STATEMENT OF WORK
PROJECT NUMBER: FBNV18-0022
225 FUEL CONTAINMENT
DAVIS-MONTHAN AFB, AZ 85707
Project includes, but is not limited to, the following work elements. All plant, labor, equipment, materials, services, and incidentals shall be furnished by the Contractor. No alterations, additions, or deletions shall be made unless by prior approval of Contracting Officer (CO) or government representative. Contractor shall design the project in accordance with the base Design Compatibility Guidelines (DCG), review it for constructability, and advise of possible problem areas or conflicting guidance prior to delivery order negotiation. Under no circumstances is the Contractor to begin any work on a contract until a bon-a-fide Notice-to-Proceed is issued by the CO. All work must be in accordance with Unified Facilities Criteria (www.wbdg.org) unless express written permission is received from the Contracting Officer for any deviations. All work must meet or exceed acceptable standards and codes of the building trades; and shall be installed in a neat, substantial, and workmanlike manner. The following descriptions are for general reference only, and shall not be construed as a complete list of all operations to be performed. This project is expected take no longer than 60 days from the awarded date.
1.0. SCOPE OF WORK.
1) Removal of existing sealant located at bldg. 225 secondary fuel containment. The contractor shall remove an existing (estimated) 900 sf. (approximately 28’x 23’ base with 28” walls) of damaged exterior jet fuel containment sealant, by wet abrasive blasting down to the concrete substrate.
2) Cleanup/pressure wash surface to remove any contaminants.
3) Patch/repair minor cracking as necessary to seal cracks in containment walls (approx. 1 bag).
4) Replace sealant. Apply two (2) coats of Kem Cati-Coat HS Epoxy (or equal) @ 10-20 mils per coat.
5) Apply one (1) coat of Phenicon HS Epoxy (or equal) @ 5-7 mils.
All debris shall be removed from the work site and placed in contractor provided disposal containers.
Upon completion, containment and surrounding area shall be completely free of the above-mentioned foreign debris/material.
Ensure any permits, manifests, environmental requirements, fall protection, etc. are accomplished prior to work start.
Work Site Preparation. Prior to commencing any work the contractor shall coordinate the date and time and any set-up with the Project Point of Contact (POC) Brian Wahl 520-228-5322. The government will make available electric power and water as necessary. The contractor will need to verify the utilities are sufficient to meet the project requirements prior to start.
Warrantee Documents. Upon completion the contractor shall provide the Project POC with two copies of all necessary warrantee documents to be distributed as necessary.
1.1. GOVERNMENT FURNISHED PROPERTY AND SERVICES.
Government Furnished Property and services. None.
1.2. GENERAL INFORMATION
1.2.1. On Site Foreman. The contractor shall provide an on-site foreman who shall be responsible for contract performance and can act for the contractor on all matters relating to daily operation of the contract.
1.2.2. Work Area and Finishes. The contractor shall, at all times, keep work premises in a clean, neat, safe and workmanlike condition. The contractor is responsible for removal and proper disposal of all job related waste material at a permitted site. Cleanup site and haul off-base all construction debris from the site daily. Replace any materials to remain that are damaged during demolition to match existing, in a neat, substantial, and workmanlike manner, to the satisfaction of the CO or government representative at no cost to the Government. All paint colors shall match the existing adjacent surfaces, and shall be approved by Project Management and/or the government representative prior to use. No asbestos-containing materials may be used or installed in this work.
1.2.3. Safety. The contractor shall comply with all applicable laws and safety regulations.
Compliance with OSHA and other applicable laws and regulations for the protection of employees is exclusively the obligation of the contractor, and the government will assume no liability or responsibility for the contractor’s compliance or noncompliance with such responsibilities.
1.2.4. Working Hours. The contractor shall perform services between the hours of 7:00 a.m. and 4:00 p.m. Monday through Friday, excluding Saturdays, Sundays and Federal Holidays. If the contractor desires to work after 4:00 p.m., weekends, or during Federal Holidays, the contractor must submit the request to the host unit. Each request will be considered on a case-by-case basis. Host unit operations sometimes require no contractors are allowed access to the site. The host unit will make every effort to provide the contractor a forecast of time spans where access will not be permitted. Additionally, although rare, operations can dictate that the contractor be requested to cease work immediately and vacate the job site.
1.2.5. Recognized Holidays. The following Federal Holidays are observed by the host unit: New Year’s Day, Martin Luther King's Birthday, Presidents’ Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans’ Day, Thanksgiving Day, Christmas Day
1.2.6. Emergency Services. The government will provide fire and police services. There are no emergency medical services available on base. The contractor shall dial 911 for emergency medical services.
1.3. SECURITY
1.3.1. Security Requirements. The construction site is in a restricted area and will require the
Contractor and subcontractors to have the requisite badging and identification to access this project site. The Contractor shall consider in the contract documents (e.g., the construction cost estimate) the reasonable costs and effects of daily, short delays in accessing the project site, as required by the restr ict ive access requirements or by local alert conditions.
1.3.2. Security Training. All contractor employees shall receive security education training from the host unit security manager. Training must be conducted in accordance with DoD 5200.1-R, Information Security Program, and AFI 31-401, Information Security Program Management.
Contractor personnel who work in Air Force controlled/restricted areas must be trained IAW AFI 31- 101, The Air Force Installation Security Program.
1.3.3. Pass and Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees and non-government owned vehicles.
1.3.4. Traffic Laws. Contractor employees shall comply with all base traffic regulations. Contractor employees are subject to random vehicle speed control checks. Contractor personnel cited for speeding on the installation may suffer loss of base driving privileges, debarment from the base, or other administrative action.
1.3.5. Random Personnel and Vehicle Searches. Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base, and may result in loss of base driving privileges, debarment from the base, or other administrative action.
1.3.6. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor owned vehicle or privately owned vehicle while on Davis-Monthan AFB.
1.3.7. For Official Use Only (FOUO). The contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.
1.3.8. Reporting Requirements. Contractor personnel shall immediately report to an appropriate government authority any information or circumstances of which they are aware may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information.
1.3.9. Physical Security. Host unit shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.
1.3.10. Base Entry Requirements. The contractor shall obtain Government identification documents for all employees, by submitting an application to the CO or government representative. The contractor must provide the full name, driver’s license number (including state), social security account number (SSAN), date of birth (DOB), and hours of required access, for each contractor employee. Any employee who does not have a driver’s license must provide another official form of picture ID, such as INS work permit or an Arizona ID (available through the Arizona Department of
Transportation). Any employee desiring to bring a privately owned vehicle on Davis-Monthan AFB must provide current registration and at least the minimum required liability insurance, as well as a valid driver’s license. Contractor ID cards or temporary base pass will be available for pick up within seven working days after application is received.
1.3.11. Contractor Vehicle Requirements. All vehicles used in the performance of the contract shall be properly registered and meet local, state, and federal safety and emissions requirements. Each contractor vehicle shall have the Contractor Logo neatly exhibited on each side of the cab. The sign must use at least 2" high letters. Vehicles shall be in satisfactory mechanical condition and present a neat and clean appearance. Any vehicle(s) not so maintained may be ordered removed from the base by and at the sole discretion of the CO or government representative, until satisfactory repairs are made. The contractor shall operate vehicles to comply with base traffic regulations. Vehicle-mounted communication equipment shall conform to all applicable base and federal frequency regulations and avoid conflict with existing frequencies.
1.4. Environmental Control.
1.4.1. The Contractor shall be required to meet or exceed the requirements and compliances of the most current 355 CES/CEIE, Davis-Monthan Contractor Environmental Guide in effect at the time of project award. To access the Davis-Monthan Contractor Environmental Guide: Go to www.esohtn.com

To get access to applicable Davis-Monthan documents and plans; from the home page Select “Installation Specific Information” then click on Davis-Monthan AFB Plans, then select the applicable plan. 1.4.2. Additionally, the Contractor shall be held responsible for complying with all federal, state, and local environmental regulations included as part of the processes in performing the work included in this project.
1.4.3. Hazardous material (HAZMAT) and/or regulated wastes may be encountered during construction.
Contractor shall plan and submit a HAZMAT worksheet to the gov’ts contract representative prior to construction - see Appendix A and Attachment(s) A-1 to A-3 of the Environmental Guide. Contractor shall provide the Contracting Officer with a chemical inventory and an estimated quantity to be used, prior to bringing hazardous materials on the base. The chemical inventory and appropriate SDS or MSDS’s shall be provided as indicated in Appendix A of the Contractor Environmental Guide. These requirements shall be included as a submittal on the AF Form 66, Schedule of Material Submittals.
1.4.4. Contractors shall comply with all regulatory requirements/considerations if the project requires soil disturbance. These requirements may include Storm Water Pollution Prevention Plans (SWPPP), Air Activity Permits (see para d above), and Natural and Cultural Resource protections. Area disturbance requiring permits is set forth by Arizona Department of Environmental Quality (ADEQ) and PDEQ.
Treat areas subject to dust producing activities with liquid palliatives which will not harm regrowth of vegetation, or other methods of dust control. Also, ensure methods to protect native plants, birds and wildlife are included in projects where required.
1.4.5. No open burning of trash, brush, or refuse will be permitted.
The Contractor shall be responsible for the proper management of both hazardous and nonhazardous waste and the required submittals IAW Section 3 and Appendix C of the Contractor Environmental Guide. The Contractor shall segregate hazardous or regulated materials and non-hazardous or non-regulated materials during demolition; and handle, transport, store, and dispose of these hazardous and non-hazardous materials in accordance with applicable federal, state, and local laws, ordinances, or other rulings having the effect of law, including but not limited to the items listed below. In addition, the Contractor shall obtain proper disposal manifests for both hazardous and regulated materials, and for non-hazardous or non-regulated materials, and submit these manifests to the Contracting Officer. These manifests shall be included as a submittal on the AF Form 66, Schedule of material Submittals. Submit original manifest-with original signature of the Contractors superintendent or authorized representative in block 14 attesting to the validity of the manifest-to the Contracting Officer within 3 calendar days of disposal. For wastes regulated as PCB’s or RCRA, the Contractor must use DM’s Environmental
Management Office at the time of shipment. All handling, transporting and disposal costs shall be considered as having been included as a factor in the Contractor’s bid price. Any Questions regarding proper handling or disposal of hazardous or non-hazardous materials may be directed through the Contracting Officer to the Installation Management Flight at 520-228-4840.

1.4.6. Liquid paints or coatings with lead shall be restrictive to no more than 0.06 percent lead by weight of the non-volatile solids for use in or on all base facilities [Part 35 of 24 CFR]
