STATEMENT OF WORK

FOR

APPLIANCES

AT

CLEAR AIR FORCE STATION, AK

DATE: 9 SEPTEMBER 2019

DESCRIPTION OF SERVICES: Purchase, transport, and deliver appliances to Clear AFS warehouse building 250.

SCOPE OF WORK:

1.1.1 Non-personal services to provide all tools, materials, equipment, personnel, supervision and transportation necessary to deliver new appliances to warehouse building 250 in a condition for proper storage; in accordance with the SOW and Appendix 1.

2.0 GENERAL INFORMATION

2.1 ACCESS TO GOVERNMENT PROPERTY AND FACILITIES: The Contractor will be allowed non-escorted access in the dormitory common areas. The Contractor shall be escorted in all occupied resident dorm rooms by Site personnel.

2.1.1 The Contractor will provide a Security Pass list that indicates all personnel who will be working on this project. The list will be submitted to the site entry POC Mr. Ricky Houck 907-585-6360, ricky.houck.ctr@us.af.mil once an installation and delivery schedule is established.

2.1.2 It will be the Contractor’s responsibility to update the list as personnel changes occur. A minimum of 14 days is required to vet personnel. The list will be printed on Company letterhead and contain the full name, full date of birth, Social Security Number, driver license number and state of issue. Background checks will be accomplished on all listed personnel.

2.1.3 Contractors will require Real ID compliant credentials to enter the installation.

2.2 PROJECT LOCATION: The project site is buildings 210 and 250, Clear AFS, AK 99704. Please keep the sites remote nature in mind as the nearest fuel station is more than 20 miles away.

2.3LODGING: Onsite lodging is not available.
2.4HOURS OF OPERATION: The normal hours of operation will be 8:00 A.M to

4:00 P.M. Monday through Friday, excluding holidays.

2.5 FEDERAL HOLIDAYS: The following are recognized Federal Holidays. The Contractor will not be required to work on these days.

2.6DAMAGES: Contractor is liable for the repairs or replacement of all damage incurred during delivery and transportation.
3.0PERSONNEL

3.1 Contract Manager: The Contractor shall provide a contract manager and an alternate who shall be responsible for the performance of the work. The name of this person and an alternate shall be designated in writing to the site POCs no more than a week after contract award. The contract manager or alternate shall have full authority to act for the Contractor on all contract matters relating to the daily operations of the contract. The contract manager and alternate must be able to read, write, speak, and understand English.

3.2 Contractor Personnel: Contractor personnel shall be courteous, professional, present a neat appearance and be easily recognized. This includes wearing distinctive clothing bearing the name of the company or by wearing appropriate badges that contain the company name and employee name.

5.3Monitors: Performance of the service for installation and removal shall be monitored by the onsite POCs or their representative.
4.0POINTS OF CONTACT:
TSgt John Charron, 213 SWS Contracting Officer Representative and Mr. Joseph Rango, Wolf Creek Federal Services Manager are the onsite POCs. They will be available throughout this project to provide any assistance required to complete this project. 5.0 PERIOD OF PERFORMANCE: The period of performance for this contract is no more than 120 days from date of award to completion.
