THIS DOCUMENT ONCE COMPLETED BY THE PROGRAM OFFICE IS USED FOR SURVEILLANCE BY THE QAP.

1. Overview. This Performance Plan details how the multi-functional team (MFT) will manage the AN/TMQ-53 Tactical Meteorological Observing System (TMOS) Performance Based Contractor Logistics Support (CLS) Contract throughout its life cycle. This plan will focus on the level of performance required by the Performance Based Work Statement (PBWS). It is designed to provide an effective surveillance method of monitoring contractor performance for each listed objective on Attachment 1 - Services Summary (SS). This performance plan is a living document and shall be revised by the Government as circumstances warrant.

2. Objective. The objective of the TMOS CLS Contract is to provide performance based logistics services to the AN/TMQ-53 program. The contractor provided service is to plan, manage, and execute engineering, logistics, and operational support tasks, including all modifications and upgrades required for the fielded systems (currently at 297), spares, including two test bed systems to be located at the Contractors facility. Services include helpdesk support necessary to trouble-shoot, maintain, update, correct, and integrate hardware and software. Software specific requirements include Air Force Network Operations and Security Center’s Time Compliance Network Order applicability and operational impacts, required patches/upgrades. DoD/AF, Original Equipment Manufacturers software upgrades requirements on TMOS laptop Operating Systems. Software support and preparing associated documents for TMOS AF network issues required to keep the Authority to Connect (AtC) on the DoD network IAW DoDD 8500.1-2 System Security Certification and Accreditations. Hardware specific requirements include repair, integration, and ensuring fielded systems remain 96% fully mission capable. Management of logistics services requirements include Packaging, Handling, and Transportation (PH&T) globally and to ensure all correct customs documentation are on file to ensure the TMOS systems remain operational to meet wartime requirements.

3. Multi-Functional Team Members (MFT) Responsibilities and Goals. The MFT is the contract specific team, composed of stakeholders in the acquisition and are responsible for the acquisition throughout the life of the requirement. The goals of the team are to assess and manage this contract, provide an avenue to collectively create a strong working relationship, and to provide the highest level of contract performance and customer service. The MFT includes, but it is not limited to the following:

3.1 Contracting Officer (CO)

Acts as the principal Business Advisor for the government and is responsible for developing the solicitation, conducting the source selection, and manages the award of the contract. The CO has the sole authority to contractually bind the Government.

3.2 Quality Assurance Personnel (QAP)

AN/TMQ-53 program manager (PM) and alternate QAP designee shall perform common surveillance or quality control methods. Perform surveillance according to this performance plan and provide documentation of surveillance observations to the CO. Recommend improvements to this plan and the PBWS. Certify acceptance of services and calculate unacceptable service prescribed by the CO.

3.3 Contract Negotiator (CN)

Provides Phase II Training (contract specific training) associated with administering the contract after contract award.

3.4 Functional Director (FD)

Assigns a Primary and Alternate QAP to the MFT. Reviews contractor performance documentation, prepared by the QAP to ensure performance is compatible with contract and mission objectives. FD reviews the AN/FMQ-19 PBWS and PP annually with the MFT.

3.5. Contractor

Performs in accordance with the contract and Performance Based Work Statement (PBWS).

3.6. Additional Stakeholders

Responsible to ensure USAF requirements are satisfied in the most efficient/effective manner in accordance with the attached PBWS.

4. Performance Assessment. The Services Summary requirements are divided into performance objectives that relate directly to mission essential items. The performance objectives describe the minimum acceptable levels of service required for each requirement. The thresholds are critical to mission success and acceptable (satisfactory) performance. The standardized form used for recording performance assessments is the Quality Performance Assessment (QPA) provided under Attachment 2.

4.1. Performance Assessment Items not Specifically Identified All other tasks required under the PBWS may be inspected on a random basis at a frequency determined by the CO, and/or QAP per the Inspection of Services Clause in the contract.

4.2. Remedies for Non-Conformance. If metric reviews indicate unacceptable performance, the QAP using the QPA will notify the contractor for correction of the discrepancies (errors). The contractor shall be given 3 working days to provide a get well plan after notification to correct the unacceptable performance at no additional cost/fee. If discrepancies are not corrected prior to the 10th working day after notification, the QAP will notify the CO. If the contractor disagrees with the noted discrepancy and an agreement cannot be reached, the CO shall be notified for a final decision.

5. Methods of Surveillance

5.1 Random Sampling. The government QAPs use of random sampling is for frequently recurring tasks, typically receipt of repaired items, field returned components. Initially, the QAP will evaluate randomly selected samples of the activity to determine the acceptability of the entire requirement. This method reduces the time QAP must spend on surveillance duties, and yet it still gives a reasonably accurate picture of a service’s overall acceptability. QAPs use of PAR will be notification of discrepancies found to the contractor.

5.2 Valid Customer Complaint. The government QAP receives weekly Air Force Weather Agency REMEDY database user satisfaction reports. The QAP will investigate customer issues and assess the action that is required as a result of the complaint and document all customer complaints. Customer complaints become a permanent part of the QAP assessment records and if QAP requires, a PAR notification to contractor.

5.3 Periodic Surveillance. The government QAP will inspect the performance objectives as indicated in paragraph 4 to ensure contractor compliance with the appropriate paragraphs of the PWS for those objectives listed above utilizing this method. Status for this contract will be provided in the monthly status report (CDRL A005) containing a review of work accomplished during the reporting period from the contractor.

5.4 Standard. Recurring discrepancies are not permitted for any of the service items listed in the SS. No discrepancy will be recorded against the contractor’s performance if it is addressed and corrected during the 10 working day reporting period. However, if the same situation occurs again during the reporting period to bring the performance threshold below the acceptable level, a discrepancy will be recorded and reported to the CO for appropriate action. Additionally if the same discrepancy(s) occur repeatedly throughout different reporting periods, this will reflect upon the contractor’s performance. The QAP will annotate the unacceptable performance and notify the CO in order to address the problem areas with the contractor in a MFT meeting.

6. Performance Management

6.1 Monthly Status Report. This contract will be reviewed on a monthly basis by the QAP IAW Services Summary. Status for this contract will be provided in the monthly status report (CDRL A005) containing a review of work accomplished during the reporting period, repair schedule, and will be included in the surveillance folder.

6.2 Multi-functional Team Meetings. TMOS program will adopt and hold the MFT team meeting at the TMOS PMR as required per contract. The goal of the multi-functional team is to give all members, government and contractor, a vested interest in maintaining the highest quality service to our customers and the ability to propose/initiate improvements. The success of the contract is a combined effort. The MFT will manage the contract for the life of the contract. This team will form a partnership between the government and the contractor to ensure the best possible service is provided. All performance assessment data and updates will be received at these meetings. If performance improvement is necessary a plan of action will be created. The MFT will continually review data and procedures to improve efficiency. Additional meetings if required will be held.

6.3 DCMA Surveillance. Appropriate DCMA office will provide additional surveillance and may use a variety of methods including random sampling of recurring service outputs, one hundred (100) percent inspection of repair item output and periodic inspection of the repair process to evaluate the contractor’s performance for the listed performance objectives.

6.4 Contract Performance Assessment Reporting System (CPARS). CPARS will be updated at least annually documenting contractor performance for this specific contract. More frequent updates may be made as required to document a significant improvement or degradation of the level of performance. The Assessing Official will be the QAP/PM, and the Reviewing Official will be the Functional Director. The contractor representative also has input prior to this report being finalized. CPARS rating areas defined in Services Summary will be used in each assessment.

6.5 Disputes. Every attempt will be made to resolve all disputes arising under this contact/plan using the Alternate Dispute Resolution (ADR) as outlined in FAR 33.214. If no resolution can be made under ADR, the Contracting Officer’s final decision will stand.
