MEMORANDUM FOR SMALL BUSINESS CONTRACTORS

SUBJECT: Request For Proposal – FA4890-16-R-0030, Line Operations Safety Audit

(LOSA) Support

1. The Government is requesting proposals for Headquarters Air Combat Command (HQ ACC) Safety Office (SE) Contract Solicitation number FA4890-16-R-0030, as defined in the PWS (Attachment 1) with an anticipated award date on or about 31 Aug 16, and performance start date of 12 Sep 16. The Government’s evaluation methodology will be Lowest Price, Technically Acceptable (LPTA) as defined in the attached Evaluation Methodology and Criteria (Attachment 3).

2. This is a new requirement and will be competed in the Small Business. Please submit your Technical and Price/Cost proposals on a Firm Fixed Price (FFP) basis.

3. Technical Proposal. The Technical Proposal shall be clear, concise, and shall include sufficient detail that substantiates stated claims. The proposal shall not simply rephrase or restate the Government’s requirements, but rather provide convincing rationale to address how the Offeror intends to meet the requirements. Offerors shall assume that the Government has no prior knowledge of the Offeror’s capabilities. The Government will base its evaluation solely on information presented in the Offeror’s proposal. The technical proposal will be limited to ten

(10) pages. Knowledge Skill and Abilities (KSA) qualifications summaries are not included in the technical proposal page count.

4. Price Proposal. Please submit a price proposal on a Firm Fixed Price basis Travel is cost reimbursable. The Government estimates $30,000.00 annually for travel (see PWS paragraph 7.0). When preparing your offer, apply your travel handling rate as appropriate to the estimated travel and submit as part of your price proposal. Total travel is estimated to not exceed $90,000.00 over the life of the contract. Price proposal will be limited to three (3) pages. The CLIN Structure will be as follow: FFP for Labor (unit of issue, LOT) and Cost Reimbursable for Travel (unit of issue, LOT).

5. Past performance will be evaluated based on responsibility analysis in accordance with FAR 9.104. The standards contained within FAR 9.104-1, including having a record of satisfactory performance record and integrity and business ethics, serve as the guidelines for this determination. The offeror must be determined responsible to receive the contract.

Please provide up to three (3) samples of relevant past performance with Point of Contact/Telephone Number.

6. The Government intends to evaluate proposals and award without discussions with Offerors. Therefore, each Offeror’s initial proposal should contain the Offeror’s best terms from a cost or price and technical standpoint. The Government reserves the right to conduct discussions, at the discretion of the Contracting Officer (CO).

7. Offerors shall address Organizational Conflict of Interest (OCI) as an attachment to the proposal in accordance with FAR Part 9.5. If an Offeror determines that a potential or real OCI issue exists and still wishes to proceed through the proposal process, the Offeror shall notify the CO prior to submission of the proposal to determine the need for an OCI Plan specific to the requirement.

8. Funds for the base year will be fully funded.

9. Contracting Officer Representatives (CORs) will be officially appointed in writing by the CO immediately after contract award.

10. Prior to submission of proposals and during source selection, Offerors shall limit all inquiries to the CO or Contract Manager (CM) for the specific contract. Any questions or concerns regarding this RFP are due to our office NLT 22 June 2016. Do not contact any other government personnel or offices until after contract award by the CO.
