PAST PERFORMANCE QUESTIONNAIRE (PPQ)
INSTRUCTIONS FOR COMPLETING THE PPQ
1. This Acquisition is to provide the 844CS, National Military Command Center (NMCC) Operations Branch (844
CS/SCON) effective and timely Mission Critical engineering facility systems services to the NMCC No Fail Mission with first response above and beyond the normal facility services available from Washington Headquarters Services (WHS) at the Pentagon for the NMCC and Global Situation Awareness Facility (GSAF) 24/7/365.
2. As part of the source selection process, this questionnaire is provided to assist the Air Force District of
Washington Procurement and Contracting Office (AFDW/PKA) with the past performance evaluation of offerors pertaining to the competitive acquisition for the NMCC. Therefore, it is important that your information be as factual, accurate and as complete as possible to preclude the need for follow-up queries by the evaluators. In an effort to expedite receipt of the requested information, the Contracting Officer, Ramona L.
Hanson, and Contract Support, Lazarus Toussaint respectfully requests that you DO NOT MAIL hard copies.
Instead, please e-mail the completed questionnaire(s) to AFDW/PKA at reamona., no later than 30 October 2019.
3. Indicate, based on the adjectives below, the contractor’s performance on the identified program. Assessments should reflect only contractor liable performance. PLEASE PROVIDE A NARRATIVE EXPLANATION FOR ANY RATINGS OF E, VG, M or U.
