ATTACHMENT 1
Statement of Work (SOW)
SOF Construct Concrete Pad
Project Number: CZQZ17-0003
Cannon Air Force Base, New Mexico
28 June 2017
1. BACKGROUND: 27 SOAMX is in need of a storage location for UTC and aircraft equipment that must be protected from outside elements. A Canopy top shelter has been purchased by the government and requires installation by the contractor on top of a new concrete pad that will be constructed near H4605 with a driveway for easy transporting of equipment using a 15k forklift. The site location is in a low drainage area, so it will need to be brought up to match the elevation of the nearby pad and not withdraw from the drainage system that is already in place. In order to keep the current drainage system, this will require regrading in the vicinity of the new concrete pad and adding a culvert as necessary.
2. OBJECTIVES: The objectives of this Work include but are not limited to:
2.1. Excavate/regrade, as necessary to establish the site for construction of a 72 foot x 72 foot concrete pad and 12' driveway (refer to drawings).
2.1.1. Construct new reinforced concrete pad, 4000 PSI, 8 inches thick, with pad footings for anchoring and thickened edge around entire pad.
2.1.1.1. New concrete pad shall be same elevation as existing concrete pad southwest of the site location. Concrete shall maintain .5% slope from the Northwest end flowing towards the Southeast end.
2.1.1.2. New concrete pad shall be located 40 foot off the NE edge of the existing pad located nearby. In addition, the new pad shall be parallel and in-line with the SE edge of the same nearby pad.
2.1.1.3. 3’x3’x3’ Pad footings shall be constructed to meet the layout as required for the
Government Furnished Shelter columns. (refer to attached manufacturer drawings)
2.1.1.4. Concrete pad shall be constructed with a thickened edge and a ½” rounder over to encompass entire pad as identified in design drawings.
2.1.1.5. Grade 35 foot out from edge of new pad each direction to provide 1% slope from 1 inch below new pad height. Utilize excavated soil to grade surrounding area to meet the
1% slope requirement. Excess soil can be taken to the Government landfill on Cannon
AFB (approx. 2 miles from worksite).
2.1.1.5.1. Reseed area being graded at 35 feet out with native grasses.
2.1.2. Construct a new 4 inch thick, 12 feet wide, asphalt drive path from existing road to center of concrete pad (refer to drawings).
2.1.2.1. Center driveway to southeast side of concrete pad and connect to adjacent asphalt road as identified in drawings.
2.1.2.2. Install a 18 inch diameter culvert, 16 foot long, under new driveway with headwalls to allow the flow through of the natural drainage route.
2.1.2.3. All utilities that are crossed over shall be sleeved and concrete encased as required in area of drive path and at minimum of 6 feet beyond edge of road.
2.1.2.3.1. Concrete encasement of primary electrical lines shall be stained red or painted red. UFC 3-550-01.
2.1.2.3.2. Sleeve existing 12 inch water line with steel black pipe, schedule 40. Pipe shall be 16 inch inner diameter and extend at least 6 feet outside the edge of road on both sides. Concrete encase and stain blue or paint blue.
2.1.2.3.3. Sleeve existing gas line with steel black pipe, schedule 40. Pipe shall be 8 inch inner diameter and extend at least 6 feet outside the edge of road on both sides.
2.1.2.3.4. Concrete encase communication line below the drive path, extending minimum of 6 feet on each side.
2.1.2.4. Install two 8 inch diameter bollards; one on each side of the water valve box that is nearest to the new road: refer to design drawings.
2.1.2.5. Install two 8 inch diameter bollards: one on each side of the gas valve box nearest to the new road: refer to design drawings.
2.1.2.6. Regrade Southwest side of driveway to provide proper flow of storm water to and through new culvert.
2.1.2.6.1. Place geotextile fabric (12 oz. per square yard) along 10 feet wide 106 feet long section of trench, flowing northeast towards the new culvert.
2.1.2.6.2. Place Rip Rap, size 3 to 6 inch, on top of the geotextile fabric, placed at 8 inches thick.
2.1.2.7. Regrade Northeast side of driveway from new pad to existing culvert to provide proper flow of storm water from the new culvert to the existing culvert: slope towards existing culvert shall be no less than a 1% slope.
2.1.2.8. Set basin on Northeast side of new driveway going from new culvert flowing southeast to existing culvert; 94 foot.
2.1.2.8.1. Elevation at existing culvert shall be no less than 6 inches below existing culvert’s invert and approx. 20 foot wide at starting point to 20 feet wide at the existing culvert.
2.1.2.8.2. Line basin and edges of driveway with geotextile fabric (12 oz. per square yard).
2.1.2.8.3. Place Rip Rap, size 3 to 6 inch, on top of the geotextile fabric, placed at 8 inches thick.
2.2. Install Government Furnished modified Quonset Fabric Shelter per manufacturer drawings; attached.
2.2.1. Openings are to run northwest to Southeast, with Southeast end being the primary entrance.
2.2.2. Each truss base shall be anchored to the 3’x3’x3’ pad footing (refer to drawings for placement of base plates).
3. PERIOD OF PERFORMANCE: 90 calendar days.
4. MEETINGS: The contractor shall attend one project kickoff meeting within one week of Notice to Proceed and one post-survey briefing following the site visit. The contractor shall prepare and distribute meeting minutes from each meeting.
5. GOVERNMENT FURNISHED INFORMATION: Upon the Contractor’s request, the Government will provide electronic and/or hard copies of available documents and correspondences pertinent to the Work.
6. DELIVERABLES: The following must be provided to the designated Contracting Officer (CO) and
Contracting Officer’ Representative (COR) or Program Manager (PM):
6.1. Project Schedule: Within one week of notice to proceed, provide a legible project schedule in printed and
PDF format for design services to include dates of anticipated site visits, team meetings, and other milestone dates.
6.2. AS-BUILT CONSTRUCTION DRAWINGS AND SPECIFICATIONS: Upon completion of construction, provide as-built drawings and specifications incorporating any departures from the original contract documents.
6.2.1. Electronic As-built Drawings and Specifications: Provide electronic drawing files in a format compatible with AutoCAD 2010 including all external references required to view and print the drawing files properly. Include all necessary *.ctb, *.pc3 or similar files needed to properly set line weights for printing. Drawing files should be organized in general accordance with the Department of Defense A/E/C CAD Standard (ERDC/ITL TR-12-X) available in PDF format on many federal websites. In addition, provide drawings in PDF format for on-screen viewing and reproduction.
Provide final project specifications in PDF and Microsoft WORD *.doc or *.rtf format.
6.2.2. The file naming convention and file structure shown in the As-built Submittal Guidelines shall be used in lieu of what is found in the Department of Defense A/E/C CAD Standard.
6.2.3. Hardcopy As-built Drawings and Specifications: Provide two (2) full size and two (2) half size printed copies of the as-built drawings and two (2) copies of the final specifications.
6.3. Operation and Maintenance Manuals: Provide O&M Manuals including all warranties, vendor contacts, relevant product data including proper maintenance and operation, and pertinent shop drawings describing all installed materials, systems and equipment in the project. Manuals must be organized by
CSI specification sections.
6.4. GEOGRAPHIC INFORMATION SYSTEMS (GIS): All new facilities and utilities shall be located by the Contractor in an electronic format that is compatible with the current base GIS software and includes descriptive information required by the Air Force. When the Contractor is unable to provide GIS information as described, the Contractor shall notify the PM so that other arrangements can be made. In such cases, the Contractor is required to assist Air Force agents or personnel in the generation of GIS information if requested by the PM. Subject to change, the following are the basic requirements for GIS information to be delivered to the Air Force on CD:
6.4.1. DATUM – WGS84
6.4.2. Coordinate System – UTM
6.4.3. Units – Meters
6.4.4. Data in attribute Table will be SDSFIE version 2.6 Compliant
6.4.5. Shapefile Geodatabase format
