FA5240-16-T-0001 Attachment 1 STATEMENT OF WORK 1 Oct 2015

Andersen AFB Concrete IDIQ

1. Delivery Hours:

Normal operating hours are 7:00 A. M. through 5:00 P.M. Monday through Friday. Delivery will be made within fourty-eight (48) hours of order unless otherwise directed by a Contracting Officer. Deliveries for

Saturday or Sunday require an advanced fourty-eight (48) hour notice. Concrete trucks will arrive at a set time, pre-arranged between the contractor and government representative. The contractor and government representative will determine truck delivery intervals for orders requiring more than one truck. Upon delivery of any material, tickets shall be provided for each load to the government representative.

2. Availability:

Contractor will provide year round availability of 2500 PSI through 6500 PSI concrete mix and all other materials listed in the solicitation. Additionally, the contrator must meet all quantities ordered for each

Pricing CLIN listed in the solicitation.

3. Additional Charges:

There will be no additional charge for truck stand-by time and there will be no set minimum order quantities per truck. Only delivery order constraints set in basic contract will be authorized.

4. Quality Control Test Reports:

Contractor shall provide quality control test results of all materials in accordance with the specifications for each mixture type upon request of a government representative.
