PERFORMANCE WORK STATEMENT
FOR
OPERATION, MAINTENANCE, AND REPAIR
WASTEWATER PRETREATMENT SYSTEMS
EIELSON AIR FORCE BASE, ALASKA

PERFORMANCE WORK STATEMENT
FOR
OPERATION, MAINTENANCE, AND REPAIR OF
WASTEWATER PRETREATMENT SYSTEMS
1. DESCRIPTION OF SERVICES
1.1. GENERAL INFORMATION: The contractor shall provide all management, tools, supplies, equipment, and labor necessary to operate, maintain, and repair wastewater pretreatment systems at Eielson Air Force Base (EAFB), Alaska. The contractor shall operate, service, clean, maintain, and repair wastewater pretreatment systems (oil-water separators) and related equipment (grit traps and connecting drains) in accordance with local, state, federal regulations including 40 Code of Federal Regulations (CFR) and 49 CFR. The contractor shall perform all work according to the appropriate industry standards. The contractor shall ensure that wastewater flow through the pretreatment systems is maintained in a manner that will prevent the cause of nuisance orders and prevent service interruptions. The contractor shall establish a service call system that is responsive to all types of service requests. A list of pretreatment systems and related workload data is included in the appendices. All costs should be Free On Board (FOB) Destination, Eielson AFB to include any containers or replacement parts IAW Par 1.4.1 and 1.6.
1.2. PREVENTIVE (PREDICTIVE) MAINTENANCE PROGRAM (PMP): The contractor shall develop and implement a PMP to correct and prevent pretreatment system discrepancies and maximize the overall operating efficiency of the system. The contractor's PMP shall take a program-driven maintenance approach to maximize productivity and reduce costs. The PMP shall demonstrate a proactive (rather than reactive) approach to work management to ensure that all assets are maintained at an appropriate level, such that equipment is neither over nor under maintained. The PMP shall require an inspection and maintenance action of all oil-water separators at a minimum interval of quarterly. The PMP shall include a mix of event driven (run to failure), preventive (based on manufacturer specified intervals), and predictive (based on reliability maintenance techniques) maintenance practices. The PMP shall address every component of the pretreatment system. The PMP shall address all of the technical and operating aspects of the pretreatment system, and provide the contractor's work schedule for a complete year. The PMP shall incorporate maintenance records for each equipment item within the pretreatment systems. The equipment records shall contain a complete history of maintenance accomplished. The contractor shall submit their PMP plan to Contracting Officer (CO) for acceptance within 30-calendar days after contract award. The contractor shall submit all updates and changes to their PMP plan to the COR (Contracting Officer Representative) within five (5) calendar days of making the updates or changes. Corrective maintenance and/or Preventive Maintenance to the Hazwaste Facility (Bldg. 4378) should only extend to the outlet of the Oil/Water Separator and not to the water treatment system. Refer to Par 1.3 for corrective maintenance requirements.
1.2.1. PMP Reports: The contractor shall develop and submit to the CO and COR a monthly report of all PMP activities. The report shall identify maintenance tasks and inspections that
Attachment 1: Performance Work Statement (REVISED 3 MAY 17) were not performed and a description of any corrective maintenance requirements noted that are not completed.
1.2.2. Sampling and Analysis: Within 30 days of contract procurement, the contractor shall develop a Sampling and Analyses Plan and collect samples and analyze pretreatment system contents as necessary to comply with regulatory guidance and/or permits. Sampling and Analysis should be completed on all systems that go directly into the sanitary sewer system on base once per year. Written results of sampling and analysis shall be provided to the CO and
COR.
1.3. CORRECTIVE MAINTENANCE: The contractor shall provide equipment repair and corrective maintenance service to return pretreatment systems and related support items to the proper operating condition. Corrective maintenance requirements may be identified during inspections, preventive maintenance, or through service calls, and when practical, corrective repairs shall be accomplished as soon as possible to sustain pretreatment operations.
1.3.1. Service Calls: The contractor shall establish a 24-hours per day, 7-days per week service call system. The contractor shall provide Quality Assurance Personnel with appropriate telephone number to initiate a service call. The contractor shall accept service calls only from the CO or a designated 354th Civil Engineer Squadron representative. Service calls may be classified as emergency, urgent, or routine. The appropriate classification will be given to the contractor along with the job service request.
1.3.1.1. Emergency Service Calls: Emergency service involves the correction of conditions that constitute an immediate danger to personnel, threaten property, and/or action required to restore essential service. The contractor shall be at the job site within four hours after receipt of an emergency service call and shall work continuously until the required service is completed or the condition is arrested.
1.3.1.2. Urgent Service Calls: Urgent service calls are used when a reported condition will not immediately endanger personnel or threaten property but, if not corrected soon, could affect the health or the well being of personnel. Urgent service calls shall be accomplished during normal working hours, or on the second or split shifts within the capability of the contractor. The contractor shall respond to the work site within 24 hours and shall complete the task within five
(5) days after the initial receipt of the service request.
1.4. WASTE DISPOSAL: The contractor is required to dispose of waste products to include but not limited to both hazardous and non-hazardous: liquids and solids. All waste must be disposed of at an off-base location, to be determined by the contractor.
1.4.1. Containers: The contractor shall provide the containers necessary to complete this contract. All containers shall meet UN/DOT specifications. The contractor shall invoice the government for the containers at a unit cost (FOB “Free on Board” Destination Eielson, AFB) as used with each separate disposal action. The contractor shall not invoice the government for any multi-use (re-useable) containers. Secondary containment shall be provided for all containers stored on EAFB. Containment units will be furnished by the government. All containers left in
Attachment 1: Performance Work Statement (REVISED 3 MAY 17) buildings shall be labeled listing the contents, point of contact and non-RCRA (Resource Conservation and Recovery Act).
1.5. SAFETY AND HEALTH STANDARDS: The contractor shall initiate and maintain programs to comply with the provisions of the Occupational Safety and Health Act concerning entry requirements for confined spaces and handling hazardous substances.
1.6. REPLACEMENT PARTS: The contractor shall provide all pretreatment system replacement parts with a unit cost of $50.00 or less for preventive (predictive) and/or corrective maintenance actions. The contractor shall not combine preventive (predictive) and/or corrective maintenance actions to exceed the $50.00 limit. The contractor shall invoice the government for replacement parts with a unit cost exceeding $50.00 for preventative (predictive) and/or corrective maintenance actions. The government will reimburse the contractor for the unit cost that exceeds $50.00. The contractor shall obtain prior approval from the CO before purchasing replacement parts.
1.7 BLAIR LAKE. Contractor shall provide the parts in accordance with 1.6, materials, equipment, and labor needed to pump, transport, and maintain repair wastewater pretreatment system at “United States Air Force” Blair Lakes Bombing Range, located approximately 20 miles South of Fairbanks, Alaska, on the west side of the Tanana River, in accordance with the PWS. The contractor will be required to respond during winter months every other year to service the complex at Blair Lake. Contractor shall respond to a call-out for this tasking within 5 business days. Sampling and Analysis must be completed every other year upon preventative maintenance of the system. Written results of sampling and analysis shall be provided to the CO and COR.
2. SERVICE SUMMARY

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES.
3.1. The government will provide the contractor with copies of the following pretreatment systems documentation upon reward of the contract:
• Analytical data generated by the government for systems content and waste classification
• Final Wastewater Treatment Plant Pretreatment Program, EAFB, Alaska (Jun 03)
• Final Oil Water Separator Assessment Report, EAFB, Alaska (Jan 03)
• Final Wastewater Pretreatment Systems Evaluation Plan, EAFB, Alaska (Feb 01)
• Historical analytical data for substances and materials taken from pretreatment systems
• Pretreatment systems manufacturer owner’s manuals
• Blueprints, diagrams, and schematics related to pretreatment systems and associated appurtenances and piping
• Secondary containment units
4. GENERAL INFORMATION
4.1. QUALITY CONTROL: Contractor shall develop and maintain a quality program to ensure maintenance and repair services are performed in accordance with the manufacturer’s instructions. The contractor shall develop and implement procedures to identify and prevent defective services from recurring. As a minimum, the contractor shall develop quality control procedures that address the areas identified in section 2, Service Summary. The government evaluator must have a specific quality control inspector to notify in case of customer complaints.
4.1.1Quality Control Plan (QCP). The Contractor shall develop and submit to the CO a QCP for approval within thirty (30) days of contract award.
It shall be designed to keep the contractor's management informed of all issues affecting quality.
The Quality Control (QC) records of inspection shall indicate the number and nature of observations made, the number and types of deficiencies found, and the nature of corrective action taken, as appropriate.
4.2. QUALITY ASSURANCE: The government will evaluate the contractor’s performance in accordance with the Performance Plan.
4.3. HOURS OF OPERATION. Normal hours of operation for EAFB run Monday through Friday (except for federal holidays) from 0700 to 1700.
4.4. SECURITY REQUIREMENTS:

4.4.1. Work under this contract will not require unescorted entry to controlled or restricted areas.
The government will provide escorts for those times when the contractor must perform work within controlled or restricted areas.

4.5. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE
NATIONAL COMMAND AUTHORITY OR OVERSEAS COMBATANT
COMMANDER: The contractor shall continue to provide all services required under this contract.
4.6. SPECIAL QUALIFICATIONS: Contractor personnel shall be certified by appropriate federal and state regulatory agencies to meet federal and local certification requirements in maintenance of oil/water separators.
