STATEMENT OF WORK

Equipment Installation, Bldg. 1225 & 4625

27th Special Operations Force Support Squadron, Cannon AFB, NM

21 June 2024

1. GENERAL INFORMATION: The contractor shall provide all required kitchen equipment, tools, and equipment necessary to install all equipment and connect to utilities in accordance with this Statement of Work

(SOW).

1.1. DELIVERY DATE: Delivery and installation of furniture shall be completed no more than 90 days after award of contract.

1.2. HOURS OF OPERATION: Contractor personnel shall perform the installation during normal business hours until completion. Exceptions to those hours will be weekends, holidays, and AFSOC indicated down-days.

Scheduled coordination will be conducted at contract award.

2. POC INFORMATION: Name and point of contact information will be provided upon award of the contract.

3. SCOPE OF WORK: The contractor shall provide all labor, materials, equipment, and supervision necessary to complete installation of the purchased equipment as outlined in this SOW. The work includes, but is not limited to, the following tasks: 1)Disconnection and removal of existing kitchen equipment, 2) Installation of new kitchen equipment, 3)Electrical and plumbing modifications as required 4)All parts necessary to install equipment to current utilities. 5)Relocation of disconnected equipment to designated DRMO holding area. and 6) 4 hour training with personal on how to use equipment.

3.1. The spaces where the kitchen equipment will be installed are specific. For design purposes, pictures of the kitchen layout will be provided in Attachments 1 & 2.

3.2. Once the contract has been awarded, it becomes the responsibility of the contractor to validate the actual site conditions as well as any specific programming needs of the customer. Design work after evaluation may result in changes to the sizing or other necessary modifications to the specifications provided. It is understood that the actual site parameters or certain customer requirements may deviate from the standard typical layouts, therefore any deviation that causes an increase in the price submitted during the bid process must be documented in writing and submitted to the Contracting Officer for review, along with a floorplan reference to show just cause. A final furniture layout and Bill of Materials (BOM) shall then be provided meeting the customer requirements and accommodating the physical environment.

4. SPECIFIC REQUIREMENTS

4.1. BUILDING 1225 & 4625: Reference Attachments 1 & 2

4.1.1. Removal and Disposal – Safety disconnect and remove all specified outdated kitchen equipment.

Move all equipment to provided location for DRMO in accordance with local regulation and Air Force policies.

4.1.1.1. Install new kitchen equipment as specified in the equipment list (Attachment 1). Ensure all installations comply with manufacturer specifications and Air Force safety standards. Perform all necessary electrical and plumbing modifications to accommodate new equipment.

4.1.1.2. Testing and Commissiong – Test all installed equipment to ensure proper operation. Address and correct any deficiencies identified during testing. Provide a commissioning report detailing the testing process and results.

4.2. SECURITY REQUIREMENTS

4.2.1. All work in Pecos Trail Dining Facility and High Plains Café must be done within a secure area. All personnel subjected to this Statement of Work must adhere to 27 SOW security requirements. All works must be cleared and escorted by 27 SOFSS personnel. No unapproved electronic devices are allowed.

5. REQUIREMENTS: The required work shall be performed in accordance with this Statement of Work and all applicable Federal, State, local laws, and regulations.

5.1. MANDATORY BACKGROUND CHECK/SECURITY REQUIREMENTS: All Contractor personnel supporting the installation shall undergo a thorough security background check to ensure supporting personnel meet the required clearance in accordance with the DoD guidance. To enable the security checks to be conducted in a timely manner, the Contractor shall submit to the Contracting Officer (CO) within three (3) duty days after the contract award, to the following information for each employee that will require to be escorted on Cannon AFB in support of the kitchen equipment delivery and installation.

• Current Physical Address

• City and Sate of Birth

• State ID Number or Driver’s License Number, State of Issue and whether or not the ID is enhanced

In addition, a rental agreement or vehicle registration and insurance shall be provided for each employee that will be operating a vehicle in the premises of the base.

5.2. DAMAGE/SAFETY: The Contractor shall conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work as required by OSHA. The Contractor shall provide protection to government property to prevent damage during the period of time the property is under the control or in possession of the Contractor. Any damage caused by the Contractor during installation is the responsibility of the Contractor and must be reported to the Contracting Officer or designated Contracting Officer Representative?

Each instance of damage to the government property shall be reported promptly (within two (2) hours) to the Contracting Officer Representative or Government Point of Contact.

5.3. UNSATISFACTORY PERFORMANCE: Unsatisfactory performance to include but not limited to damaging equipment while installing, installing damaged equipment, installing equipment incorrectly, damaging government property while installing equipment, or incorrect installation of equipment shall result in work being re-accomplished at the Contractor’s expense with no additional cost to the Government.

5.4. PHOTOS: No worker shall take photos at any time while on Cannon AFB. All photos shall be coordinated with 27 SOW/SSO office via M. Nicholas Kingsley and Mr. Robert Brewster.

5.5. STORAGE: The Contractor shall store and safeguard materials until the contractor has prepared the worksite for installation. No storage is available on Cannon AFB unless authorized by the CO.
