1.0. DESCRIPTION OF SERVICES:
1.1 BACKGROUND: The purpose of this effort is to acquire non personal healthcare services to provide medical aseptic housekeepers to implement the United States Air Force, Hospital Aseptic
Management System (HAMS) at United States Air Force Medical Treatment Facilities (MTF), 48th
Medical Group (48 MDG), Royal Air Force (RAF) Lakenheath, United Kingdom. Services performed under this Performance Work Statement (PWS) shall comply with established standards, Air Force directives, and British Standards. Compliance with all standards and directives will be evaluated during the quality assurance and inspection process as outlined in AFI 63-101, Performance Based Services Acquisition.
1.2. SCOPE: The contractor shall furnish all personnel, supervision, tools, materials, equipment, transportation and other replenishment supplies and services necessary to provide cleaning services and healthcare waste disposal services for all facilities listed in Attachment C of this PWS. Quality of services shall be consistent with Association for Professionals in Infection Control and Epidemiology
(APIC) and The Joint Commission (TJC) guidelines to ensure the MTF’s commercial standard custodial services are performed at 48th Medical Group, RAF Lakenheath, to include Aerospace
Medicine and Dental Clinics at RAF Mildenhall and the Logistics Warehouse and Veterinary Clinic at RAF Feltwell, in accordance with this PWS. In accordance with best commercial practice, a micro-fiber cleaning system will be used throughout wherever practical. The Operational Procedures
Manual (OPM) shall define the contractor’s methods of accomplishment. As medical operations include a wide range of services from inpatient to outpatient care, the HAMS program is designed to accommodate differing work hours (7-days or 5-days a week) and differing drivers for cleaning (case driven, non-case driven, patient check-out, etc.). Further, HAMS requires the use of an Infection
Control Council (ICC) approved disinfectant solution to destroy surface borne pathogens and keep them from spreading throughout the MTF thru Heating Ventilation and Air Conditioning systems to limit healthcare associated infections. This HAMS program also has provisions for management of linen service and healthcare waste.
1.2.1. The area cleaned, by square feet, will be designated in the Room Listing, Attachment C2, when this contract is awarded. Variances in area greater or less than 5%, due to construction, acquiring new space, etc. will require a contract modification.
1.3. CLEANING SERVICES: The contractor shall accomplish all custodial tasks to satisfy the minimum cleaning standards stated in this PWS. Note that these are the minimum requirements and the contactor is encouraged to apply innovative techniques and managerial approaches that will yield increased levels of service and greater cleaning efficiencies.
1.3.1. Basic Cleaning Services. All rooms/areas shall be pre-inspected and receive cleaning necessary, protection, and beautification in accordance with the individual Contractor's written procedures OPM which shall specify the proper solutions, and mixtures appropriate to the intended use for room/area location within the MTF. This PWS employs a “Total Clean” concept. Upon arrival at an area for cleaning, the contractor will survey the area to determine which tasks must be accomplished to restore it to “total clean” as part of daily scheduled inspection and cleaning. Other than cycle task items, the entire area will be clean when the contractor finishes. Interior windows
(including between panes when accessible), walls, floors, door vents, airway louvers, behind radiators, protruding parts of window/ceiling/wall air conditioners, and any other item or surface that can be touched from the room interior are included. Total clean includes trash collection, re-supplying consumables dispensers (including soap, air freshener, toilet paper, paper towels, alcohol hand gel and hand lotion), dusting, floor maintenance (wet/damp/dust mopping, vacuuming/wet vacuuming, buffing, stripping and refinishing floors, and carpet maintenance), furniture maintenance, spot cleaning, and cleaning glass, utility fixtures, wheelchairs, artificial plants, drinking fountains, public latrines, entrance platforms, patios, stairwells, government furniture and equipment, blinds, and light fixtures. Respiratory stations will be resupplied as needed to include alcohol hand gel, facial tissues and masks.
1.3.1.1. Remove Trash. All trash bins located inside and outside within the Medical Group campus shall be emptied. Some exterior bins are exempt, see map in attachment E, exterior services. The interior and exterior of all trash bins must be clean and free of any kind of debris. Soiled or torn plastic trash/ bin liners shall be replaced with serviceable proper fitting liners. The trash shall be deposited in the nearest designated storage area for collection and/or deposited in the dumpster/skips.
Cleaned trash bins shall be returned to their designated location. Bags shall not be visible on bins equipped with a liner and lid.
1.3.1.2. Clean Glass/Mirrors/Doors. Clean all interior glass, along with adjacent trim, including doors, interior wall windows, partitions, walls, display cases (exterior only), directory boards, and exterior of entrance doors. Where double glazing allows access to the interior surfaces, these are included. After glass/mirror cleaning, there shall be no traces of film, streaks, dirt, smudges, water, or other foreign matter.
1.3.1.3. Surfaces. The contractor shall clean all surfaces to include; non-invasive medical equipment, all furniture including but not limited to: bedside tables, desks, conference tables, chairs, televisions, fans (outer surfaces), table lamps, wall-mounted lamps and medical equipment, computer equipment, cable runs, windowsills, window blinds, window screens, ledges, handrails, wall-mounted items etc. At no time will contractor personnel move books/paper/records or personal effects to clean under them. With the exception of wall-mounted items, dusting of medical staff personal effects is not required. Upon completion of task, services shall be free of dust, lint, smudges and any foreign matter. Medical equipment attached to a patient will not be touched by contractor personnel. All horizontal and vertical surfaces from floor to ceiling, unless expressly exempted, shall be dusted to present a clean appearance.
1.3.1.4. Any surface not bearing carpets shall be swept, dust mopped, damp mopped, wet mopped, dry buffed, and/or spray buffed as applicable to ensure they have a uniform, glossy appearance and are free from dirt, debris, dust, scuff marks, other stains and discoloration, and other foreign mater.
Baseboards/skirting boards, corners, wall/floor edges and under entrance mats shall also be clean.
All floor maintenance solutions shall be removed from baseboards/skirting boards, furniture, trash bins, etc. Chairs, trash bins, and other moveable items shall be moved or tilted to maintain floors underneath these items. All moved items shall be returned to their original and proper position. At no time will the floor be overly wet. Cleaning of floors shall be performed in a way that prevents medical staff and patients from crossing wet floors. The contractor shall ensure that “wet floor” signs are posted and visible at all times floors are wet. When a wash/vacuum machine is used in corridors, waiting rooms and entrance areas, special precautions shall be taken to prevent any hazards to patients, medical staff and visitors. Upon completion of task, floors and baseboards/skirting boards shall be free of dirt, trash, scuff marks, dust balls, and foreign matter and shall have a uniform, shiny appearance. Any hazard warning signs posted will be removed when floors are dry and stored in a housekeeping storage area.
1.3.1.4.1. Carpet Maintenance. Carpet maintenance (spot cleaning) shall be carried out as part of
“total clean” as required. Additionally, the contractor shall develop a schedule for cleaning all carpeted areas at least once a year. Full cleanings that are carried out as part of “total clean” may count toward the annual requirement; where this causes a deviation from the submitted schedule prior approval from the COR. See paragraph 1.3.3.3.
1.3.1.4.2. Vacuuming. Vacuuming shall be done in such a manner that no dust is raised and is done with mechanical vacuum equipment on carpeted areas. All tears and raveling shall be brought to the attention of the Contracting Officer’s Representative (COR) on the day they are noticed. Floors shall be free of dirt, dust, film, stains, and debris after completion of task.
1.3.1.4.3. Disinfectant Application. Disinfectant shall be applied when required by the ICC.
1.3.1.4.4. Walk-Off Mats and Runners (Contractor Supplied – See Room Listing, Attachment C2).
Interior mats and runners shall be properly cleaned according to their composition. Soil and moisture shall be removed from the area underneath mats and runners before they are returned to their normal location. They shall be made of material or treated with a chemical, which is dust and dirt attracting, must lay flat; and cover entry/exit points and heavy traffic areas in the buildings shown in the room listing. After vacuuming or cleaning, mats shall be free of all visible lint, litter, soil and other foreign matter.
1.3.1.5. Stairways and Lifts.
1.3.1.5.1. All stairway surfaces shall be cleaned in accordance with paragraph series 1.3.1.4., as appropriate for floor covering. Grease, soil, and grime shall be removed from stair guards, handrails and baseboards. Contractor shall remove all marks, dirt, smudges, scuffs, and other foreign matter from adjoining stairwell walls to provide or maintain a clean, uniform appearance.
1.3.1.5.2. Lift interior surfaces, including all light fixtures, ventilator slots, emergency phone boxes and floor door guide tracks of passenger and service lifts shall be cleaned as often as necessary to maintain them in a polished and presentable appearance. Other than spot cleaning, lifts will be cleaned after normal duty hours. Only one car is to be out of service for cleaning at a time, and it shall be cleaned at the lowest floor level only.
1.3.1.6. Ceiling/Light Cleaning. The contractor shall ensure all light fixture covers and ceiling mounted light fixtures are clean and free of insects. In case the light fixture needs to be removed so that cleaning can be performed then the contractor must ensure that it is reinstalled in the original location once the task is completed. The contractor shall also clean all tiles or surfaces around the air vents. Upon completion, the area should be free from dust, cobwebs and other debris.
1.3.1.7. General Spot Cleaning. Spot cleaning shall be performed on a continual basis, and immediately upon identification of a discrepancy to the identified standards. Spot cleaning includes but is not limited to removing or cleaning smudges of fingerprints, marks, streaks, spills, etc. from washable surfaces including, but not limited to walls, partitions, vents, grillwork, doors, door guards, door handles, push-bars, kick-plates, light switches, climate controls, fixtures, carpets, and hard floors. After spot cleaning is performed, the item shall have a clean uniform appearance, free of streaks, spots and other evidence of soil. Gum, tape/sticker residue or any other sticky substance on floors, furniture or any other surface shall also be removed. Carpet spot-cleaning shall be done in accordance with manufacturer’s recommendations or standard commercial practices. When the stain will not lift under any circumstances, the COR will be notified in writing.
1.3.1.8. Basic Restrooms/Locker Rooms Cleaning Services.
1.3.1.8.1. Clean and Disinfect. Completely clean and disinfect, with an ICC-approved disinfectant, all surfaces of sinks, toilet bowls, urinals, lavatories, showers, shower mats, dispensers, plumbing fixtures, partitions, doors, shower curtains, walls, toilet brushes, toilet brush holders, and other such surfaces. Clean all types of furniture to include chairs, benches, tables and any other furniture-like items placed in restrooms. Disinfect all surfaces of partitions surrounding urinals, toilets, sinks, stalls, stall walls, entry doors, including floors, handles, kick plates, ventilation grates, metal guards, tile walls, and wall areas adjacent to wall mounted lavatories, urinals, and toilets. After cleaning, surfaces will be free of deposits, dirt, streaks, mold, mildew and unpleasant odors. Where indicated on the room listing, attachment C2, provide and service feminine hygiene disposal units, nappy disposal units, toilet seat sanitizers and air fresheners. (Note: some air fresheners are in areas other than restrooms/locker rooms)
1.3.1.8.2. De-scale Shower, Toilet Bowls and Urinals. De-scaling with an ICC-approved chemical shall be performed as required to keep all areas free of scale, soap films, mold, mildew and other deposits. After de-scaling, surfaces shall be free from streaks, stains, scale, scum, mold, mildew, bodily waste deposits, rust stains, etc.
1.3.1.8.3. Clean/Disinfect Showers. The contractor shall scour and disinfect shower with an ICC-approved disinfectant to include walls, shower curtains/ rod, door, pipes, fixtures, benches and floor mats. Any benches within the showering area shall be cleaned. Upon completion of the task, the surfaces shall be free of scale, mildew, calcium deposits, water spots, streaks, smudges, soap residue and any foreign matter.
1.3.1.8.4. Clean Sinks, Drinking Water Fountains, and Ice Machines. Clean and disinfect with an
ICC-approved disinfectant all porcelain and polished metal surfaces, including the orifices and drain, as well as exterior surfaces of fountains and ice machines. Sinks, ice machines, and drinking water fountains shall be free of residue, deposits, watermarks, streaks, stains, spots, smudges, scale, and other soil and foreign matter.
1.3.1.8.5. Re-Supply Restroom Supplies/Clean Dispensers. Contractor shall ensure restrooms are stocked sufficiently with contractor-supplied consumables (listed in Attachment C3) such as soap, toilet paper, paper towels, and air deodorizers. Supplies shall be stored in designated cleaning cupboards. At no time will “spare” toilet paper or paper towels be placed in the room other than in designated dispensers. All restrooms shall contain soap, toilet paper, paper towels, toilet brush/holder and air deodorizer. Upon completion of re-stock, all dispensers, including internal surfaces, will be free of soap, streaks, dust and build-up.
1.3.2. Cleaning Requirements by Area and Frequency Indicated in the Room Listing (Attachment
C2) and Floor Plans (Attachment D).
1.3.2.1. Zone 1 - Low Risk (General Administrative Areas). The contractor shall perform all basic cleaning services as outlined in paragraphs 1.3.1 through 1.3.1.8.5. (Five days per week, one shift daily Monday – Friday).
1.3.2.2. Zone 2 – Medium Risk (Patient Care and Ambulatory Care Areas). The contractor shall perform all basic cleaning services as outlined in paragraphs 1.3.1 through 1.3.1.8.5. (Five days per week, one shift Monday – Friday).
1.3.2.3. Zone 2A – Medium Risk (Patient Care and Ambulatory Care Areas). The contractor shall perform all basic cleaning services as outlined in paragraphs 1.3.1 through 1.3.1.8.5. (Seven days per week, one shift Monday – Sunday).
1.3.2.4. Zone 3 – Medium Risk (Kitchen Area). The contractor shall perform all basic cleaning services as outlined in paragraphs 1.3.1 through 1.3.1.8.5. In addition, the contractor shall damp sweep and mop the kitchen floor and empty trash 3 times a day, following every meal. Cleaning shall begin within one hour of each completed meal. (Seven days per week, Monday through
Sunday, three times daily).
1.3.2.5. Zone 4 & 5 - Medium Risk (Public Toilets). The contractor shall perform all basic restroom cleaning services as outlined in paragraph 1.3.1.8.
1.3.2.5.1. Zone 4 – Five days per week, two times daily Monday – Friday at least 4 hours and no more than 6 hours apart.
1.3.2.5.2. Zone 5 – Seven days per week, two times daily Monday – Sunday at least 4 hours and no more than 6 hours apart.
1.3.2.6. Zone 6 - Very High Risk (Labor/Delivery/Operating Rooms, Endoscopy Room and
Associated Areas). The contractor shall perform all basic cleaning services outlined in paragraphs
1.3.1. through 1.3.1.8.5. in the operating suite to include the hallway that is located behind the operating room and in the endoscopy room. An ICC approved disinfectant will be used. (Seven days per week – Monday through Sunday to include pre-cleaning, between procedures cleaning and end of day cleaning).
1.3.2.6.1. Daily Cleaning Requirements. In addition to the coverage requirements stated above, one person shall be dedicated to the Operating Suite between 0630 and 1530 hours, Monday to
Friday.
1.3.2.6.2. Pre-Cleaning. Pre-cleaning shall be performed prior to start of day/use of the area. By
0630 hours all operating rooms shall be cleaned as follows: The floor and all horizontal surfaces will be disinfected. Horizontal surfaces include, but are not limited to: back table, ring stands, mayo stand, prep stand, IV poles, horizontal surfaces of anesthesia machines, overhead lights, kick buckets, and operating room bed.
1.3.2.6.3. Between Surgical Procedure Cleaning: The following surgical, labor, and delivery room clean up tasks shall be performed at the end of each scheduled or unscheduled case and each morning:
1.3.2.6.3.1. Between surgical procedures the following surfaces will be disinfected: back table, ring stands, Mayo stand, prep-stand, IV poles, overhead lights, kick buckets, horizontal surfaces of the anesthesia machine, operating room bed, and stretchers.
1.3.2.6.3.2. Replace all linen on beds and transport stretchers, clean operating room beds and transport stretchers with ICC-approved disinfectant, place dirty linens in designated linen bags and place linen bags in designated collection point. Replace linen bag in hamper.
1.3.2.6.3.3. Disinfect floors by either wet vacuuming or damp mopping.
1.3.2.6.3.4. Replace plastic liners in kick buckets and trash hamper.
1.3.2.6.3.5. Spot clean walls and doors.
1.3.2.6.3.6. Empty all trash to include healthcare waste.
1.3.2.6.4. End of Day Cleaning: In addition to the tasks required above, the Contractor shall accomplish the following tasks after the last surgery or labor/delivery room at the end of the normal hospital duty hours for the area.
1.3.2.6.4.1. Disinfect faceplates of all vents and doors of cabinets and operating theaters using fresh
ICC-approved disinfectant.
1.3.2.6.4.2. Clean scrub sinks.
1.3.2.6.4.3. The floors and horizontal surfaces in the hallway and sub-sterile rooms behind the red line shall be disinfected using ICC-approved disinfectant.
1.3.2.6.4.4. Clean all walls.
1.3.2.6.4.5. Clean all window glass and mirrors.
1.3.2.6.5. Emergency Cases After Normal Duty Hours. If there are emergency cases after normal duty hours, the Contractor shall perform “Between Surgical Procedures Cleaning” when necessary and “End of Day Cleaning” IAW paragraphs 1.3.2.6.3. and 1.3.2.6.4.
1.3.2.6.6. Recovery Room (M-F 0800-1630 and as needed after hours). Recovery room finishes approximately one hour after the Surgical Suite closes. Cleaning will be accomplished after the last patient of the day vacates the room.
1.3.2.6.6.1. Empty all trash bins/medical waste bins and replace liners at end of day.
1.3.2.6.6.2. Disinfect all horizontal surfaces with ICC-approved disinfectant. Include operating room beds, transport stretchers, window sills, tape dispenser, tops of electrical sockets, oxygen and suction fixtures, tops of medicine cabinets, open counters, top of refrigerators, file cabinets, desks, stretchers, and curtain rails.
1.3.2.6.6.3. Clean sinks and fixtures.
1.3.2.6.6.4. Wet vacuum with ICC-approved disinfectant, moving all station unit carts after last patient is removed from recovery.
1.3.2.6.6.5. Wipe down all baseboards/skirting boards with ICC-approved disinfectant
1.3.2.6.6.6. Wipe down walls with an ICC-approved disinfectant once weekly with no more than seven days between cleanings.
1.3.2.6.6.7. Replace curtains every six months or when soiled. Curtains shall be tagged showing the date of change.
1.3.2.7. Zone 6A Very High Risk (Invasive Procedures Areas). The contractor shall perform all basic cleaning services, as outlined in paragraph 1.3.1 through 1.3.1.8.5.
1.3.2.7.1. Contractor personnel must be available throughout the normal duty day for immediate between-case terminal clean. (Five days per week, Monday through Friday, available for immediate between procedures cleaning and end of day cleaning.)
1.3.2.7.2. Between Surgical Procedure Cleaning: The following invasive procedure area clean up tasks shall be performed at the end of each scheduled or unscheduled case and each morning:
1.3.2.7.2.1. Between procedures all horizontal surfaces will be disinfected.
1.3.2.7.2.2. Replace all linen on beds and transport stretchers, clean operating room beds and transport stretchers with ICC-approved disinfectant, place dirty linens in designated linen bags and place linen bags in designated collection point. Replace linen bag in hamper.
1.3.2.7.2.3. Disinfect floors by either wet vacuuming or damp mopping.
1.3.2.7.2.4. Replace plastic liners in kick buckets and trash hamper.
1.3.2.7.2.5. Spot clean walls and doors.
1.3.2.7.2.6. Empty all trash to include healthcare waste.
1.3.2.7.3. End of Day Cleaning: In addition to the tasks required above, the Contractor shall accomplish the following tasks after the last procedure at the end of the normal hospital duty hours for the area.
1.3.2.7.3.1. Disinfect faceplates of all doors, vents and doors of cabinets using fresh ICC-approved disinfectant.
1.3.2.7.3.2. Clean sinks.
1.3.2.7.3.3. The floors and horizontal surfaces disinfected using ICC-approved disinfectant.
1.3.2.7.3.4. Spot clean walls.
1.3.2.7.3.5. Clean all window glass and mirrors.
1.3.2.8. Zone 7 – High Risk (In-Patient Areas). The contractor shall perform all basic cleaning services, as outlined in paragraph 1.3.1 through 1.3.1.8.5. (Seven days per week, Monday through
Sunday to include Terminal Cleaning as required.)
1.3.2.8.1. Terminal Discharge Cleaning
1.3.2.8.1.1. Whenever a patient is discharged or relocated within the hospital, the following requirements must be met prior to a new patient being admitted to the room. In private rooms, the basic cleaning services outlined in paragraphs 1.3.1 through 1.3.1.8.5 using ICC-approved disinfectant must be completed. In multi-bed rooms, the linen changing (including curtains) and cleaning of the bed, closets and drawers shall apply to those used by the discharged patient only.
1.3.2.8.1.2. Contractor shall remove linen from the bed and place in laundry bag. Contractor shall also remove any trash left on the bed by patient before placing linen in laundry bag.
1.3.2.8.1.3. All mattress surfaces must be cleaned. Clean and disinfect entire bed including the top, bottom, and four sides of mattress, the wheels, bed control unit, rails, headboard and footboard.
Damp wipe of pillow can only be done if pillow is enclosed in plastic cover. Report any tears in protective coverings and in mattress protective outer covering to the Unit’s Medical Staff. The contractor shall clean and disinfect telephones/nurse call devices and bedside stands inside and out.
Upon completion of this task, all interior and exterior surfaces shall be cleaned and disinfected and the bed made. The contractor shall notify the unit’s medical staff when cleaning is complete.
1.3.2.8.1.4. Discharge cleaning shall not take place if patient’s belongings are still in the room. The unit’s medical staff shall be notified in such a case.
1.3.2.8.1.5. Time requirements. During day and evening shifts, the contractor shall begin terminal cleaning within 60 minutes of notification by the medical staff. Where a patient is discharged or transferred after 2130 hours, terminal discharge cleaning will begin upon arrival of the day shift.
1.3.2.9. Zone 8 – High Risk (Emergency Department). The contractor shall perform all basic cleaning services as outlined in paragraphs 1.3.1 through 1.3.1.8.5. In addition, two cleanings will be performed per 24 hour period, with one taking place in the day shift, Monday through Sunday. Also perform terminal clean as required. (Seven days per week, Monday through Sunday. Two cleanings per 24 hour period, with one taking place during the day shift. Terminal cleaning as required.)
1.3.2.10. Zone 9 – Very High Risk (Isolation Rooms). When these rooms are used for infectious cases, they will be cleaned according to isolation room procedures. Otherwise they will be cleaned according to standard in-patient procedures, as outlined in the OPM. (Note: in case of a standard room becoming infected, it will be cleaned according to Zone 9 procedures.) Contractor shall have someone available for immediate cleaning within 15 minutes of request. (Seven days per week, Monday through Sunday. Only if in use as isolation room. Notification of room use will be provided by MSU medical staff or the COR)
1.3.2.10.1. Unit’s medical staff will designate whether cleaning is required to Zone 7 or 9 standards.
1.3.2.10.2. Isolation room procedures may change during the course of the contract according to
Association of Professionals in Infection Control and Epidemiology (APIC) best practice.
1.3.2.10.3. Unless directed by the unit’s medical staff, isolation rooms shall be cleaned after all other patient rooms.
1.3.2.11. Zone 10 – High Risk (Central Sterile Supply and Dental Instrument Processing
Centers). All surfaces will be cleaned daily; all horizontal surfaces including the floors shall be disinfected with an ICC-approved disinfectant prior to the duty day. (Five days per week, Monday through Friday)
1.3.2.12. Zone 11 - High Risk (Healthcare Waste & Linen Storage) (One day per week, same day every week unless COR is notified)
1.3.2.12.1. The contractor shall clean and disinfect all surfaces to include walls and racks.
1.3.2.12.2. The contractor shall maintain the floor as outlined in paragraph. 1.3.1.4.
1.3.2.13. Zone 12 - Low Risk (Exterior Services)
1.3.2.13.1. The contractor shall remove cobwebs, mud, dirt, bird droppings and litter from the outside walls, canopies, entrance lights and the immediate vicinity of the entrances and patios.
1.3.2.13.2. Walkways, footpaths and kerbs. The contractor shall sweep and wash down, as needed but at least quarterly, the walkways, footpaths, and along kerbs, within the limits shown in attachment E, Exterior Services/Snow and Ice Control Zones, except when the ambient temperature drops below one degree Celsius (33 degrees Fahrenheit). In the event the temperature drops below one degree Celsius, the contractor shall be required to salt and clean any ice/snow from all sidewalks entrances and patient entrance walkways surrounding the facilities. The contractor shall ensure walkways, footpaths and kerb areas are kept free of dirt, built-up bird droppings, leaves, rubbish, and debris, to include pulling weeds from walkway, footpath and kerb areas.
1.3.2.13.2.1. Remove snow and ice from indicated walkways and footpaths so that entire pathway is clear from end to end. Pathways shall be cleaned and safe for pedestrian traffic, see Attachment E.
Removal is to be completed by 0700, 30 minutes before the beginning of the facility day shift (0730), and maintained as necessary to prevent an accumulation of more than two inches of snow. Snow deposited or thrown on sidewalks by street maintenance crews shall be removed by the Contractor as necessary to ensure safe pedestrian traffic. Ice control shall consist of removal of ice and/or application of ice melt, as necessary, during both day and evening shifts to maintain safe conditions for pedestrian traffic.
1.3.2.13.3. Water plants in designated planters around the buildings, (under the building 932 canopy, outside the Emergency Room Entrance, on the north-west corner of building 932, and in front of building 955), pull weeds, and replenish plants as necessary to maintain a visually appealing appearance. Water can be obtained from the tap outside of building 933 or in housekeeping cupboards.
1.3.2.13.4. Loose bulk items (grass, dirt, leaves, rocks, trimmings, etc.) will be disposed of in designated dumpsters after being placed in large rubbish bags prior to disposal.
1.3.2.13.5. Cigarette Butt Receptacles (Posted on campus perimeter, see map in attachment D). All containers shall be emptied and cleaned leaving them free from dust, ash, foreign matter and cigarette butts.
1.3.2.13.6. Service Yard. The contractor shall police the service yard daily, Monday through Friday, removing any litter, leaves or debris within the confines of the fence. (The yard shall be swept monthly under “Cycle Tasks”, see 1.3.3.)
1.3.2.13.7. Pavilion. The contractor shall police the pavilion area, to include all areas around tables daily, removing any litter, leaves or debris. (The Pavilion areas shall be power washed monthly under
“Cycle Tasks”, see 1.3.3.)
1.3.3. Cycle Tasks: A cleaning schedule shall be provided to the COR no later than 5 working days before the beginning of the month, showing the anticipated cleanings.
1.3.3.1. Exterior Windows: This includes cleaning all glass surfaces on the exterior of the MTF, including “blanked out” panels, frames and sills. Initial cleaning (all facilities) shall take place within the first 2 months of the contract award.
1.3.3.1.1. Semi-Annually - Patient care facilities shall be cleaned
1.3.3.1.2. Annually – Non-Patient care facilities shall be cleaned.
1.3.3.2. Light Fixtures: Annually - This includes cleaning inside ceiling lights, emergency exit lights, patio lights, outside entrance lights, interior lighted signs, canopy lights, delivery and operating room lights, and all other lights (except desk, table, bed, floor). This cleaning is in addition to the cleaning of the outside of the light fixtures performed as part of daily scheduled inspection and cleaning, and to the removal of insects which is on an as-required basis.
1.3.3.3. Carpet Cleaning: Annually - All carpeted areas shall be cleaned at least annually.
Shampooing is accomplished with a deep extraction machine. Carpet shall be free of all types of soil, stains and loose carpet fibers. This is separate from spot cleaning which is part of “total clean,” as defined in paragraph 1.3.1.4.1. Contractors invoice shall specify rooms/square footage cleaned and billed against the carpet cleaning CLIN.
1.3.3.4. Registers, Diffusers, and Grills: Annually - Interior portions shall be cleaned as specified in this PWS. Exterior portions of registers, diffusers, and grills shall be cleaned as part of daily scheduled inspection and cleaning.
1.3.3.5. Service Yard: Monthly - The service yard shall be swept to remove soil, debris and leaves from the paved surfaces and kerbs within fence.
1.3.3.6. Pavilion: Monthly - The paved areas in the pavilion area shall be swept to remove soil, debris and leaves from the paved surfaces. Tables and benches shall be cleaned monthly to remove soil and bird droppings.
1.3.3.7. North Fire Escape Tower: Monthly - Sweep stairs, landings and pathway to remove soil and debris. Remove cobwebs from walls, ceilings, railings and around doorways. Remove any bird droppings.
1.4. LINEN SERVICES: The HAMS contractor shall manage, schedule, and perform the requirements listed below.
1.4.1 Linen Inventory:
1.4.1.1. Semi-annual Inventory: A physical inventory of linen shall be performed at the beginning of the contract and at least every six months thereafter. However, an audit must be performed and the results provided to the COR not later than 1 August of every year, to coincide with the government end of fiscal year purchasing.
1.4.1.1.1. A six month inventory report shall be provided to the COR within 15 calendar days after the linen inventory.
1.4.1.1.2. The report shall identify all linen on hand, new linen received, and old linen salvaged since the previous inventory.
1.4.1.2. Monthly Report: Prepare and submit a monthly report of linen on hand.
1.4.1.2.1. A Medical Linen Supply Record shall be maintained for each line item in the inventory.
1.4.1.2.2. Five workdays after the end of the month, a written report showing the quantity of each item on hand shall be provided to the COR.
1.4.1.3. Daily Linen Services.
1.4.1.3.1. All linen items shall be weighed by HAMS contractor personnel at the time of pickup/delivery by the laundry contractor.
1.4.1.3.2. HAMS contractor personnel shall verify the quantities of items received from the laundry contractor to be consistent with the delivery tickets. Delivery tickets shall be forwarded to the COR, and shall include a separate list of all discrepancies noted. The laundry contractor may modify the delivery tickets in pen to obtain the concurrence/verification by HAMS contractor personnel.
1.4.1.3.3. Clean linen shall be stored in the central clean linen area, with its plastic wrapping intact, in accordance with facility infection control guidelines.
1.4.1.3.4. Linen shall be delivered, with its plastic wrapping intact, in designated clean linen bins with lids in place to the departments according to the schedule in Attachment C4. At no time will clean linen be moved on top of a linen bin lid. Linen bins shall be cleaned weekly with an ICC approved disinfectant.
1.4.1.3.5. HAMS Contractor staff will top up each ward /department’s linen store directly, to include outlying buildings, with the identified linen issue quantities according to the identified delivery frequencies, see Attachment C4, Linen Schedule. The amount of linen delivered shall be recorded by department and provided, along with the laundry contractor’s delivery/pick-up receipts, to the COR within 5 working days of the following month.
1.4.1.3.6. At the point of delivery, all previously existing linen will be refolded and straightened as required. New linen will be removed from its plastic wrapping, checked for legibility of laundry marker and serviceability, and placed on the appropriate shelf.
1.4.1.3.7. Linen that is torn, permanently stained or excessively worn will be placed in a salvage holding area for examination by the COR and accounted for on the monthly report.
1.4.1.3.8. Linen with illegible markings will be returned to the linen room and remarked.
1.4.1.3.9. HAMS Contractor staff will return all empty linen bins to the linen room for use the following day. Clean linen shall not be transported or stored in the same rooms, carts, bins, lifts, or shelves used for the storage or transportation of soiled linen. Further, clean linen bins are to be used for clean linen only. They are not to be used for soiled linen, rubbish or to move any other items.
1.4.1.4. Linen Stock/Issue Levels
1.4.1.4.1. Linen stock and issue levels will be constantly reviewed based on continuing usage patterns within each ward/department, and amended by either increasing or decreasing the quantities.
Agreed ward/department quantity lists will be maintained and serviced with variation to quantity being approved by the COR.
1.4.1.4.2. Ward / Departmental staff may request additional quantities/ items of linen in response to local ad hoc requirements, by speaking to linen services staff when they are topping up the linen stocks or by contacting the HAMS Contractor.
1.4.1.4.3. Clean and soiled linen shall not be handled at the same time by one person. Linen personnel who handle soiled linen shall change all PPE (outer garments and shoes/shoe covers) and wash their hands before handling clean linen.
1.4.1.4.4. All soiled linen shall be placed in laundry bags provided by the MTF. Once closed, bags shall not be opened. Bags shall only be opened by the laundry facility contractor personnel.
1.4.1.4.5. In the event of a medical emergency, the HAMS contractor shall replenish depleted linen levels on an on call basis.
1.4.1.4.6. All new linen shall be marked with the name of the medical facility and sent to the laundry facility immediately upon removal from the original shipping packages or no later than five days after receipt.
1.5. REGULATED HEALTHCARE WASTE DISPOSAL SERVICES
1.5.1. Healthcare Waste Handling:
1.5.1.1. MTF personnel shall place all healthcare waste into appropriate containers/bins.
Healthcare waste shall be collected and removed by HAMS contractor personnel in a transport cart separate from general refuse and shall be delivered to a locked storage area designated in the Room Listing. Waste shall be stored in this area until destroyed, treated, or picked up by a licensed contractor (If the waste is transported in the bins provided by the medical waste disposal contractor, those bins will be thoroughly cleaned with a pressure washer and ICC-approved disinfectant prior to entering the facility). Contractor personnel must wear appropriate
PPE during waste handling. Chemotherapy waste designated as healthcare waste by the Facility
Waste Management Program shall be picked up by contractor personnel and treated in the same manner as healthcare waste. Contractor personnel shall also pick up healthcare waste from outlying MTF buildings.
1.5.1.1.1. Prior to placement in transport bins, all bags and containers will be tagged using a plastic tag containing a unique tracking number. The tracking number will be logged on a HAMS contractor developed, department log-sheet showing the date, time, producing department, the housekeeper’s initials, and the transport bin tag identifier. The log sheets will be maintained for the life of the contract.
1.5.1.1.2. Movement of healthcare waste from outlying MTF buildings consists of transporting small quantities (under 333 kg/L) to the waste storage area located next to the main hospital. Transport drivers must be given documented awareness training and the vehicle must carry the appropriate number of fire extinguishers as required by ADR/UK legislation.
1.5.1.2. All waste bins/containers and holding areas shall be cleaned daily, and as needed.
Cleaning of the healthcare waste storage area shall be accomplished using an ICC-approved disinfectant to ensure floors, walls and ceilings are clean. Cleaning of the area/room containing the healthcare waste treatment or destruction equipment shall also be accomplished on the same schedule. Waste holding areas shall be kept secure.
1.5.1.3. Healthcare waste shall be handled, treated, destroyed, packaged and labeled, and disposed of as specified in this PWS, USAFE 23-104. USAFE Command Dangerous Goods Program, The
Final Governing Standards – UK (FGS-UK) and/or the Accord Européen relatif au Transport
International des Marchandises dangereuses par Route (ADR) (European Agreement Concerning the International Carriage of Dangerous Goods by Road, and applicable UK legislation. If there is a conflict between standards, the most stringent will be followed.
1.5.1.3.1. In accordance with (IAW) the FGS-UK, paragraph C8.3.8., all infectious healthcare waste shall be separated, stored and transported in bags or receptacles of 3 mils (75 microns). A test certificate for bags provided by the Contractor shall be provided to the COR prior to use.
1.5.1.4. Healthcare waste is to be disposed of off-site by the contractor. HAMS personnel shall package healthcare waste in containers that are rigid, leak-resistant, and impervious to moisture
(containers to be supplied by the waste contractor). Containers shall be sealed to prevent leakage and handled in a manner that shall prevent bursting or tearing during transport. Sufficient absorbent material shall be placed in containers to further prevent leakage, as required. Sharps containers shall also be placed in similar containers. Liquid containers shall be placed in break-resistant and tightly sealed packages. All exterior containers used shall display the appropriate hazard symbol. All exterior containers shall be marked in accordance with UK and European guidelines.
1.5.1.4.1. The HAMS contractor shall act as the packer and certifier IAW ADR, for all healthcare waste shipments. The contractors shall ensure safety obligations as consignor, consignee, carrier, loader, unloader, and driver are executed IAW Ch 1.4 ADR, and as implemented by the Statutory
Instrument (SI) 1348/as amended by SI 1885. The 48 MDG will be considered the “shipper.”
1.5.1.4.2. In the event a Unit Dangerous Goods Advisor (UDGA) identifies non-compliance to safety obligations delegated to the contractor, the pickup of waste will be stopped at no cost to the government.
1.5.2. Healthcare Waste Incineration/Disposal Services
1.5.2.1. Healthcare waste incineration/disposal services for RAF Lakenheath will include removal and disposal of bio-hazardous waste, offensive waste, sharps containers, anatomical waste, cytotoxic and cytostatic medicines, and non-hazardous pharmaceuticals.
1.5.2.1.1. Waste will be collected twice weekly, at least 2 days apart, either Monday and Thursday or
Tuesday and Friday. As such, there will be no requirement on the part of the government or the
HAMS contractor to submit an individual request for waste pickup, unless Category A infectious waste is to be moved.
1.5.2.1.2. Contractor will submit information required for the instruction in writing to the carrier by traceable means or electronically and ensure drivers moving HM/HW IAW table 1.1.3.6, Ch. 3.4 or
Ch. 3.5 ADR receive a safety briefing based on the instructions in writing for this item. If the safety briefing was not provided by the carrier, ensure a copy of the safety briefing is filed with the movement documents. Waste Consignment notes will be signed by a properly trained and authorized
HAMS contractor staff member. Original copies of the notes will be provided to the COR not later than close of business of the day received.
1.5.2.1.3. The waste disposal company will provide a “quarterly return to producer”, IAW Hazardous
Waste Regulations, accompanied by detailed “burn reports” to the COR within 30 calendar days of the end of quarter.

2.1.1. Task Requirements:
2.1.1.1. Administrative: The Contractor shall accomplish all administrative tasks necessary to ensure performance of the aseptic services, including but not limited to preparation and distribution of work schedules, emergency recall rosters, facility defects reports, and all other data submissions listed below.
