Base Access Requirements
A site visit is scheduled for:
· Solicitation#: FA665624B0002
· Project ZQEL 24-0004, Convert Corrosion Control Shop B302
· Location: 3976 King Graves Rd, Vienna OH 44473
· Date: 10 May 2024
· Time: 10:00 AM Eastern If you are planning to attend the site visit, please review the vetting process for a badge or pass (see attached – page 2). If any of those criteria are met, you will be denied access to the installation. The information required on the Entry Authority List (see attached - page 3) must be completed and submitted no later than 3 May 2024 by 2:00 PM Eastern and the names must be in alphabetical order. No exceptions will be given for additional time to submit this information. Due to Personally Identifiable Information (PII), please submit the information to SAFE at https://safe.apps.mil/. This is an encrypted website. You will need to contact Kim Hitchcock at kimberly.hitchcock.1@us.af.mil to request a code, then upload the information to the website.
DAY OF SITE VISIT:
· The Entry Authority List will be posted at the front gate. As long as you submitted your information by the deadline, your name will be on the list for the site visit. They will check that you are on the list and let you on base.
· Parking is located at B302.
· The site visit attendees will meet at B302. Please arrive no later than 10:00 AM Eastern on 10 May 2024.

In order to obtain a DBIDS card a favorable Criminal Background History (CBH) check obtained via the National Crime information Center (NCIC) must be completed for all contractors, subcontractors, and their employees prior to allowing access/entry to Youngstown Air Reserve Station. Individuals who have any of the following negative screening criteria may be denied access to the installation:
(1). The individual is known to be or reasonably suspected of being a terrorist or belongs to an organization with known terrorism links/support.
(2). The installation is unable to verify the individual's claimed identity.
(3). The individual has previously been barred from access to a federal installation or stand-alone facility.
(4). The individual is wanted by Federal, State, Local, or other civil law enforcement authorities, regardless of offense or violation.
(5). The individual has any conviction for espionage, sabotage, treason, terrorism, or murder.
(6). The individual’s name appears on any Federal or State agency’s watch list or hit list for criminal behavior or terrorist activity.
(7). The individual has been convicted of a firearms or explosive violation.
(8). The individual has been convicted of sexual assault, armed robbery, rape, child molestation, child pornography, trafficking in humans, drug possession with intent to sell, or drug distribution.
(9). The individual has knowingly and willfully engaged in acts or activities designed to overthrow the U.S. Government by force.
