DIVISION 1 - GENERAL REQUIREMENTS 30-Apr-19
SECTION 010000 - GENERAL REQUIREMENTS
PART 1 GENERAL
1.1 SUMMARY OF WORK FOR PROJECT
This work shall be performed at Mountain Home AFB, ID. Work under this Statement of Work generally consists of the following:
The Contractor shall provide all labor, materials, services, oversight, and direction to complete the construction and installation of the requirements identified in this Statement of Work (SOW) and attached drawings. This project will involve demolishing an existing 24’ wide double leaf chain-link gate, creating a temporary dirt/gravel road around the site, installing a 20’ K12 rated Drop Arm Barrier including keypads, and providing electrical service from existing electrical to the new crash beam and LED lighting. All construction shall to adhere to the current standards set forth by ACI, ASTM, AASHTO, ISPWC, UFC, MUTCD, the manufacturer’s instructions, and other applicable standards as referenced in this SOW. This SOW only provides explanation of the basic desired facility and any requirements that are in addition to the aforementioned standards. Since this SOW does not reference all specific applicable standards, the Contractor shall present a proposal that reflects a thorough review of all standards, codes, and regulations that apply to the requirements within this SOW.
The following is a list of requirements:
1. Conduct nesting bird survey prior to construction activities. Contact the Natural Resource Program Manager at 828-6351 to coordinate survey. Any holes located at project site shall be filled in.
2. Remove existing 24’ double leaf vehicle gate:
a. Gate panels
b. Cabling
c. Hinges
d. Posts
3. Remove adjoining fencing as necessary to provide temporary gravel road to bypass construction area.
4. Demolish existing asphalt at locations required to install the new vertical crash beam and supporting items.

5. Construct foundation for vertical crash beam as required by the manufacturer. Concrete must be air entrained to improve durability and shall have a water to cement ratio not greater than 0.45. Minimum compressive strength shall be 4000 pounds per square inch (psi).
a. Contractor to provide stamped foundation drawings.
6. Install new electric 20’ clear opening wide K12 rated Drop Arm Barrier and keypads.
7. Provide utility trench and cabling from Drop Arm Barrier site to existing electrical panel located at building 3240. The vehicle barrier must be on the most current Department of Defense (DoD) Anti-Ram Vehicle Barrier List.
8. Install new overhead LED streetlights (minimum of 3 ft-candles, 5000k color rendition) to adequately supply light for the barrier keypad areas while minimizing light pollution.
Lighting to be powered by photocell and rated at 120/240V.
9. Install new, weatherproof, lockable panel to new light pole. Provide shut-offs for new lighting and barrier.
10. Connect new panel to existing service panel located at building 3240. Contractor to provide electrical shop drawings and calculations for voltage drop, type, size & number of wires with applicable conduit type & size, and circuit breakers.
11. Electrical installations shall be performed by licensed electricians. Proof of license shall be submitted prior to start of work.
12. Repair all disturbed roadway surface areas as specified by design plans.
B. The following work shall commence after the Vehicle Crash Beam system installation is complete and operational.
1. Remove temporary gravel road.
2. Replace fencing as needed including the following:
a. Install new fence posts and chain link fencing (7 feet tall) with barbed wire instead of existing 5 strand barbed wire.
b. New posts to be spaced 8’ apart. Fence shall conform to UFC 4-022-03.
c. Tie new fencing to existing fencing.
d. Install new vehicle-rated bollards to fill all gaps between the new barrier and replaced fencing as needed.
3. Fill in all holes made during construction and grade smooth all disturbed areas.
4. All disturbed areas shall be reseeded using the following seed mixture and shall meet with manufactures recommendation for planting.

5. Provide 4 hours of training to maintaining personnel regarding operation and maintenance of Barrier and keypads.
Construction area shall be maintained to provide security to the flight line at all times (24 hours per day). Security plan shall be approved by the Contracting Officer before construction begins. Contractor shall provide fencing to restrict area during non-work periods but allow access for government personnel.
The contractor shall provide all labor, equipment, supervision, services, and materials to execute and satisfy the requirements of this Statement of Work.
1.2 SITE VISITS
It shall be the responsibility of each Offeror to determine the nature, extent, and pertinent details of this Statement of Work. The drawings attached to the SOW show the general arrangement and approximate dimensions at the site. The Contractor shall be responsible for determining number of items, dimensions and details required for the proper accomplishment of the work as covered in this Statement of Work.
1.3 CODES AND STANDARDS
The most current codes shall apply or as noted on this SOW. Work shall meet the requirements of the plans and documents and shall not be less than the minimum requirements of applicable sections of the latest Codes and Standards of following organizations (Not All Inclusive)

1.4 SAFETY
A. The Contractor shall comply with the requirements of the Corps of Engineers Safety Manual EM-385-1-1 at all times in the performance of this SOW. Hazards to the safe use of the premises due to the Contractor's work and/or equipment shall be suitably marked at all times. Signage shall be provided to direct vehicle traffic to temporary bypass road.
B. The Contractor shall submit with the Safety Plan, the name of the designated on-site safety persons responsible for ensuring that all safety and accident prevention provisions of this SOW are implemented and enforced. The Contractor shall ensure his personnel receive safety training and briefings and that all safety inspections, practices, and procedures are within compliance. The name of the designated on-site safety persons, safety training, briefings, inspections, practices and procedures shall be included in the Safety Plan.
C. The Contractor shall indoctrinate each employee to ensure clear understanding of the following subject areas, but not limited to:
1. The purpose of the accident prevention program (i.e., to minimize hazards and prevent injuries).
2. A review of the hazards inherent in the work environment and the precautions to be taken.
3. Location of first aid and other emergency facilities.
4. A detailed description of what to do in case of injury, fire, or identification of a safety hazard.
5. Review of cleanup procedures.
6. Required safety equipment including earplugs, goggles, face guards, respirators, protective clothing, hard-hats, etc. (when and how to use).
7. Location of company safety rules (posting or handout).
8. Identification of the Contractor employees responsible for safety/accident prevention.
D. In addition to normal safety practices, the Contractor shall comply with the following requirements:
1. The safety of Contractor personnel on-site is the Contractor's responsibility.
2. All personnel shall receive an initial Air Force safety briefing, prior to site entry, and will comply with all requirements set forth in that briefing. All personnel shall comply with Air Force on-site safety procedures and the US Army Corps of Engineers Safety Manual EM-385-1-1 as directed by the Contracting Officer.
3. The Contractor shall ensure that safety barriers/portable ladders, ladder extensions, or environmental covers are properly installed at access hatches and excavations.
4. Equipment shall be placed so as not to impede normal traffic in and out of the work area, nor to interfere with emergency exit from work area and off-site.
5. Extreme caution must be exercised to preclude personal contact with energized electrical circuits. Contractor shall comply with NFPA 70 and 72E for electrical safety.
6. Hard-hats shall be worn in accordance with EM-385-1-1 requirements and hard-toed safety shoes shall be worn by all personnel at all times on the work site. Other safety items such as ear protection, respirators and safety glasses, which are required by EM-385-1-1 and/or the manufacturer of products for specific hazards, shall be available and used as necessary.
7. All heavy equipment shall be equipped with backup alarm in accordance with EM- 385-1-1regulations.
8. All vehicles shall be in safe operating condition. Vehicles and equipment not in compliance with EM-385-1-1requirements shall not be allowed on site.
E. A number of hazards inherent to the work environment must be recognized. They include, but are not limited to:
1. Electrical circuits and devices pose serious and generally continuing hazards.
Contractor shall comply with NFPA 70 and 72E for electrical safety. The Contractor shall take every precaution to prevent personnel injury. Ground Fault Interrupters (GFI's) shall be in-line appliances on all temporary electrical circuits installed near the supply.
2. Mechanical devices can pose serious hazards when operated. Appropriate precautions shall be taken.
F. First aid and medical requirements are as follows:
1. The Contractor shall comply with accident response requirements. These requirements are provided in a safety briefing by base personnel at the pre-construction conference. The Contractor shall keep these requirements on file at the working site for inspection by Air Force personnel and the Contracting Officer Representative (COR).
2. The Contractor shall maintain an emergency first aid kit at the work site.
3. All accidents (employee injuries and vehicle, building, or equipment property damage, etc.), regardless of severity, shall be reported promptly to the Contracting Officer.
1.5 SUBMITTALS
A. Submit the following according to the Contract and as required elsewhere in this SOW:
• Safety Plan
• Stamped foundation drawings
• Electrical shop drawings, with calculations; see Part 1.1 list of requirements, Item 10
• Construction Schedule
• Vehicle Gate Product Data and Crash Test Certification
• Photo documentation of Existing Conditions

PART 3 EXECUTION
3.1 INSPECTION
The Contractor shall schedule with the inspector during normal work hours the times for inspecting items such as "Concealed Work" or other times. The inspector will be the spokesman for compliance with the documents and drawings.
3.2 SUBMITTAL SCHEDULE
Contractor shall provide submittals listed in section 1.5 in a timely manner such that construction is not delayed. Submittals will require a ten business day turnaround period.
3.3 SUBMITTALS
Submittals shall be recorded on Form AF IMT 3000, shall have the Submittal ID No., Specification paragraph number and description of material as labeled on Submittal Schedule.
The Contractor shall submit to the Contracting Officer for review and approval the following information: Manufacturer's product brochures and catalog data for all equipment, components and materials to be installed. All material Submittals shall be original vendor information (not copies) and include location on Drawings with cross referencing information to Drawing schedules and Documents, dimensional, assembly, and sub- assembly Drawings, parts lists, and other data required to completely describe each item. Include the Drawing schedule letter, symbol, or plan mark on the Submittal. Where more than one product is listed in a document, the item submitted shall be properly identified by circles, arrows, or a description with cross-references. Contractor shall use electronic submittals unless otherwise directed by the CO. Circle or arrow all descriptive literature to accomplish identification of specific components/items which are proposed to be furnished. Government will not react to descriptive material/literature, which has not been circled or arrowed, except to reject the Submittal.
3.4 PERMITS
The Contractor shall be responsible for securing the dig permit and all permits as may be necessary for the completion of all work required by this project.
3.5 WORK TIMES
The Contractor’s working hours shall be from 0700 to 1700 Monday thru Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. Night lighting surveys may be authorized upon request to the Contracting Officer.
Deviations from the above listed work hours must be approved by the Contracting Officer. No night work will be allowed without prior approval by the CO and an approved night lighting survey. Note: When possible, the Government will allow the Contractor to continue to perform duties in the event of an administrative closing. The Contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this SOW when the Government facility is not closed for the above reasons. When hiring personnel, the Contractor shall keep in mind that the stability and continuity of the workforce are essential.
3.6 START OF WORK NOTIFICATION
The Contractor shall notify the Contracting Officer when the initial construction work shall begin and shall notify when work shall resume after a work stoppage of more than three workdays.
3.7 WASTE, PROCESSING AND STORAGE SITES
Processing and storage sites shall be only those areas described at the pre-construction meeting.
Location of stockpiled materials will be coordinated with the COR. Pre-Construction site photographs shall be submitted to the Government prior to disturbing any approved site. Upon Project completion, the site shall be restored to original condition (equal to that before Project start) to include all finish grading, leveling, shaping, contouring, and seeding. All waste shall be disposed of at a properly licensed disposal facility, unless otherwise approved.
3.8 UTILITIES
Reasonable quantities of utilities will be made available (from existing utilities) to the Contractor without charge. Any temporary connection or lines that may be required will be installed, maintained and removed by the Contractor at his own expense and in a manner satisfactory to the Contracting Officer. Connections to MHAFB electrical systems shall be done in accordance with the National Electrical Code (NEC). Removal of such connections or lines will be accomplished prior to final acceptance of the construction. The Contractor shall provide any necessary portable power. No restroom facilities will be available for Contractor use. It is the responsibility of the Contractor to provide a portable latrine (Porta-Potty). Porta-Pottys shall be emptied weekly.
3.9 UTILITY OUTAGES
All utilities programmed to be interrupted during construction shall be scheduled at least 10 working days in advance of the outage and at a time convenient for the government. No interruptions shall be made until the Contracting Officer approves the outage.
3.10 MAINTENANCE OF AIR FORCE OPERATIONS AND CONTINUED USE
Scheduling of work at the site is of paramount importance. Every effort should be made to coordinate the work schedule with the COR in order to ensure a minimal disruption of operational activities. Proper coordination will help ensure that Contractor work at the facility will continue with minimal interruption due to Air Force maintenance activities. In the event of an unplanned maintenance emergency at a site under construction by the Contractor, the Contractor shall have an approved contingency plans in place.
3.11 ROAD CLOSURES
Contractor shall maintain access to and from site and the temporary gravel road at all times.
Road closure requests shall be submitted at least 14 days in advance. Signage, barricades, etc., shall be in accordance with the MUTCD.
3.12 CONTRACTOR VEHICLES, EQUIPMENT AND MATERIALS
A. The Contractor shall provide all vehicles and equipment necessary to accomplish the work.
Power equipment shall be equipped with safety and noise limiting devices. The equipment shall be in a safe and efficient operating condition.
B. All vehicles shall be maintained in a safe operating condition in accordance with the requirements of EM-385-1-1. All vehicles not essential to the job will be left off site.
C. The Contractor will be permitted to store necessary materials and vehicles on site. Only those materials, storage vehicles or equipment mounted vehicles that must remain overnight or over the weekend and directly pertain to the work being performed at the site may be stored. Material must be placed in a designated area per Waste, Processing, and Storage Site paragraph and will not interfere with site access. If the designated area is not of sufficient size to accommodate all of the materials or vehicles, then all excess must be removed from the location. Storage must be coordinated with the Contracting Officer. The Contractor shall submit to the Contracting Officer at the pre-construction conference the size of storage area requested. The Government shall not be responsible for the loss of Contractor material, tools, or equipment.
D. At the end of each workday, the Contractor shall clean up the work area debris, ensure the security of equipment, tools and work/Contract materials. Contractor shall at all times ensure that construction work/stockpiled material/debris does not impact or impede Base Operations. Secure all materials, waste, etc. from blowing off site.
3.13 CONCEALED WORK
All items of work to be concealed shall be inspected by the Government prior to concealment.
All Government inspection shall be within the work hours specified in paragraph 3.6 “Work Times.”
3.14 CONSTRUCTION AREA
The Contractor shall coordinate laydown/storage/staging areas, use of water and use of electricity with the Contracting Officer. Complete restoration of all areas disturbed by the Contractor to existing conditions shall be the responsibility of the Contractor and shall be at the Contractor's expense. Provide photo documentation of existing areas to be utilized by the Contractor to the Government before beginning construction operations.
3.15 CONSTRUCTION NEAR UTILITY SYSTEMS (AF Form 103)
Utility Locates: The Contractor is responsible for calling Idaho Digline to initiate the dig permit process. (1-800-342-1585) Digline in turn calls all local utilities, and the base. The base will perform any locates on utilities that are owned by the Air Force. Air Force Civil Engineering Customer Service section will prepare the AF Form 103 that the Contractor must pick up (Building 1300). Contractor shall allow for at least 10 work days for this process after calling Digline. The Contractor shall clearly identify the perimeter of the area to be marked with white paint, and is responsible for maintaining all utility markings for the duration of the work.
3.16 CONSTRUCTION STAKING
The Contractor shall provide construction staking.
3.17 ENVIRONMENTAL
See Attachment #1 for Mountain Home AFB Temporary Environmental Controls (01 57 19).
3.18 NUCLEAR DENSOMETERS
A. Where nuclear densometers or other devices containing radioactive elements are used on the Project, the Contractor must submit the following for each device to be used:
1. Serial number of all nuclear densometers to be used on AF property,
2. Copy of the corresponding US Nuclear Regulatory Commission (NRC) license,
3. Agreement State License and NRC Form 241 (for "Agreement State" firms),
4. Proof of a Nuclear Regulatory Committee (NRC) permit for its use and
5. Contractor site-specific health and safety plan (HASP).
B. The Contractor must also keep all radioactive equipment locked up when not in use and remove it from the work site at completion of the workday.
3.19 NOTICE OF FINAL INSPECTION AND ACCEPTANCE
A final inspection will be held after completion at the site. This inspection shall be coordinated with the Contracting Officer at least 7 calendar days prior, shall include personnel from Base Civil Engineering and a representative from the using activity. Final Inspections will only be accomplished once all work is done and all deficiencies have been corrected from the Pre-Final inspection punch list.
3.20 AS-BUILT DRAWINGS
Redlined plans of all deviations to provided plans shall be submitted to CE in the form of AutoCAD 2014 and PDF.
3.21 OPERATING AND MAINTENANCE INSTRUCTIONS
A. Operating and Maintenance Instructions shall be submitted on Form AF IMT 3000 per submittals paragraph 3.4. Provide complete sets of instructions (O&M Manuals), for all equipment and materials including architectural, civil, mechanical and electrical to the Contracting Officer.
B. 2 electronic copies (PDF) and 2 hard copies of complete Operating and Maintenance Instruction sets shall be submitted twenty working days after final inspection acceptance of the site.
C. These O&M Manuals shall include as a minimum: all material, equipment and associated existing equipment cross referenced to Contract Documents and Drawing Schedules, existing and new electrical wiring and interfaces, mechanical interfaces, troubleshooting guides, parts lists, parts manuals, operating and maintenance instructions. Include all equipment and material, parts manual information required to order (purchase) replacement, install, maintain, and repair items that was actually installed.
D. All AS-BUILT FIELD DATA, including a copy of the final approved AS-BUILT full size plot Drawings, the marked Shop Drawings, survey notes, sketches, submittals, nameplate data, pricing information, description, and serial numbers of all installed equipment shall be included in the Operating and Maintenance Manuals.
E. Each manual shall be in a three-ring binder with a rigid and durable cover. Where information or instructions are included for more than one model or part, the Contractor shall clearly identify and mark the model and parts with circles, arrows, and a description with cross references of equipment and material actually installed on the job. Large-size Drawings or diagrams shall be folded and placed in heavy weight sheets with pockets. See each Division for additional requirements.
F. The manuals shall be sectionalized using dividers with labeled tabs identifying the product/item behind the dividers. Include all Shop Drawings with the product/item. All drawings and schematics shall include ALL new and existing items with interface connections, and all details associated with the installation. The first Section shall include a table of contents with the equipment list, all guarantee information, extra material list as described in Paragraph Contractor Furnished Equipment List. Each individual product/item shall be labeled per Equipment List with Specification Section, and Drawing schedule nomenclature and location.
G. Include all tests with the completed System Checklist, and the completed Submittal Schedule with each manual.
H. Include all Material Safety Data Sheets (MSDS) of all materials on the site.
I. Each binder shall be indelibly marked on the exterior front and edge with the Project name, Project number, Contract number, name of Contractor, and date.
3.22 CONTRACTOR FURNISHED EQUIPMENT LIST
A. The Contractor Furnished Equipment Lists, Guarantees (warrantees) and extra materials list for the site shall be provided with the Operating and Maintenance Instructions. The Contractor shall submit the following data.
B. Equipment List: An itemized list of equipment showing retail value and nameplate data including, but not limited to, serial number, model number, size, manufacturer, installed location, cross reference to Specification Section, Drawing schedules nomenclature, and all other pertinent information, for all Contractor furnished items installed under this Contract. This list shall be filled out for each item installed.
C. Guarantees (Warrantees): Provide a list of all equipment and material items, which are specified to be guaranteed, accompanied by a copy of each specified guarantee. Each specific guaranteed item on the list shall include the name, address, and telephone numbers of the supplier or distributor and the manufacturer. The start and expiration dates of the guarantee period shall be included. Warrantees shall be for one (1) year minimum from date of final acceptance of the work or longer if provided by equipment manufacturer.
3.23 AIRFIELD SECURITY
Contractor shall ensure the access to Airfield is restricted at all times through a locked gate on the temporary gravel road only. The lock will be a government lock. When the project requires removal and replacement of the fence, the area where the fencing is removed shall not be left open for any length of time during construction process. The fencing shall be removed for the shortest time possible. Security Forces need to be notified with removal of the fence before it occurs. The contractor is responsible to provide temporary fencing as needed to only allow vehicle access through the locked gate on the temporary gravel road. No vehicles are allowed to access the airfield except through contractor installed temporary gate and by opening government lock.
