1. PURPOSE: The purpose of this Performance Plan is to establish procedures for monitoring, reporting and evaluating the performance of the contractor for work performed pursuant to the Performance Work Statement (PWS) for METIS.

2. OBJECTIVE: This Performance Plan is designed to provide the Multi-Functional Team (MFT) with a vehicle to ensure delivery of the desired mission support services within cost and on schedule. The Performance Plan will provide the Quality Assurance Personnel (QAP) with a systematic surveillance method for each item listed on the Services Summary.

3. DESIRED RESULTS: The MFT will strive to manage this performance-based acquisition through effective monitoring, evaluating, and reporting of contractor performance. The Government will maintain an accurate awareness of contract status and contractor work performance. The team will manage risk to ensure mission performance is within cost and schedule constraints.

4. ROLES AND RESPONSIBILITIES: The Multi-Functional Team (MFT) is a customer-focused team responsible for planning and managing the service acquisition throughout the life of the requirement. The MFT is responsible for planning and designing a mission support strategy that aligns mission support needs with a performance-based strategy.

Responsibilities are outlined as follows:

4.1 The Functional Commander/Director (FC/FD) is responsible for assigning competent and capable functional experts to the MFT who will be available full time or as warranted by the individual procurement. The FC/FD will assign, as a minimum, a primary and an alternate QAP and notify the Contracting Officer of any changes to this designation. The FC/FD will review contractor performance documentation, prepared by the QAP, on a regular basis to ensure performance is acceptable. The FC/FD is responsible for identifying mission essential services and developing the necessary documents in accordance with DoDI 3020.37, Continuation of Essential DoD Contractor Services During Crisis.

4.2 The Quality Assurance Personnel (QAP) provide quality assurance policy and guidance and ensure contract quality requirements, provisions, standards and thresholds are defined, practical, enforceable, necessary and verifiable.

The QAP responsibilities include (but are not limited to) the following:

- Evaluate and document a contractor’s performance in accordance with the Performance Plan,

- Notify the Contracting Officer (CO) of any significant performance deficiencies,

- Maintain surveillance documentation,

- Recommend improvements or changes to the Performance Plan, Services

Each QAP will be available to receive Phase I and Phase II training before they perform any QAP functions. This training should be completed before contract award but NLT 30 days after award.

4.3 The Contracting Officer (CO) is the only person with the authority to enter into contracts and contractually obligate the Government. The CO facilitates the MFT meetings and ensures all items on the checklists are discussed, documented and minutes are distributed. As the facilitator, the CO will ensure all members of the MFT work together to develop the acquisition. The CO has the overall responsibility for the day-to-day monitoring of the contractor’s performance in the areas of contract compliance, contract administration, cost control, and property control. The QAPC serves as a member of the MFT and participates in the development of the acquisition documents specifically ensuring the requirements are clearly stated and enforceable and assists in the development of the Services Summary (SS), especially the performance method(s) of assessment. The QAPC ensures the QAPs are properly trained on the contracting requirements associated with the quality assurance program and maintains Phase I and Phase II training documentation.

to the CO. Prompt feedback is essential for the contractor to develop and implement a corrective action plan. The contractor’s corrective action must be reported to the QAP and the CO for tracking purposes.

8. CERTIFICATION OF SERVICES: The Government expects the contractor to meet all of the requirements of the contract. The contractor expects timely payment for all services rendered. Certification of services is performed by the QAP via the Wide Area Work Flow (WAWF) system at https://wawf.eb.mil/. At the end of each billing period, service providers input invoice information in the WAWF system. WAWF automatically notifies the appropriate QAP via email of pending invoice. The QAP assesses WAWF and verifies accuracy of the contractor’s data. If data is correct, the QAP certifies the invoice and payment is conducted by the Defense Finance and Accounting Service. If the QAP encounters errors in the contractor’s invoice, the QAP consults with the contractor’s on-site manager to mutually rectify the data. If the contractor disagrees with the QAP, the issue is forwarded to the CO for resolution.

9. CUSTOMER COMPLAINTS: The QAP will gather all customer comments. The QAP will investigate negative comments and determine their validity. Valid negative comments will be discussed with the contractor and corrective action will be taken by the QAP and Contracting Officer. All customer complaint forms must be maintained in the QAP file.

10. LIST OF NON-SDS ITEMS SUBJECT TO OBSERVATION: The Government retains the right to inspect any item included in the contract. These items will be inspected periodically during the contract period.

11. CONTRACTOR RATING (CPARS): Contractor’s Performance Assessment Rating (CPAR) is an annual reporting requirement applicable to all service contracts valued over $1,000,000. Guidance on CPARS is available from the Department of the Air Force CPARS Guide and the AF ISR Agency A7KO representative or at the following web http://www.cpars.csd.disa.mil/.

12. REPORTING CHANGE IN FC/FD: The QAP must report to the Contracting Officer when there is a FC/FD change. QAP must include this as a reporting item on their monthly Quality Assurance Surveillance report. QAP must provide by name the current FD/FC and report if this is a change based on the current FC/FD QAP Appointment Letter.

13. CONTRACT DELIVERABLE TRACKING SYSTEM: Every service contract is required to have a contract deliverable tracking system. The following deliverables are prepared by the contractor and delivered to the QAP throughout the period of performance:
