Addendum to FAR 52.212-1, Instruc ons to Offerors SAF/FM Quality Assurance (QA) Program Support

1. All communica ons (ques ons, proposals, etc.) shall be submi ed via email to the Contrac ng Officer and Specialist:

a. Chris ne Dolan at Chris ne.dolan@us.af.mil

b. Bri ney Devallon at Bri ney.devallon@us.af.mil

2. Proposals shall consist of 3 separate parts:

a. Pricing Volume

b. Technical Capability Volume

c. Past Performance Volume

3. Pricing Volume (completed on the SF1449)

a. Terms & Condi ons – Offerors are instructed to complete blocks 12, 17a and 30a, b, and c of the SF1449. In doing so, the quoter accedes to the contract terms and condi ons as wri en in the solicita on, with a achments. The solicita on cons tutes the model contract.

b. Pricing – Insert proposed unit and extended prices in the Pricing Schedule for all Contract Line Items (CLINs) on the SF1449.

c. Representa ons & Cer fica ons – Complete the necessary fill-ins and cer fica ons in the Contract Clauses and Provisions sec on of the SF1449, including FAR 52.212-3, Offeror Representa ons and Cer fica ons—Commercial Products and Commercial Services.

4. Technical Capability Volume (10 pages max, 11pt font max) – This program is highly dependent on the contractor’s depth of knowledge and experience working with DAF FM processes, paired with web-based analy c site crea on skills. Therefore, the Technical Capability Volume shall be specific, detailed, and complete to clearly and fully demonstrate that the offeror has a thorough understanding of the requirements for the associated effort. The validity and thoroughness of the offeror’s Technical Capability Volume will be evaluated as evidence of the offeror’s understanding and capability to perform this requirement. Broad statements that the offeror understands, can or will comply with the PWS; or statements paraphrasing the PWS or parts thereof will be considered unacceptable. This volume shall contain no references to cost or pricing. The Technical Capability submission shall include, at a minimum, the following:

a. Program Management and Staffing Plan – This shall include, at a minimum:

i. A detailed management and staffing approach that illustrates an adequate plan to manage personnel throughout the life of the contract to ensure performance is accomplished IAW the PWS.

ii. An organiza onal structure, with descrip ons of proposed roles and responsibili es

iii. A staffing plan that addresses the labor mix of personnel currently employed and those projected for the requirement at hand

iv. A descrip on of offeror’s recruitment and reten on strategy to ensure employment of an adequate labor mix at all mes. This includes maintaining the necessary quan ty of qualified individuals with the requisite skillsets and experience at the appropriate level.

b. Transi on Plan – Offerors shall provide a Transi on Plan to illustrate in detail how they will mobilize for contract performance, ensuring no degrada on in service from the previous contract. Offerors are instructed to propose a 45-day transi on period at the start of the contract, during which me employee turnover and training with the incumbent contractor will commence. The Transi on Plan shall include, at a minimum:

i. Gan chart (or similar) outlining key mobiliza on ac vi es, including a plan to a) recruit,

b) train, and c) qualify and/or cer fy personnel during the 45-day transi on period that will result in a 100% staffed and qualified contractor workforce fully performing on the CLIN 0002 start date.

ii. If offerors do not believe a 45-day transi on is necessary, submit instead a detailed ra onale explaining why this is. The Government does not have resources to train new employees to perform these services at this me. Therefore, offerors’ ra onale for decreased or excluded transi on period should include their specific strategy for ensuring no gap in performance at the end of the current contract.

5. Past Performance Volume (5 references max; each 1 page max, plus CPARS if available) – Offerors are instructed to provide informa on ONLY for recent and relevant past performance accomplished on Government and commercial contracts. Accordingly, only informa on regarding contract performance within 3 years of solicita on and similar in scope and magnitude to this effort will be considered.

a. Past Performance Volume – Submit a list of no more than 5 recent and relevant contracts performed for Federal agencies or commercial customers that demonstrates the ability to successfully perform the scope and breadth of the requirements described in this solicita on’s PWS. The Past Performance Volume must contain the following informa on for each contract listed:

i. Company/Organiza on Name:

iii. Contrac ng Agency/Customer:

iv. Contract Number:

v. Contract Dollar Value:

vi. Period of Performance:

vii. Current Customer POC (Contrac ng Officer if available) Name & Email:

viii. Descrip on of compliance with contract terms & condi ons:

ix. Comments regarding any known unacceptable performance reported, and/or correc ve ac ons taken:

x. Explana on of how the scope of the referenced contract is similar to the work in this

PWS:

xi. Associated quality/sa sfac on ra ngs, including CPARS if available (a ach actual CPARS report, which does not count towards page limit):

b. Past Performance Ques onnaires (PPQ) – Offerors shall send PPQs, A achment 3, to each of the Customer POCs listed in their Past Performance Volume, reques ng that the Customer POCs complete and return the PPQ directly to the Government at Chris ne.dolan@us.af.mil and Bri ney.devallon@us.af.mil. It is the offeror’s sole responsibility to track the submission of PPQs and ensure the Government receives them not later than the due date and me established in this solicita on for offer submission.

c. No Past Performance – Offerors with no recent and relevant past performance experience must submit a statement iden fying this in their proposal submission.

d. Addi onal Sources of Informa on – The Government reserves the right to consider informa on regarding vendor performance from other sources not provided as part of the offeror’s proposal.

However, offerors will be afforded an opportunity to respond to any derogatory informa on not submi ed as part of the Past Performance Volume or PPQs.
