1. Description Of Services

The F-16 Mission Module Computer (MMC) Chassis Tester is a critical component of the 523rd Electronics Maintenance Squadron (EMXS) located within the OO-ALC Complex. The MMC tests 90% of the F-16 MMC Chassis for the US Air Force. This requirement is to provide and initial evaluation, scheduled preventative maintenance, telephone support, on-site support and software support for the MMC.

2. Scope

2.1. The Contractor shall be knowledgeable in how the MMC works due to its uniqueness.

The Contractor shall provide all management tools, tools, supplies, equipment and labor necessary to maintain the MMC to OEM standards. The contractor shall perform quarterly scheduled preventative maintenance on the MMC; this would include any rework and travel expenses associated with rework to ensure that the MMC is fully operational and fully functional to test F-16 MMC Chassis.

2.2. The contractor shall provide telephone support to help the government trouble shoot or assist in performing repairs.

2.3. The contractor shall provide software support. All software updates and data generated under this contract will be delivered to the government with unlimited rights as defined in DFARS 227 with no proprietary markings or restrictive legends. The contractor shall be responsible for updating CPINs.

2.4. The Contractor shall provide personnel, equipment, tools, replacement parts, materials, and other items necessary to complete the tasks.

3. Basic Services

3.1. Initial Evaluation

3.1.1. The contractor shall conduct an initial evaluation at the beginning of the contract prior to any preventative maintenance or government requests for service. The initial evaluation will identify all current anomalies with the MMC and diagnose any work that needs to be performed on the MMC. Once the initial evaluation is complete the contractor shall provide the government with a report within 15 days detailing the following items: (Service Summary) (CDRL A001)

• The present condition of the MMC

• A parts list of parts needing updated or needing replaced

3.2. Quarterly Scheduled Preventative Maintenance & Phone Support

3.2.1.1. The Contractor shall perform quarterly scheduled maintenance on the

MMC to specifications as outlined in the Technical Orders (TO) as listed in Appendix D. Prior to starting any work the contractor shall provide an inspection report to the Program Manager (PM) listing any work that needs to be completed. The contractor shall provide the government with a copy of the report. Additionally the contractor will provided unscheduled phone support two –three times monthly and will have three (3) business days to respond by telephone or e-mail to arrange for a date or time acceptable to the Government to provide assistance via telephone support to bring the MMC Chassis back to fully operational condition. 3.2.2. The contractor shall perform the following maintenance on the MMC for each quarterly scheduled maintenance visit:

• Run a complete diagnostic of the entire MMC

• Report all findings to the PM in the Technical Report. (CDRL A003)

• Lubricate the MMC

• Replace/repair all worn parts

• Report any parts that may be in danger of failing or becoming worn in the

Technical Report. (CDRL A003)

• Install, run, and verify new paddle cards as required

3.2.2.1. Failure Diagnostic Report

3.2.2.1.1. The contractor shall provide Test Failure Sheets illustrating results for all testing that was accomplished on the MMC.

Technical Report

3.2.2.1.2. The Contractor shall record the following and submit it in the form of a Technical Report within five business days after quarterly maintenance has been completed. (Service Summary) (CDRL A003)

• All maintenance performed

• Repairs performed

• Number of personnel needed

• Number of hours to complete task(s)

• Courses of action taken in resolution of any problems

• Courses of action taken for the results of repairs (solutions)

• Any recommendations to prevent the same error from occurring

• The repairs completed

• A parts list separately identifying:

• Parts repaired with their part name and number

• Parts replaced with their part name and number

3.2.2.2. Verification Testing

3.2.3. Upon completion of work and prior to leaving the test facility, the contractor shall coordinate with the PM to verify testing of the work done on the MMC. The contractor shall provide a final verification test after repairs are completed. The contractor shall coordinate with the PM to certify, in writing, using a signed test failure sheet that the systems are fully functional and to submit the verification/certification in the Technical Report.

3.2.3.1. The contractor shall provide the PM copies of the printed test failure sheets recorded during testing on the MMC.

3.2.3.2. The final test shall be tested using an “A Condition Unit”.

• All tests shall be pass / fail when testing with the “A Condition Unit”.

• All passes shall meet the Current CPINs and Technical Orders applicable to maintenance and service.

3.2.3.2.1. All identified failures shall be corrected immediately.

3.3. Software Support

3.3.1. All Software modified or created will remain or become property of the US Government.

4. Service Summary

5. General Information

5.1. Security Requirements

5.1.1. No security clearances will be required; however contractors will be escorted at all times.

5.2. Government Furnished Property

5.2.1. The contractor shall have access to, the necessary facilities, tools, and property as required for the contract. Work is to be performed at Hill Air Force Base, Utah.

5.2.2. Government telephones shall be used for official business only.

5.2.3. Physical Security

5.2.3.1. The contractor shall be responsible for safeguarding all government property provided for contractor use. At the close of each work period, government facilities, property, and materials shall be secured.

5.3. Environmental Controls

5.3.1. The Contractor shall ensure their employees are made aware that the performance of these services will occur in industrial areas. The industrial complex at Hill AFB have the potential to expose workers to hazardous materials which may include, but is not limited to: hexavalent, chrome, cadmium, beryllium, lead, and others. While the Contractor shall not perform duties in a regulated area (where exposure are expected to exceed the permissible exposure limits set by 29 CFR1910.10xx), there may be contact hazards with these materials. The contractor shall ensure proper protective measures and training is taken to ensure contracted employees are protected from hazards. Survey of these areas which will be made available upon request.

5.3.1.1. The contractor shall be knowledgeable of and comply with all applicable Interstate, Federal, State, and Local laws, regulations, and requirements regarding environmental protection.

5.3.2. Notification of Environmental Spill

5.3.2.1. The contractor shall immediately report any spills or releases of any substance contained in 40 CFR 302 into the environment to the designated Government Representative. The liability for the spill or release of such substances rests solely with the contractor and its agent.

5.3.3. Material Storage and Use

5.3.3.1. The contractor shall follow manufacturer’s guidelines and professional recommendations for control of humidity, temperature, cleanliness, and materials handling. This includes hazardous materials.

5.4. Contractor Quality Control Program (QCP)

5.4.1. The QCP shall be provided within 5 days after award and shall be evaluated at the basic contract level. The QCP shall be updated, provided, and evaluated as requested. The contractor shall make appropriate corrections and modifications to the QCP and obtain acceptance of the plan by the contracting officer before the start of the first operational performance period. Updates shall be provided whenever there is a change in any of the Quality System Elements (with the exception of Contract Review, Design Control, Purchasing, Product Identification and Traceability, and Handling, Storage, Packaging, and Delivery). The contractor shall ensure quality service is maintained throughout the life of the contract.

5.4.2. The plan shall reflect the contractor’s overall approach, internal management controls, processes, and procedures for reporting to the Government.

5.4.3. The plan shall identify the means by which the contractor will ensure quality effectiveness and demonstrate comprehensive management and review of data. The plan shall describe what is measured, how often it is measured, tracked and provided; who reviews and assures that appropriate action is initiated when trends are unfavorable, who the Government will contact for contractor quality issues, what method and process is used to track and resolve issues, where reports are sent and maintained, and how often quality issues are reviewed and reported. The plan shall identify how the contractor identifies and resolves deficiencies, identifies potential improvements, and maintains and makes available to the Government, documentation reflecting quality control inspections and any corrective actions taken. The Contractor shall provide copies of all discrepancies to the Government Program Manager and the COR.

5.5. Contracting Officer Representative (COR)

5.5.1. See Paragraph 5.9; Government Observations

5.6. Hours of Operation

5.6.1. Normal Hours of Operation

5.6.1.1. The contractor shall perform the services required under this contract during the following hours: Monday through Thursday 0600–1530 – EXCEPT FEDERAL HOLIDAYS. All Friday times are to be negotiated with the PM and are at the PM discretion if Fridays are needed.

5.6.2. Recognized Holidays

5.6.2.1. The contractor is not required to provide service on the following days:

New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday.

5.7. Manpower Reporting

5.7.1. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this MMC Chassis Service contract. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil.

5.7.2. Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September.

While inputs may be reported any time during the FY, all data shall be reported no later http://www.ecmra.mil/

11/18/2015 than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.

5.8. Travel Requirements

5.8.1. All travel required shall be performed in accordance with FAR 31.205-46, Travel Costs. The contractor is responsible for making all necessary travel arrangements for their personnel, including air travel, car rental, lodging and subsistence. Travel is restricted to round trip coach airfare tickets, economy class car rental, and per diem rates not to exceed Federal Travel Regulation (FTR) rates for the TDY location. All travel must be in accordance with FTR, Joint Travel Regulations (JTR) and local security requirements. The Contractor shall also provide a proposed itinerary and estimated travel cost, IAW FAR 31.205-46, to the contracting officer for approval prior to travel. (Service Summary)

5.8.2. The contractor shall provide expenditure documentation for all reimbursable cost in accordance with FAR 31.205-46, to the Contracting Officer (CO).

5.9. Government Observations

5.9.1. Government personnel, other than CO and COR, may from time-to-time, with CO coordination, observe contractor operations. However, these personnel may not interfere with contractor performance.

5.9.1.1. During the period that the contractor is working on the Test Stands, trained personnel from the 524th EMXS, 525th EMXS, 526th EMXS, or 309 EMXG/EN, will oversee the work and will make the final acceptance.

5.9.2. A COR will provide surveillance of tasks in the Service Summary (SS) and other items required by the contract (e.g., Air Force specific guidance, Operating Instructions, etc.)

5.9.3. If the COR indicates deficiencies in performance, the COR or alternate(s) will process a Corrective Action Request and provide a brief of the finding to the contractor. The contractor shall provide a corrective action and corrective action to prevent recurrence to the CO regarding the deficiencies within 10 business days.

5.9.3.1. If corrective actions or corrective actions to prevent recurrence are not sufficient, the COR will notify the CO for action. If the contractor disagrees with the noted discrepancy and an agreement cannot be reached, the CO shall be notified for a final decision.

5.9.4. Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the CO and not to the COR.

SAFETY, FIRE PROTECTION AND HEALTH

SECTION I - GENERAL REQUIREMENTS

A. Safety Program Requirements.

The contractor will implement a safety program plan that ensures protection of Government personnel and property. The program will consist of, as a minimum:

1. Mishap reporting, as defined in paragraph B1 below.

2. A Safety Plan that addresses, as a minimum, the subjects listed in Section II –Specific Requirements, and will be used during the performance of the work described in the contract.

The Safety Plan will be accepted by OO-ALC/SE (Safety Office) prior to commencement of any work described in this contract.

3. Routine and recurring surveillance to ensure the safety requirements of this contract are enforced.

4. Competent personnel to provide surveillance of the Safety Plan.

5. Identification of segregated work site locations for operations that cannot be co-mingled with general industrial operations and the process for ACO approval of operations and changes at these specific sites.

6. All contractor personnel shall be trained and qualified to perform their duties safely.

7. The contractor shall include a clause in all subcontracts requiring the subcontractors to comply with the safety provisions of this contract, as applicable.

B. Mishap Notification

1. The contractor shall notify OO-ALC/SE (801-586-6038) or the Hill AFB Command Post (801-777-3007) after normal duty hours, and the designated Government Representative (GR), i.e., the ACO, PCO, or DCMA COR (Contracting Officer's Representative) within one (1) hour of all mishaps or incidents at or exceeding $2,000 (material + labor) in damage to DOD property entrusted by this contract, even if the government is wholly or partially reimbursed. This notification requirement shall also include physiological mishaps/incidents. A written or email copy of the mishap/incident notification shall be sent within three calendar days to the GR, who will forward it to OO-ALC/SE. For information not available at the time of initial notification, the contractor shall provide the remaining information no later than 20 calendar days after the mishap, unless extended by the ACO.

Mishap notifications shall contain, as a minimum, the following information:

(a) Contract, Contract Number, Name and Title of Person(s) Reporting

(b) Date, Time and exact location of accident/incident

(c) Brief Narrative of accident/incident (Events leading to accident/incident)

(d) Cause of accident/incident, if known

(e) Estimated cost of accident/incident (material and labor to repair/replace)

(f) Nomenclature of equipment and personnel involved in accident/incident

(g) Corrective actions (taken or proposed)

(h) Other pertinent information

2. The contractor shall cooperate with any and all government mishap investigations.

Additionally if requested by government personnel or designated government representative (GR), i.e., the ACO, PCO, or DCMA COR, the contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by safety investigators.

3. The contractor shall provide copies of contractor data related to mishaps, such as contractor analyses, test reports, summaries of investigations, etc. as necessary to support the government investigation.

4. The contractor shall support and comply with the safety investigation and reporting requirements of AFI 91-204, Chapters 1 – 5.

C. General Safety Requirements:

If the safety plan is modified, the contractor shall submit the proposed modification, in writing, to the Contract Administration Office safety representative.

The contractor is solely responsible for compliance with all federal, state and local laws, the Occupational, Safety and Health Act (OSHA) (Public Law 91-596) and the resulting standards, OSHA Standards 29 CFR 1910 and 1926, as applicable, Air Force Occupational Safety and Health Standards (AFOSH STD), to include AFI 91-203, Air Force Consolidated Occupational Safety Instruction, AFI 91-204, Safety investigations Reports, AFPAM 91-210, Contract Safety, and the protection of their employees. Additionally, the contractor is responsible for the safety and health of all subcontractor employees. All documents required as a result of OSHA 29 CFR 1910 and 1926 regulations, (i.e. certifications, training, respiratory protection program, workplace hazard assessments), shall be made available to the COR upon request.

The contractor shall ensure assigned personnel are adequately trained and qualified for the task being performed. Brief all personnel on the hazards involved with operations and applicable precautions to be taken.

Hazard Assessment. In accordance with 1910.132(d)(2), the contractor shall conduct a certified Hazard Assessment on all work areas. A follow-on survey shall be conducted by the contractor when the workload or environment conditions change. If respirators are necessary to protect the health of the employee or whenever respirators are required by the contractor, the contractor employer shall establish and implement a written respiratory protection program according to 29CFR 1910.134.

Contractor personnel must receive Explosive Safety Awareness Training if their duties require entry into the Explosive Clear Zone. This training will be provided by the OO-ALC Safety Office. Contact Eric Covington at 586-0168.

A Risk Assessment will be required prior to any work being accomplished, if munitions will remain in the facility.

Compliance with OSHA and other applicable laws and regulations for the protection of contractor employees is exclusively the obligation of the contractor. Note: AFOSH STDs are annotated because many of the Air Force Standards exceed the OSHA standard criteria. If a conflict is noted, the most stringent requirement takes precedence. The government shall assume no liability or responsibility for the contractor's compliance or non-compliance with such requirements. The contractor shall furnish to each of his/her employees a place of employment, which is free from recognized hazards. The contractor shall brief his/her employees on the safety requirements of this contract and on hazards associated with prescribed tasks. The contractor is responsible for compliance with OSHA Public Law and the resultant standards identified within. In addition, the contractor is required to flow down the safety requirements/specification to all subcontractors. This applies to Federal Acquisition Regulation (FAR) 12 commercial acquisitions as well. 11/18/2015 in surroundings or under working conditions which are unsafe or dangerous to their health. The contractor must coordinate and perform work so as not to impact the safety of government employees or cause damage to government property. This requires providing personnel with protective equipment and associated safety equipment as may be necessary. The contractor must also protect personnel from hazards generated by the work. If the contractor employs BILINGUAL speaking employees, they must post bilingual signs and have written procedures for specific tasks in applicable languages.

SECTION II – SPECIFIC REQUIREMENTS

The contractor’s prepared Safety Plan shall:

• Demonstrate a management commitment to employee safety and health

• Identify applicable rules and regulations

• Identify the roles and responsibilities of Management, Supervisors, Employees and

Safety Coordinator

• Identify work to be performed and location of expected operations

• Provide a description of safety program, safety monitoring responsibilities, organizational structure, and contact information for on-site personnel

• Include a work hazard analysis of the worksite and operations to be performed to include baseline hazard identification and required control measures

• Identify employee safety and health training requirements and the documentation process

• Include emergency response plans and procedures that relate to protection of government personnel and property

• Include hazard reporting procedures and identify individual(s) responsible for the correcting identified hazards

• Identify procedures for accident reporting and investigation

• Identify the process for tracking controlled hazards in contractors work area

The contractor shall ensure that each element identified below is adequately addressed in detail in the safety and health plan:

PEDESTRIAN CROSSWALKS: All contractor personnel are required to use the closest crosswalk, or traffic controlled intersection when crossing the road. Pedestrians must look both ways to ensure the coast is clear before stepping out into the crosswalk. Pedestrians DO NOT have the right of way unless they are already in the crosswalk. Contractor vehicle operators have the same responsibilities as pedestrians, to share the road and mutually observe and yield to pedestrians.

MOTOR VEHICLES: Contractor shall comply with the standards in: DoD Directive 5525.4, Enforcement of State Traffic Laws on DoD Installations", Nov 2, 1981, Para 3-4; DODI 6055.4, DoD Traffic Safety Program, 20 Jul 99; AFI 91-207, USAF Traffic Safety Program, 22 May 07;

and AFI 91-207 AFMC SUP1, The US Air Force Traffic Safety Program, 1 Oct 07. Each applies to all persons at any time on an Air Force Installation and includes all leased, owned, or privatized property including housing areas. In addition: AFI 13-213, Airfield Management, 29 Jan 08, Paragraphs 1.3.6 and 4.4.2.1 applies to all contractors, sub-contractors, vendors, commercial delivery companies, and all other private business vehicles who operate anywhere on Hill Air Force Base, including the airfield (to include the industrial areas and any buildings or hangars located upon the airfield) in support of their mission.

HOUSEKEEPING: Housekeeping shall be conducted according to the requirements in OSHA Standard 29 CFR 1910.141. CLEAN AS YOU GO will be enforced.

The authorized Government Representative (OO-ALC Safety Office) shall approve the contractor’s procedures within the Health and Safety Plan as meeting the intent of the below referenced standards, with the exception of Mishap Notification / Reporting Procedures, which are outlined in Section I, paragraph B of this Appendix C.
