Request for Quote: FA4855-21-Q-0007
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this procurement is FA4855-21-Q-0007 and is a Request for Quotation (RFQ). The North American
Industry Classification System (NAICS) is 238210. Small Business Size Standard is $16.5 Million.
This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2021-03, Defense Federal Acquisition Regulation Supplement Change Notice (DPN) 20210224, and Air Force Acquisition Circular (AFAC) 2019-1001

Special Notes and Instructions:
1. This is a notice that this order is a total set-aside for small business concerns under NAICS 238210. Only quotes submitted by small business concerns will be accepted by the Government. Any quote that is submitted by a contractor that is not a small business concern will not be considered for award.
2. There will be no Site Visit.
3. Questions. Any questions shall be submitted in writing via email to SSgt Rachel Rickabaugh at E-mail:
rachel.rickabaugh@us.af.mil and Robyn Knight at robyn.knight.1@us.af.mil no later than 4:00 p.m. MST, 11 March 2021. All questions will be answered and posted to BetaSAM.gov.
4. Basis for Award. 27 SOCONS/PKA will issue a purchase order to the vendor who is determined to offer the best value to the Government. In accordance with FAR 52.212-2, the Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
a. Price Evaluation. Offers will be ranked from lowest to highest based on price. A price evaluation will be conducted on the lowest priced proposal to determine price reasonableness.
b. Technical Acceptability. The Offeror shall indicate that they understand and meet all requirements outlined in Attachment 1 – Performance Work Statement. If a vendor has exception(s) to the requirements of the PWS, offeror shall clearly state all exceptions. Provide as specifically as possible the actual methodology you would use for accomplishing/satisfying each factor. Do not merely reiterate the objectives or reformulate the requirements specified in the solicitation. The Government will evaluate the technical acceptability on an acceptable or unacceptable basis in accordance with the technical rating factors outlined below. Offeror must be rated Acceptable to be eligible for award.
i. FACTOR 1: The Offeror shall submit their proposed schedule that shows the ability to meet the required transformer inspection, testing, and preventative maintenance as outlined in the PWS within 20 days after contract award. The Offeror shall provide a Gantt chart or equivalent schedule that clearly shows the timeline for each performance element in the PWS. The Offerors proposed schedule shall demonstrate their ability to start work within 20 days after contract award and complete work within 30 days after starting work.
ii. FACTOR 2: The Offeror shall submit at least one (1), but no more than three (3), satisfactorily performed contract(s) in the last five years that has the same or similar scope and magnitude as the requirements outlined in Attachment 1 – Performance Work Statement. The previous contract information shall include the following:
• Contract #
• Contract/Project Title
• Place of Performance
• Performance Work Statement or equivalent document that describes the scope of work performed
• Customer POC: Name, phone number, and email
• Moreover, offeror shall demonstrate that they are able to produce the required documentation outlined in PWS paragraph 1.1.5. The Offeror shall submit a sample of finalized reports from previous contracts that are similar in scope and magnitude to the documentation requirement in PWS paragraph 1.1.5.
Notes:
1) Previous contracts performed more than five (5) years ago will not be considered.
2) To be considered similar or the same scope and magnitude as the PWS of this requirement, previous contracts must be similar in scope and magnitude to, but not limited to the following efforts:

a. Perform internal and external transformer inspection for units rated at three phase, 480 Volts Alternating Current (VAC) primary, up to 500 kilovolt amp (kVA) capacity.
b. Perform transformer turns ratio testing, insulation resistance testing, and winding conductor resistance testing for units rated at three phase, 480 VAC primary, up to 500 kVA capacity.
c. Perform PM for transformers rated at three phase, 480 VAC primary, up to 500 kVA capacity, in a manner that will not damage any transformer component, which includes PM for transformer windings and connections, and removal of foreign debris.
d. Perform forensic analysis for transformers rated at three phase, 480 VAC, up to 75 kVA capacity, which includes inspection, disassembly and testing that is similar in scope and magnitude to what is described in PWS paragraph 1.1.4.2.
To be deemed acceptable under this factor, Offeror’s proposal shall demonstrate that within the last five (5) years, they have satisfactorily performed on previous contract(s) that have the same or similar service requirements as Attachment 1 – Performance Work Statement.
iii. FACTOR 3: To be deemed acceptable under this factor, as referenced in Unified Facilities Guide Specifications (UFGS) 26 08 00, Change 1, Apparatus Inspection and Testing, paragraph 1.4.1, the Offeror shall provide a statement affirming the following representations:
1) The Offeror is independent of the supplier, manufacturer and installer of the equipment listed in PWS Appendix B
2) The Offeror has been engaged in the testing of electrical materials, devices, installations and systems for a minimum of five (5) years; and,
3) The Offeror served as primary contractor or first-tier subcontractor responsible for contract performance.
5. Best Value Determination. The technically acceptable, lowest priced quote will be determined to represent the best value. Technical acceptability will be determined by an evaluation of the quoted requirements that meet the technical criteria that is contained in the solicited item description. If the lowest priced submission does not meet the technical criteria described in the solicited item description, the government reserves the right to evaluate the next lowest priced submissions until it has determined a technically acceptable submission. The evaluation will stop at the point when the government determines an offeror to be technically acceptable with the lowest evaluated price, because that offer will represent the best value. Contingent upon a determination of contractor responsibility, award will be made to that offeror without further evaluation of other offers. Award will be made to the responsible vendor whose quote is the lowest evaluated price among technically acceptable quotes.
6. Discussions. The government intends to award a purchase order without discussions with respective vendors/quoters. The government however, reserves the right to conduct discussions if deemed in its best interest.
7. Mandatory Registrations. To be eligible for award, registration with the System for Award Management (SAM) must be current (legacy for On-line Registration and Certifications Application (ORCA) and Central Contractor Registration (CCR) registrations).
8. Place of Performance. Service is to be performed on Cannon AFB, NM 88103 for a period of performance of 50 days ADC.
9. Attachments. The following attachments are to be included with this Request for Quote;
a. Attachment 1 – Performance Work Statement, dated 18 February 2021.
b. Attachment 2 – Wage Determination, dated 21 December 2020

NEW MEXICO GROSS RECEIPT TAX: The state of New Mexico assesses a Gross Receipts tax for firms conducting business within the state. This tax is imposed on the basis of revenue derived from business operations within the state without regard to the location of the business entity. You are advised that any offer received will be presumed to include this cost of business and any request for adjustment of your price as a mistake in bid either before or after award of a contract will not be considered. The rate of the New Mexico Gross Receipts Tax may be obtained from the New Mexico Taxation and Revenue Office in Roswell, New Mexico by calling 575-624-6065.

5352.201-9101 Ombudsman (JUNE 2016)
(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.
(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).

5352.223-9000 Elimination of Use of Class I Ozone Depleting Substances (ODSs) (NOV 2012)
(a) Contractors shall not:
(1) Provide any service or product with any specification, standard, drawing, or other document that requires the use of a Class I ODS in the test, operation, or maintenance of any system, subsystem, item, component, or process; or
(2) Provide any specification, standard, drawing, or other document that establishes a test, operation, or maintenance requirement that can only be met by use of a Class I ODS as part of this contract/order.
[Note: This prohibition does not apply to manufacturing.]
(b) For the purposes of Air Force policy, the following products that are pure (i.e., they meet the relevant product specification identified in AFI 32-7086) are Class I ODSs:

5352.223-9001 Health and Safety on Government Installations (NOV 2012)
(a) In performing work under this contract on a Government installation, the contractor shall:
(1) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor
(2) and Government personnel performing or in any way coming in contact with the performance of this contract;
and
(3) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes.
(b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract.
(c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract.

5352.242-9000 Contractor Access to Air Force Installations (NOV 2012)
(a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.
(b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver’s license, current vehicle registration, valid vehicle insurance certificate, and obtain a vehicle pass.
(c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.
(d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with comply with AFI 31-101, Integrated Defense, and AFI 31-501, Personnel Security Program Management.
(e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the http://www.e-publishing.af.mil/shared/media/epubs/AFI31-501.pdf issuing office.
(f) Failure to comply with these requirements may result in withholding of final payment. (End of clause)
