’s unilateral authorization document for payment. The Government shall make a determination—within 5 business days after receipt of the contractor’s final status report (CDRL A008) for each period of performance. If the determination is made that the Contractor will receive a performance incentive, the Contracting Officer shall advise the contractor of this determination in writing and the Contractor may then bill against the Performance Incentive CLINS for the period of performance.
Performance Remedy ($300,000): The Contracting Officer shall withhold remedy amount as follows: The Contractor will bill for the 12th month of the period of performance only after the determination has been made regarding whether or not the contractor will receive a performance incentive or a performance remedy. The Government shall make a determination—within 5 business days after receipt of the contractor’s final status report (CDRL A008) for each Period of Performance. If the determination is made that the Contractor will receive a Performance remedy, the remedy amount shall be subtracted from the month 12 payment of the Program Support (CORE) CLIN. The Contractor will then bill for the amount remaining after the remedy amount has been subtracted.
