SECTION J - LIST OF DOCUMENTS, EXHIBITS AND OTHER ATTACHMENTS

The following have been modified:

Attachment 1 - WAGE DETERMINATION (See Section I, Contract Clause 52.222-6.)

General Decision OH100003 Dated 06/03/2011, with modifications through Modification 20 dated 06/03/2011.

(See Section I, Contract Clause 252.236-7001)

Attachment 4 – RESPONSIBILITY QUESTIONNAIRE/FINANCIAL INSTITUTION REFERENCE SHEET.

MAKE THE FOLLOWING ADDITIONS TO THE SPECIFICATIONS:

01-00-00 General Requirements - Section 1.6 - Contractor Use of the premises.

H. Phasing Plan. There is a preferred phasing plan for the building for the contractor (see Phasing Plan attachment for illustrations)The areas in pink (number 4) have to be open during each UTA for their designated use. Therefore the contractor will have roughly 30 days to get those spaces ready for use after construction is started in that area assuming construction is started immediately after any given UTA weekend. Room 140 in Area 1 must be completed before Area 2 can be started as equipment from Area 2 will be moved to that room and will be expected to be ready for use upon movement of that equipment. Work in Room 132 and Room 131 may be accomplished during the timeframe that Area 4 (Pink area) is under construction or during the timeframe that Area 2 is under construction (Green area) depending on the Contractors preference. However, there are no changes to the phasing plan, but be advise it will be subject to change due to mission requirements.

07-41-13 Metal Roof Panels – Section 1.10 - Warranty B.2. Provide a no dollar limit (NDL) total system performane warranty from a single source for 20 years for insulation, membrane, flashing, and miscellaneous materials, including coverage for damage from winds up to 72 MPH.
