1 Exterior windows and screen and interior windows over seven (7) feet high shall be cleaned, as outlined in the IMFE. There is no indication in the IMFE’s as to how much glass there is at each facility. Is there data available as to how many windows there are at each facility and how many panes there are per window
2.2.1 of the PWS applies No, there is currently no breakdown of the number of windows or panes per window for each facility. See PWS, 2.2.8.2 data.

2 Government furnished support; there is no mention of a Computer being made available to the Contractor for use. If a Government computer is not available for Contractor use, will the Contractor be authorized to establish their own services and have a Company
Computer on Site, or at a minimum a Lap Top and be allowed the use of an Air Card?
In PWS 5.1, Government furnished support
An amendment will be forthcoming updating the
Atch 1. PWS. A computer will be available for use with a Common Access Card (CAC) which is typically issued to the CEH/AEH. If a contractor wishes to bring additional computers, it will have to operate independent of the government's system/network.

3 There is an apparent disconnect between the RS
Levels in the PWS verses the IMFE's and Room
Inventories. Examples: RS Level #4 is 5 days per week services in non clinical admin areas. However, there is no square footage listed in any of the 56
Room Inventories for RS #4. RS Level #8 is 7 days per week services in common/high traffic areas, elevators, break rooms, kitchens. However, there is no square footage listed in any of the 56 Room
Inventories for RS #8. RS level #9 is 5 days per week services in common/high traffic areas, elevators, break rooms, kitchens. However, there is no square footage listed in any of the 56 Room
Inventories for RS #9. To accept the IMFE/Room
Inventories is to accept that there are no non clinical, administrative or common/high traffic areas, elevators, entrance ways or break rooms in any of the
MTF's, which is obviously not correct. Please provide an accurate foot print of the MTF's with corrected Room Inventories. (Note: We are assuming that "common/high traffic areas" includes all lobbies and corridors. Please also verify if this is a correct assumption?)
PWS 2.1.1. 1.-10. Revised Atch 2. Propaks. Room Inventory Tab:
includes updates to RS levels 4, 7, 8, and 9, and updated square footage. Square footage totals were updated if there was a change since the original RFP was posted. Yes, lobbies and corridors are to be considered common areas and will require ongoing policing throughout the day.
An amendment will be forthcoming updating
PWS Section 2.1: Housekeeping Requirement
Summary (RS) Levels & Cleaning, have been revised to reflect the changes.
Atch 1. PWS
Atch 2. Propaks
4 How is RS Level #10 to be priced ? PWS 2.1.1.11 Generally, RS #10 is considered to be part of the total clean requirement. CAT A cleaning is standard performance requirement and part of the
FFP. RS #10 will be utilized if/when CAT A cleaning is required in a specific location for a dedicated time or event for an emerging CAT A disease.
5 This is a follow-up to the 15 Dec 2016 Q&A, Item
No. 9. A specific question was asked regarding the requirement for contractor employees to be re-immunized for Polio as an adult. A detailed answer regarding overall immunizations was given, however the answers did not address the original question regarding Polio vaccinations. The question is resubmitted:
What CDC, medical protocols or other Air Force standards are being relied upon to require that all housekeeping personnel be re-immunized with a series of polio vaccines as an adult? In our research, HAMS housekeeping personnel do not fall into any high risk groups that would indicate a requirement for re-vaccination for Polio as an adult. We are primarily concerned that this requirement may be (1) exposing personnel to unnecessary immunizations with possible side effects and (2) secondarily, adding unnecessary costs to the contract.
PWS 2.10.3.1.4. No re-immunization for Polio will be required.
AF aligns the requirements for immunizations with CDC guidelines. Per the CDC, polio vaccine is not routinely recommended for HCW. Most adults do not need IPV because they were already vaccinated against polio as children. For a list of
CDC recommended vaccines for healthcare workers, please visit the below website:
6 With respect to the costing report described here for the various cycle tasks, how is "square footage" to be shown in the report? Is this square footage of the facilities on the contract at that moment?
PWS 2.2.8.2 Currently the answer is no. In response to the question on how the square footage is to be shown in the report, please provide the information wherein the format and content be in a manner that allows the Government to discern the pricing associated with the individual cycle tasks and the nature of contract support it requires to execute (e.g. CMEs, frequency, duration).
'"Data other than certified cost or pricing data" means pricing data, cost data, and judgmental information necessary for the contracting officer to determine a fair and reasonable price or to determine cost realism. Such data may include the identical types of data as certified cost or pricing data, consistent with Table 15-2 of
15.408, but without the certification. The data may also include, for example, sales data and any information reasonably required to explain the offeror's estimating process, including, but not limited to-
(1) The judgmental factors applied and the mathematical or other methods used in the estimate, including those used in projecting from known data;
and
(2) The nature and amount of any contingencies included in the proposed price.
7 This is a follow-up to the 15 Dec 2016 Q&A Item
No. 2 wherein the government gave an answer that
"the Contractor decides which (key personnel)is most appropriate based on the MTF". In our opinion, the contract documents themselves are not nearly as clear as this answer. The PWS considers key personnel as
CEH, AEH and Shift Leader in 2.6.1. However, the second sentence in 2.6.1. in yellow highlight states that the contractor shall provide and onsite CEH or
AEH. (There is no mention of a Shift Leader). The
PWS goes on to state that AEH will serve in absence of CEH and Shift Leaders will assume AEH duties during AEH absence. In addition, the IMFE's have a statement that the CEH, AEH or Shift Leader will available during certain hours. When comparing the
PWS, IMFE and Q&A, the specific requirements as to when and where a CEH, AEH or Shift Leader will be required is blurry and ambiguous. Please clarify this essential requirement in the contract documents.
PWS 2.6.1.
PWS 2.6.1.2.
PWS 2.6.1.3.
All IMFE's
The intent of this requirement is to require either a CEH or AEH be designated at each MTF as determined by the contractor. Each IMFE will designate "core hours" in which CEH or AEH must be available. During non-core hours, in the absence of either of these positions, a shift leader must be designated.
Atch 1. PWS
8 Contractor shall purchase & implement environmentally preferable cleaning products or equivalent. Must be compatible with per IMFE…
…supplies and equipment … must be compatible with government furnished dispensers and fixtures as outlined in the IMFE.
Per IMFE, varied products/dispensers are in use at the MTFs. Is Contractor permitted to replace/standardize dispensers & products across the awarded MTFs, using products meeting section 2.9.8 specifications?
PWS 2.9.8 & 5.2.1/
Pg. 14 & 24
Yes, consumable supplies can be standardized as long as they meet the minimum specifications in the IMFE.
Atch 1. PWS
9 Can the government please provide a listing of all the
USAF bases (hospitals or clinics) that will have a
TJC, DNV and CARF inspections in the Base Year?
This information is not relevant to a proposals.
All MTFs are to be in compliance with the TJC at all times.
10 In regards to the ProPaks, are we required to submit documentation on how we derived at the cost associated within the Input Tab of the ProPaks (i.e..
labor, materials, etc…)?
ProPaks No. In the event we decide we need to other than certified cost or pricing data, we will request it.
Atch 2. Propak
11 Can the government provide the flooring ratio of carpet vs hard surface for each facility?
See ProPaks Carpet is generally only found in areas designated as administrative departments and waiting areas.
Offerors can utilize the room classifications on the IMFE to determine approximate flooring/carpet ratios.
Atch 2. Propak
12 Can the government please provide a listing the various floor types (VTC, Tile, Carpet, etc.) for the in-scope MTFs? This will allow accurate selection & costing of equipment needed for contractually compliant aseptic services for floor maintenance.
No consolidated listing of floor types exist.
Flooring types vary at each MTF. The types of flooring are consistent with standard flooring types found in healthcare facilities

13 Can we obtain more info regarding only the Hospital
& Amublatory Care Centers statistics? Ie… number of surgeries per year, number of emergency department visits per year, number of L&D deliveries per year, number of ambulatory surgery procedures per year, annual in-patient days per year, number of patient discharges per year, number of patient transfers per year.

15 The majority of the MTF locations are covered by a
CBA. Each Year, usually no later than 1 September, through negations between the Union and The
Contractor, new wages are determined along with increases in fringe benefits. For pricing purposes, the wages and fringe benefits will be those that became effective 1 Oct 2017 for FY-17. When Contract is awarded, however, new wages and fringe benefits, in all likelihood, will have increased. Will successful offerers be entitled to a Contract Price Adjustment to recoup those dollars that must be paid to the employees/Union?
Attachment 6 of the
Solicitation (Collective
Bargaining Agreements and
Area Wage Determinations applies).
The contractor is solely responsible to ensure
SCA and CBA compliance. Should changes occur to the SCA/CBAs during the solicitation process and award be made without discussions the contractors should follow the process outlined in terms and conditions and 52.222-41 to request an equitable adjustment.
Atch 6. CBA & WD
16 In the CBA file - what is the purpose of all of the
DOL wage determinations for all of the union locations ? The yellow highlighted wage rates in the
DOL determinations are different than the wage rates in the CBA's. This is confusing as to which rates prevail. Also, why are only Page 1 and 2 of the DOL wage determinations in the file. The DOL pages with health and welfare rates, paid holidays, paid vacations are missing. Please clarify these ambiguities and identify which rates are to be used for bidding; Union or DOL ? The Service Contract
Act is clear that all bidders should be on a level playing field in terms of wage and benefit rates.
Please clarify.
CBA / Wage Determination file.
The Government provided short forms of the
DOL wage determinations for all locations. The contractor's are solely responsible for ensuring compliance with applicable prevailing wage rates in accordance with the SCA or CBAs. There are instances where changes to minimum wages occur off cycle from CBA negotiations, in these instances the contractor is required to comply with the Fair Labor Standards Act (29 U.S.C.
206).

17 Are all UFCW Pension liabilities at Robbins reimbursable to contractor (i.e. Withdrawal
Liability)? This withdrawal liability can accrue to hundreds of thousands of dollars depending on the the individual employees length of service, and is leveid against the participating company at the end of the contract, and for some years to follow.
Not identified in UCFW
CBA but is part of their
Pension Fund provisions that do apply to company participating in the multi-employer Pension Fund.
The contractor will have to reach out to the Labor
Advisor concerning UFCW Pension liabilities and withdrawal liability.

23 The #43 answer states the SBA 8(a) Mentor/Protégé program will be considered as a "partnership". As a recognized partnership for a non-8(a) competition does this mean that the receipts for both companies will be appropriately added together in order to determine the partnership size status as small?
Q&A #43 The SBA is responsible for certifying the partnership...if you have an approved SBA MPA in place, you will be protected from affiliation and therefore the protégé’s size would only be considered.
Market Research Q&A
24 In the previous Q&A, PWS 2.10.3.1.4 was referenced for Polio Immunization and the government referred to the CDC website. It appears that there is still an error. I believe the government would like 2.10.3.1.4 to read, "Hepatitis B.
Contractor personnel are required to have a completed series of Hepatitis B Vaccinations. The contractor....." The CDC website does not require
Polio.
An Amendment will be forthcoming to address the revised PWS.

26 " a revolving line of credit not less than $2.5M, per regional submission (offerors submitting on more than one region may submit a single bank statement indicating a line of credit equal to all proposed regions)."
Contractor requests a revision such that the required line of credit per offeror shall be not less than $2.5M irrespective of the number of regions proposed as only one region per offeror will be awarded, therefore an aggregate line of credit equal to all proposed regions does not seem warranted.
52.212-1, pg 443 An amendment will be forthcoming to address that the required revolving line of credit, per offeror, shall be not less than $2.5M irrespective of the number of regions proposed.

28 Offerors shall submit documentation with their proposal indicating that the Offeror, including the
Contractor Teaming Arrangement, has adequate financial resources or the ability to obtain such resources to perform the requirement. At a minimum the documentation shall include a letter from the
Offerors financial institution and a bank statement indicating a revolving line of credit not less than
$2.5M, per regional submission (offerors submitting on more than one region may submit a single bank statement indicating a line of credit equal to all proposed regions).
What does the requirement for a $2.5M line of credit need to consist of? Is is strictly a line of credit and/or available capital within a financial institution OR could it be the summation of an available line of credit, available funds/capital, and lines of credit for supplies and equipment? The lines of credit for supplies and equipment could be provided from a contractor's vendors vs. a financial institution. Please clarify.

35 "The Government will lock Offerors into a single region, and may only participate in one region". Does this mean Offerors will only be allowed to submit for one region?
Page 442 of 489 last sentence.
The intent of the sentence was to address that contractors are limited to participating under one contractor teaming arrangement for the duration of the HAMS II contract, as stipulated in
Proposal Preparation Instructions, para. 2. An amendment is forthcoming that will remove this sentence to improve clarity/intent.
No, the Offerors are not limited to submitting a proposal to only one region. The RFP contains no limiting language to the number of regions to which offerors may propose, but rather contains instructions on which volumes may be submitted once to apply for all regions proposed against.
SF 1449
36 Where to find the SF 30 Amendment page 1 of acknowledgement for 2/22 and 2/24 Amendment postings?
Acknowledgment of

37 Financial institutions won't approve a $2.5M revolving credit line for a small business without them first having the contract(s) that support that amount of credit need, and/or have equivalent colateral. Is a promissory statement of a $2.5M credit line consideration pending the actual award of contract(s) sufficient to warrant and support that amount of a credit line acceptable? It would take more than the ceiling of $18M per year to support the need for revolving credit line of $2.5M.
Page 443 of 489 Instructions to Offeror
An amendment will be forthcoming to address that offerors shall submit documentation with their proposal indicating that the Offeror, including the Contractor Teaming Arrangement, has adequate financial resources or the ability to obtain such resources to perform the requirement.

49 We are questioning the 15 page limit for the
Technical Proposal. For the large amount of material being required; recruitment plan, retention plan, selection and continuity of key personnel, regional management support, a comprehensive training program, joint venture and teaming arrangements explained, transition planning, personnel management, equipment, supplies and all required deliverables - the 15 limit page is inadequate.
Offerors should be given the maximum opportunity to submit their best technical material for the in-depth review and evaluation by the government. In our opinion, the 15 page limit is unduly restrictive. We are requesting that the technical page limit be increased to 50 pages.
RFP Page 445 An amendment will be forthcoming to increase the 15 technical proposal page limit to 20 pages.

50 Which specific sub section(s) of FAR 6.202 is the agency relying on in making its determination to exclude particular sources from a contract action by limiting offerors eligibility to receiving only one regional award ?
RFP Page 452 The Government has made a determination, consistent with FAR 6.202(a)(1) and (4) that limiting offerors eligibility to one regional award will enhance competition and ensure a continuous pool of available vendors within the SB size standard.

53 How can a small business bid on a entire region, and stay a small business?
More than likely, the SB will graduate upon completion of the contract; however, that's typical for any large contract. What is important is that they are a certified SB at time of proposal and/or award. If a business is small at proposal time, then they are considered small on that contract, for the life of that contract.

55 Volume I Cross Reference Matrix, page 446 of
Instructions to Offeror. Please explain the governments expectation for the Cross Reference
Matrix. Is the government expecting a list of the contractors response to each of the PWS requirements referencing where they are found in
Volume I? If so, would the government consider excluding the matrix from the 15 page limit?
Page 446 The Government does not intend to stipulate how offerors format their proposals to address the
Government's technical evaluation criteria. An amendment will be forthcoming to increase the
15 technical proposal page limit to 20 pages.
