2.1. QUALITY CONTROL
The Contractor shall develop and maintain a quality control program (QCP) to ensure custodial services are performed in accordance with commonly accepted commercial practices and services identified in this PWS. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-performance and continual repeat of defective service does not occur. As a minimum, the contractor shall develop quality control procedures addressing the areas identified in paragraph 2, Service Summary. The Contractor shall establish and maintain a complete QCP that ensures services are maintained and performed IAW this contract, applicable laws and regulations to reach the objective stated in this PWS. The QCP should address all aspects of quality to include responsibility for surveillance, description of records to be kept, and corrective actions to be taken to remedy unsatisfactory performance. A copy of the contractor's QCP shall be submitted with the Contractors
Quote. Proposed changes to the plan shall be provided to the Contracting Officer (CO) prior to implementation. The contractor shall submit monthly metrics reports to the Contract Administrator
CO, QAPC, (Quality Assurance Program Coordinator) and COR within 7 calendar days from the beginning of each month. Metric reports shall include date of inspection, building number, type of inspection, discrepancies, corrective actions, inspector’s name, and comments.
2.1.1. DEFINITION OF DEFECT
For purposes of this PWS and the Service Summary, a "defect" is defined as any single instance in which a contractor-performed service does not meet the applicable standard described in this PWS, as measured against the Air Force Common Output Level Standards (AFCOLS) cleaning standards referenced in Appendix A. A defect is counted per individual task, per individual facility (building number), per inspection event or validated customer complaint.
2.2. QUALITY ASSURANCE
The government shall inspect and evaluate the contractor’s performance to ensure services are received in accordance with the requirements set forth in the contract. The CORs will use the contractor’s work schedule or modified version thereof, to record validation results. The results of the validation then become the official Air Force record of the Contractor’s performance. When a standard performance has not been met, or contractor performance has not been accomplished, the COR will initiate and provide the CO with a Contract Discrepancy Report (CDR) for issuance to the contractor. The contractor shall respond to the CDR IAW instructions provided and return it to the CO within 10 calendar days of receipt.
2.3. PERFORMANCE ASSESSMENT
2.3.1. PERIODIC INSPECTIONS
This method requires the CORs to employ a “spot check” style of evaluation based on the contractor’s incorporated Cleaning Frequency Schedule (CFS, PWS paragraph 4.21). Periodic validations will be conducted on a scheduled basis (daily, weekly, monthly, quarterly, semi-annual or annually) and may be adjusted, based on quality trends. The CFS or modified version thereof shall be used for CORs scheduled inspections and results documentation. Any unsatisfactory inspection result shall be recorded, and the Contractor shall acknowledge receipt and respond within 2 hours of notification.
'Respond' means the contractor shall acknowledge the deficiency, dispatch personnel to assess and begin corrective action, and communicate an estimated completion time to the COR. The contractor shall complete re-performance within the timeframe specified in the applicable Performance Objective of the Service Summary. The 2-hour response requirement does not supersede or replace the re-performance completion deadlines established in the Service Summary; rather, it establishes the maximum time within which the contractor must initiate corrective action. Exceeding the performance threshold defects for any of these performance objectives in any one-month period shall result in a warning or letter of concern from the CO. In turn, the CO will take any required action, if deemed necessary. Exceeding the performance threshold of these performance objectives, in any combination for any two, or more consecutive or non-consecutive months during a contract period shall constitute an immediate Progress Meeting with the Multi- functional Team. All remedies shall be in accordance with FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance.
2.3.2. CUSTOMER COMPLAINTS
The CORs will receive and investigate complaints. The CORs shall be responsible for initially validating customer complaints. The CO shall make final determination of the validity of customer complaint(s) in cases of disagreement with the CORs and the Contractor. Validated customer complaints shall be re-performed as per Service Summary Remedy.
2.3.3. PERIODIC QUARTERLY PROGRESS MEETINGS
The CO, Functional Commander, CORs, and other government personnel as appropriate, and the
