FOR
INSTALLATION & PURCHASE OF
PREFABRICATED FACILITY
YOKOTA AIR BASE,
JAPAN

SECTION 1
DESCRIPTION OF SERVICES/GENERAL INFORMATION
1.0. DESCRIPTION OF SERVICES. The contractor shall provide all necessary labor, personnel, supervision, management, tools, equipment, materials, and any other items and services necessary to install one (1) prefabricated facility (hereafter, referred as the facility) located at Yokota Air Base (YAB) in accordance with (IAW) this Performance Work Statement (PWS), commercial standards, manufacturer standards, Japanese ordinances and laws, and the attached appendices.
1.1. FACILITY TYPE. The contractor shall install one (1) 418 sq meter (27.4m X 15.24m) facility exterior to Building 584 IAW Appendix A. An additional modular facility will be provided and placed next to the facility noted in Appendix A on a separate contract and is annotated in the appendix. The
Government will accept Japanese adjustments to facility sizes to match the module variations.
Installation of the facilities shall be completed by 1 December 2018.
1.2. INSTALLATION SPECIFICATIONS.
1.2.1. The facility shall be constructed of materials to sustain commercial office use for at least the life of the contract. The facility shall have flat-type ceilings, insulated walls, windows, fixed and/or sliding doors, floor panels, access stairways and door canopies. The modular enclosures shall form a continuous, uninterrupted weather tight shell over the structure including flashing/sealed joints. Floors shall be able to support at least a 181.4 kg (450 lb) point load.
1.2.2. All interior and exterior walls shall be constructed with non-combustible material with a commercial thermal insulation appropriate to maintain a temperature range between 20 to 22 degrees C, year-round. The exterior walls should be rated for the local elements and a design wind speed of 200 kph.
Any perforations will need to be sealed and weatherproofed. Outdoor facility must include gutters, down spouts, and splash blocks. The facility must meet applicable commercial seismic requirements. Interior walls shall be rated for STC-31 sound attenuation.
1.2.3. Finished floor height shall be at least 304 mm above the existing ground level and have floors intended for high traffic areas.
1.2.4. Canopy, exterior lighting, metal stairs, and landing shall be provided at each exterior entrance.
Exterior entrance doors shall have metal hinges. All interior and exterior doors shall be key lock, and have at least two (2) keys provided to the customer.
1.2.5. All windows shall be able to open and include screens and blinds. Requested locations are identified on the attachments.
1.2.6. Recessed raceways shall be provided that allow for the installation of electrical, telephone, and/or
LAN wiring at requested locations outlined in the attachments. 1/2" (115 mm) deep junction boxes shall be installed in the appropriate locations. The installation of the raceways shall provide for a seamless transition on the floors and walls and provide adequate space for any future additional cabling. The raceways shall have removable cover plates.
1.2.7. A multi evaporator system shall be provided for both heating and cooling in the facility. The size and installation of building space conditioning equipment shall be IAW UFC 3-410-01 Heating, Ventilating, and Air Conditioning Systems (most current version as of date of award). Equipment must be able to maintain the space at 22 ℃/72F in the summer and 20℃/68F in the winter. Commercially available HVAC systems are acceptable the facility of para 1.1 above. Industrial filters shall be provided.
The contractor shall recommend locations, size, and specification of the units to the Contracting Officer’s
Representative (COR) for approval.
1.2.8. The contractor shall provide bathrooms in the temporary squadron operations facility. The women’s bathroom will consist of two (2) showers, two (2) toilets, and two (2) sinks. The men’s bathroom will consist of four (4) showers, two (2) toilets, two (2) urinals, and two (2) sinks. The facility shall also have two (2) water fountains and a sink. The contactor shall tie the facility into the existing water and sewer lines on the site and reference UFC 3-420-01 Plumbing Systems (most current version).
The tie-in locations are listed in Appendix A.
1.2.9. The Government is contemplating awarding all, some, or none of the modifications in this paragraph. Pricing shall be submitted separately in accordance with the solicitation.
1.2.9.1. (Option 1) The contractor shall provide perimeter doors constructed of metal with X-10 locks installed in accordance with Federal Specification FF-L-2740.
1.2.9.2 (Option 2) The contractor shall ensure that all out-swing perimeter doors have hinges that are peened, brazed, or spot welded to prevent removal.
1.2.9.3. (Option 3) All penetrations in the exterior of the facility shall be kept less than 96 square inches or be hardened in accordance with Military Handbook 1013/1A.
1.2.9.4. (Option 4) The contractor shall install a Vindicator V5 Intrusion Detection System (IDS) and the
IDS shall detect any unauthorized penetration in to facility. The IDS equipment shall be located inside the facility and panels shall be located near the main entrance. The contractor shall provide motion detection sensors at each possible entrance and sensors shall be connected to the IDS. The contractor shall protect each perimeter door with a balanced magnetic switch (BMS) that connects to the IDS and meets the standards of UL-634.
1.3. ELECTRICAL SYSTEM SPECIFICATIONS.
1.3.1. Electrical installation shall comply with all requirements specified by the National Fire Protection
Association (NFPA), No. 70, National Electrical Code, Chapter 5, Special Occupancies, Article 590, ‘Temporary Installations’ and the Japan Electric Association (JEAC).

1.3.2. The contractor shall provide and install electrical distribution wiring, fixtures, outlets, cables, wiring, accessories, and molded case circuit breakers. Electrical systems shall also include cable wiring, panel boards, fluorescent lighting, wall switches, and receptacles. The contractor shall provide and install exterior lighting. The contractor shall provide electrical service from existing main distribution panels located next to installation site.
1.3.3. The facility shall be wired with Cat 6 communication wire. Communication ports and power outlets shall be of US style and be located every 2 m, with at least two (2) power outlets and two (2) communications ports. Each facility will require a wall mounted quarter communications rack with a power outlet inside the rack. It will require a conduit near the communications rack so that fiber optic cable can be run from outside the facility into the communications rack. Wiring should be installed within walls or with a clean finish utilizing wire mold.
Power outlets and panels can be either surface or flush mounted.
1.3.4. Ground rods and wire conductors for equipment grounding shall be installed IAW Article 590 of
National Electrical Code and Japan Electric Association (JEAC) standards.
1.3.5. Ground-resistance measurements shall be certified by the contractor and provided to the COR upon completion of electrical system installation. The data provided will include the measured ground resistance of each grounding rod, the rod’s location, grounding system used, soil conditions, and noted resistance at the time of measurement. Ground-resistance measurements shall be made in normal dry weather, not less than 48 hours after rainfall, with the underground test location isolated from other grounding rods. If applicable, the contractor shall dig a hole and shall remove frozen soil to make dry conditions for the measurements. The ground resistance shall be measured using the ‘fall-of-potential’ method before the operating test and shall not exceed 25 ohms when measured in normal dry conditions.
1.4. SUBMITTALS.
1.4.1. The contractor shall provide submittals in both English and Japanese to the COR for approval for each item listed below within the number of days specified for each submittal. The descriptive information shall be supported by catalogs, certificates of compliance, or other data published by the manufacturer to demonstrate conformance with specification requirements prior to installation.
1.4.2. Building Shop Drawings. The contractor shall provide building shop drawings, with bracing structural locations, to the COR within 30 calendar days after contract award. The drawings shall include floor plans (based on the above specifications), a reflected ceiling plan, HVAC drawings, wall/ceiling section details, anchorage detail to existing concrete slabs, and appropriate installation details, as required.
1.4.3. Materials and Equipment Manufacturer’s Data. The contractor shall provide the below materials and equipment manufacturer’s data to the COR within 30 calendar days after award.

1.5. FACILITY ACCEPTANCE INSPECTION. The CORs will inspect the facility within 15 calendar days following the installation of each building. If any discrepancies are found during the inspection, the
CORs will notify the contractor to perform corrective actions. The contractor shall correct the discrepancies within four (4) calendar days.
2.0. GENERAL INFORMATION.
2.1. CONTRACTOR PERSONNEL.
2.1.1. Contract Manager. The contractor shall provide a contract manager who shall be responsible for all requirements in this PWS. The name of this person and an alternate(s) who shall act for the contractor when the manager is absent, shall be designated in writing to the KO. The contract manager or alternate shall have full authority to act for the contractor on all matters relating to this contract. The contractor shall provide the contract manager’s names, addresses, and telephone numbers. The contractor shall maintain a current listing of employees, providing updates as changes occur. The contract manager or alternate shall be proficient in English and have a good understanding of the requirements pertaining to this contract for interpretation and implementation of directives (written and oral), issued in English by the KO or COR. If the contract manager or alternate do not meet these requirements, the contractor shall provide a translator for them. The contractor manager shall be present during the entire installation and removal periods.
2.1.2. Contractor Employees. Contractor personnel shall be easily recognizable as contractor employees. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges which contain the contractor’s company name and the employee’s name. The contractor shall ensure personnel have the appropriate license and certifications for each trade
(i.e. fire protection, electrical, plumbing, etc.) and be able to provide a copy of the license upon COR request.
2.1.3. Removal of Contractor’s Employees. The contractor shall utilize only experienced, responsible and capable employees in the performance of this contract. The KO may require that the contractor remove any of his/her employees who endanger persons or property, or whose continued employment under this contract is inconsistent with the interests of the U.S. Government.
2.1.4. Other Country Nationals (OCN). The security forces on each base control entry to the base.
Under certain circumstances, the OCN might be denied entry. It is the contractor’s responsibility to coordinate with security forces at each base when there is a question about particular entry circumstances.
2.1.5. U.S. Government and/or Master Labor Contract Employees. The contractor shall not hire employees of the U.S. Government and/or Government of Japan Master Labor Contract employees in violation of the Procurement Integrity Act. See FAR 3.104. Furthermore, the contractor shall not employ any Department of Defense employee, either military or civilian, unless such person seeks and receives approval in accordance with Department of Defense 5500.7-R, Joint Ethics Regulation.”
2.1.6. Military Personnel. The contractor is cautioned that off-duty active military personnel hired under this contract may be subject to permanent change of station (PCS), change in duty hours, or deployment.
Military Reservists and National Guard members may be subject to recall to active duty. Abrupt absence of these personnel could adversely affect contractor’s ability to perform; however, their absence at any time shall not constitute an excuse for non-performance under this contract.
2.2. QUALITY CONTROL. The contractor shall utilize its existing quality control program to ensure the services are performed IAW commonly accepted commercial practices. The contractor shall develop and implement procedures to identify, prevent and ensure non-recurrence of defective services. At a minimum, the contractor shall develop quality control procedures addressing the areas identified in paragraph 2, Services Summary.
2.3. QUALITY ASSURANCE. The government will evaluate the contractor’s performance IAW the
Quality Assurance Surveillance Plan (QASP) by appointing a COR to monitor performance to ensure services are received.
2.4. GOVERNMENT REMEDIES. The KO shall follow FAR 52.212-4, Contract Terms and Conditions-
Commercial Items, for the contractor’s failure to perform satisfactory services or failure to correct non-conforming services.
2.5. CONTRACTING OFFICER (KO) AUTHORITY. Only the KO has authority to enter into, administer, terminate or modify the terms and conditions herein. Any changes made by persons other than the KO will be considered an unauthorized commitment and subject to ratification actions in accordance with FAR 1.602-3.
2.6. CONTRACTING OFFICER’S REPRESENTATIVE (COR). The contractor shall coordinate all aspects of this requirement with the COR. Completed work shall be subject to inspection and acceptance by COR. This designation will be made in writing by the KO at the time of contract award, or prior to commencement of COR duty. The COR shall NOT direct or redirect any contractor action(s). This can only be done by the KO.
2.7. AVAILABILITY OF FACILITY.
2.7.1. Hours of Service. The contractor’s personnel will perform scheduled work on weekdays, Monday through Friday (exclusive of the U.S. legal and other designated holidays listed below), during normal duty hours, 0700 - 1700 hours unless prior arrangements have been made with the CORs.

2.8. SECURITY AND SAFETY REQUIREMENTS.
2.8.1. Installation/Base Passes. The U.S. Government will issue all required Installation/Base Passes and Controlled Area Badges to contractor personnel at no cost to the contractor. Vehicle insurance certificates are required to obtain Base Passes and must cover the automobile liability insurance in the types and minimum amounts of (a) Bodily Injury - ¥30,000,000 per person per occurrence, in addition to the Japanese Compulsory Insurance (JCI), and (b) Property Damage - ¥3,000,000 per occurrence. The prime contractor shall be responsible for all passes issued to his/her own or subcontractor’s employees under this contract. The contractor shall return all applicable passes upon termination of employment or completion of this contract to the Japanese Pass Section, and a certificate of receipt shall be obtained.
NOTE: Contractor personnel are required to comply with U.S. Air Force instructions for personnel pass requirements. Contractors are also required to comply with U.S. Air Force instructions for vehicle pass requirements. These regulations are available for the selected contractor’s review upon personnel and/or vehicle pass issuance.
2.8.2. Security Requirements. The contractor shall ensure each employee obtains all the required passes and identification items for access to U.S. Government’s installations. These items are issued by the
Security Forces Pass and Registration Section. The contractor and employees shall return identification cards to Security Forces within 24 hours of termination or completion of this contract. All other
Government identification for terminated employees shall be returned to the KO within 24 hours of termination.
2.8.3. Off Limit Signs. The contractor shall provide warning and/or ”OFF LIMITS” signs in both
English and Japanese and post them at the entrances of jobsite, and barricades around the jobsite while the equipment and/or systems are being serviced. The contractor shall control all personnel desiring access into jobsite areas when work is in progress.
2.8.4. Photography Permission. The contractor shall obtain COR approval prior to commencing any photography.
2.8.5. Reports of Damages, Destruction, Injury, or Death
2.8.5.1. The contractor shall report all incidents of damage or destruction to property, or personal injury or death to any person, arising out of and during the performance of this contract. The term “property” includes equipment, freight, or property of any type, whether Government owned or not. The term
“contractor” includes the contractor, its agents, servants or employees.
2.8.5.2. The contractor shall submit these reports to the KO through the COR as follows:
(a) A preliminary report shall be made as soon as practicable after the time the incident occurs. The report may be either verbal or facsimile.
(b) A complete written report shall be submitted within 4 days (96 hours) after occurrence of the incident. A copy of this report shall be furnished to the COR.
2.8.6. Security Danger Warning, Accident Prevention, Fire Prevention and Fire Protection
Requirements. The contractor shall comply with all pertinent requirements of the Occupational Safety and Health Administration basic requirements or laws of Japan, whichever is more stringent, to ensure compliance by all subcontractors.
2.8.7. Environmental Regulations. The contractor shall comply with all U.S. Government and Japanese environmental laws (whichever are most stringent) to protect the health and safety of employees/ residents, and preserve the local and global environment. The contractor shall comply with the Japan
Environmental Governing Standards (JEGS) and shall minimize or eliminate environmental pollution and energy/utility waste.
2.8.8. Delivery and Storage of Materials. All materials delivered and placed in storage shall be protected from the weather, humidity and temperature variations, dirt and dust, or other contaminants by the contractor.
2.8.9. Disposal of Materials. All debris and unserviceable materials generated under this contract such as industrial waste, non-recyclable materials and equipment/material not listed on hand-receipts shall be disposed of off-base by the contractor in accordance with the laws of Japan. Hand receipt items require turn-in action by the contractor when the listed items have to be replaced.
2.8.10. Responsibility for Damages. Any damages caused to U.S. Government property during the course of performing this contract shall be restored, replaced and/or repaired to its original condition at no additional cost to the U.S. Government.
2.9. CLEANLINESS. The contractor is responsible for cleanliness of the service areas. This includes, but is not limited to, washing, sweeping, dusting and other janitorial services necessary to maintain the area in a clean and sanitary condition. The contractor shall furnish such supplies as required.
2.10. CONSERVATION OF UTILITIES. The contractor shall instruct employees in utility conservation practices. The contractor shall be responsible for operating under conditions, which preclude the waste of utilities. The contractor shall comply with additional energy policies upon notification by the KO.
SECTION 2
SERVICES SUMMARY
The following Services Summary identifies the performance objectives and performance thresholds for critical tasks associated with providing support services for this requirement. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success. As a general rule, defects are categorized as follows: (1), ‘Major’ defects that present significant safety or health hazards to customer or contract employees (e.g.
improperly braced or attached exterior stairs) and (2), ‘Minor’ defects that though inconvenient or annoying, do not impede or delay mission accomplishment (e.g. missing light fixture or plug receptacle).
