About this file

This Performance Work Statement (PWS) details Asbestos Containing Material (ACM) testing requirements for the Koke'e Astronomical Observatory in Hawaii. The project involves comprehensive testing of buildings 1, 5, 10, and 14100, covering approximately 20,000 square feet, with specific focus on floors, walls, pipes, ceilings, upstairs support section, downstairs radar tech offices, kitchen/common area, and bedrooms of building 1.

The contractor must be EPA Model Accreditation Plan (MAP) accredited, follow EPA/OSHA guidelines, and use a National Voluntary Laboratory Accreditation Program (NVLAP) approved lab. Deliverables include an Asbestos Inspection Report, Laboratory Analysis Reports (using Polarized Light Microscopy), potential Asbestos Abatement Plan, Chain of Custody Forms, and an Updated Asbestos Management Plan. All work must be completed within 15 business days of site access, with all documentation submitted to the 611th CES Squadron. The project is part of a solicitation (FA521625R0004) by the Department of the Air Force Pacific Air Forces, with a targeted completion date of 27 February 2025.

HAZARDOUS MATERIAL

TESTING

Koke’e, HI Kauai Astronomical Observatory, 49X3+6W, Hanalei, HI 96746

611th CES Squadron 27 FEB 2025

1. Scope: This Performance Work Statement (PWS) outlines the necessary procedures for conducting Asbestos Containing Material (ACM) testing in buildings 1, 5, 10, and 14100 located on Koke’e.

1.1. Background: Asbestos has been a known issue at the Koke’e location since a report completed in 1995. The report revealed asbestos levels ranging from 2-7% in the floor tiles of buildings 1, 5, 10, and 14100. These levels are significant, as asbestos exposure can pose serious health risks. Personnel at the Koke’e location have reported various medical conditions and issues related to asbestos exposure. Asbestos exposure can lead to both cancerous and non-cancerous diseases.

2. Applicable Documents:

2.1. References: EPA Guidance

3. Requirements:

3.1. General Requirements

3.1.1. Unless otherwise noted in writing by the CO, the CO is the only person with the authority to change, modify, deny or approve any part and has sole authority for all aspects of contract, SOW’s, contract changes, approvals, payments and project changes.

3.1.2. The Contractor shall provide all material, equipment, labor, supervision, housing, food, and transportation for material and to/from the job site required to accomplish the requirements of this PWS.

3.1.3. The contractor shall be responsible for all personnel and material transportation to and from the job site and any accommodations for the duration of the project.

3.1.4. The contractor shall adhere to all NFPA, OSHA, EPA and all federal, state and local regulations while performing the requirements of this PWS.

3.1.5. All ACM testing, in addition to the regulations listed above, must follow the Asbestos Hazard Emergency Response Act (AHERA), Asbestos Containing Materials in Schools Rule, and Asbestos National Emission Standard for Hazardous Air Pollutants (NESHAP).

3.1.6. Contractor responsible for following Installation Design Guide.

3.1.7. Contractor may be required to provide all their equipment fuel requirements.

3.1.8. All demolished and scrap materials used/generated in this project must be removed and disposed of in accordance with site policy.

3.1.10. Third Party Support: The Government will not furnish materials or equipment to support the contactor performing ACM sampling and testing work at the Koke’e project site. The contractor will be solely responsible for developing and planning the project specifications.

3.1.11. Billeting: Billeting expenses are to be covered by the contractor.

3.2. Specific Requirements:

3.2.1 The Contractor shall submit workers for site access to the COR and contracting office. The Contractor will coordinate with local government personnel regarding access to each facility prior to sampling and testing activities.

3.2.2 The Contractor must be accredited under the EPA Model Accreditation Plan (MAP) or equivalent state/local standards for each building.

3.2.4 The Contractor must utilize a National Voluntary Laboratory Accreditation Program (NVLAP) approved lab for the sample report.

3.2.5 The Contractor will undertake a comprehensive assessment and sampling procedure, which includes ACM (Asbestos-Containing Materials) testing for buildings 1, 5, 10, and 14100, with a total area of approximately 20,000 square feet. Adequate sampling will be conducted on the floors, walls, pipes, ceilings to ensure the government receives thorough and accurate information for each facility. Contractor will ensure adequate testing is done in the upstairs support section room, downstairs radar tech offices, kitchen/common area, and the bedrooms of building 1 and the results for these areas should be specifically denoted in the report. See attached for a Map of the buildings.

4. Submittals:

4.1. All project submittals must be accompanied by a completed AF Form 3000, Material Approval Submittal. All AF3000 will be submitted to the CO for signature. If there is a change in process or material, different from what was proposed and awarded, the CO or PM’s signature on the AF3000 does not constitute an approved change order. Any changes must be coordinated with the CO prior to submission of the AF3000 for signature.

4.2. The Contractor shall submit to the Contracting Officer the following Schedule timeline within 15 days of award for all major project milestones.

4.3. All sitework completed and testing results submitted to government within 15 business days of contractor having access to installation.
