SUBJECT: SITE VISIT, ACCESS REQUEST LETTER
The following personnel are requesting access to Tyndall AFB for the purpose of a site visit. They will be escorted at all times by government personnel.

Escorted base access shall only be granted for the above individuals for the purpose of a site visit to the:
Contract name: FAMCAMP Internet Contract site: Building 4017, Tyndall AFB, FL 32403
Located on the following side of base: Near Bonita Bay
Site visit hours: (start) 1400 hours – (end) 1530 hours
Site visit date: 09 June 2017
All individuals are aware that they must bring a valid driver’s license or government issued photo identification to be granted access to the installation.

CAC-card holders can escort a max of 6 personnel, who must remain with you at all times. Line badge holders must conduct site visits in restricted areas. BDOC must be notified in advance of site visits in restricted areas.
