Performance Work Statement

1. SCOPE

This performance work statement (PWS) is to provide fiber optics and 100 Mbps Synchronous internet service. Project is designed to support the Tyndall AFB Fam Camp customers by allowing internet access via Wi-Fi connectivity.

1. APPLICABLE DOCUMENTS

Engineering Technical Letter (ETL) 02-12, 27 June 2002

1. GENERAL DESCRIPTION

The contractor shall provide all design, labor, and materials to install, program, and train Fam Camp Staff on infrastructure/equipment. Project will provide 100 Mbps Synchronous internet service. Project is designed to support Tyndall AFB Fam Camp staff and customers with internet access.

The following is a summary of work:

1. Supply and install fiber optics for 100 Mbps internet service to Bldg. 4017.

1. Provide and install any equipment necessary to deliver 100Mbps internet service.

1. Coordinate location of fiber termination and equipment installation with 325 Force Support Squadron IT personnel.

1. Provide 1 static public IP address for use on Customer Premise Equipment (CPE).

4.0 SCHEDULE: All work must comply with environmental laws and regulations. The contractor shall prepare and deliver an integrated master schedule detailing program activity. The contractor shall baseline the project schedule/plan upon contractor and Government agreement. In addition, the contractor shall identify any changes to the schedule by providing an updated schedule. The contractor shall include justification and requests for approval for all schedule modifications.

5.0 CONTRACTOR PROFESSIONAL SERVICES AND RESPONSIBILITIES

5.1 General

The Contractor shall provide infrastructure/equipment and all applicable fiber optics for 100 Mbps internet service to Building 4017. The fiber connection shall provide immediate access to the internet upon completion of work. Completed system must adhere to all Air Force and base installation practices, USAF/DoD regulations, technical letters and instructions, and applicable industry standards. Any work deemed to be not compliant shall be re-worked at the contractor’s expense.

5.2 Dig Permits

The Contractor shall coordinate with the state and TAFB for all dig permits and markings as needed prior to the beginning of the project.

5.3 Maintenance Support

The Contractor shall provide all maintenance on the system when a component of the system fails. There shall be a twenty-four hour response time for on-site maintenance to fix a failed system component (during normal business hours (8:00 a.m-4:00 p.m. CST M-F)). This should be included in the service contract.

6.0 PROCUREMENT

6.1 Equipment Warranty

7.0 INSTALLATION

The contractor shall provide all equipment and supplies necessary to support this requirement. The contractor shall provide a list of all components installed per this project, as well as a list of the components removed if any.

7.1 Testing and Acceptance

7.1.1 Testing

The contractor will ensure installation is operational and meets all required commercial, local and international industry standards, military instructions, standards, policies, guidelines, regulations and practices, and Tyndall AFB/325 FSS standards. The testing procedures will be provided in writing to the 325 FSS and also a copy of the outcome of the performed test.

7.1.2 Acceptance

The expansion shall operate with minimal failure after acceptance testing and activation of service. The contractor shall provide all support necessary during this period to resolve any issues that may arise. As-built drawing shall be provided upon completion of the project to a 325 FSS representative.

8.0 PLACE OF PERFORMANCE

The location of installation is on Tyndall AFB Fam Camp. The contractor is expected to work during normal Tyndall business hours, 0800 to 1600 CST unless coordinated with the Fam Camp manager.

9.0 HOURS OF WORK

The contractor shall perform all work during normal duty hours, 0800 to 1600 CST, Monday through Friday excluding federal holidays.

10.0 SITE SURVEY

No more than a 1-day site survey will be required and scheduled through the 325 CONS office before vendor selection. Provide a site survey report with allied support requirements, proposed install drawings, updated bill of materials, and coordination information unless coordinated with the Fam Camp manager.

11.0 POINTS OF CONTACT (POC):

Point of Contacts will be provided at time of award of the contract.

12.0 ADDITIONAL REQUIREMENTs:

All work area shall be kept as clean as possible during working hours. Debris will not be allowed to accumulate on work site. All debris will be deposited in an authorized container supplied by the contractor.

13.0 Base access requirements:

1. In order to promote security and safety at Tyndall Air Force Base all contractor employees requesting access to the installation are subject to AFI 31-101 AMC Sup Tyndall AFB Sup 11 JAN 2008 which requires individuals to be screened by criminal background history (CBH) checks, National Crime Information Center (NCIC) wants/warrants checks, and California Law Enforcement Telecommunication System (CLETS) driver license history checks.

1. Contractors are divided into two categories, to ensure proper screening and to minimize installation access delays, as follows:

1. Category 1 (Cat-1) – primary and subcontractors who are identified against a contract prior to the contract performance start date.

1. Category 1 (Cat-2) subcontractors and affiliate workers not previously identified against a contract prior to contract performance start date.

1. Contractors are required to provide an EAL (entry authorization list) all Cat-1 employees which must include the following information:

2. Contract Number

2. Work Site/Location

2. Inclusive dates of the contract (start date – end date)

2. Employee Name (Last, First, MI)

2. Date of Birth

2. Social Security Number

1. Contractor is to have each employee identified on the EAL complete TAFB Form 251, Contractor Consent to Background Check and submit it with the request for issuance of Tyndall AFB Contractor Badge or Pass a minimum of 30 days prior to performance on the contract.

1. Contractor will be notified of any individuals whose background check meets any of the following 12 disqualifiers as they will not be allowed access to the installation.

4. U.S. citizenship, immigration status, or Social Security Account Number cannot be verified.

4. Barred from entry/access to any military installation or facility.

4. Individual is wanted by federal or civil law enforcement authorities, regardless of offense or violation.

4. Individual’s name appears on any federal agency’s list for criminal behavior or terrorist activity.

4. Individual was convicted of espionage, sabotage, treason or terrorism.

4. Individual was incarcerated for 12 months or longer within the past three years, regardless of offense or violation.

4. Individual was convicted of a firearms or explosives violation within the past three years.

4. Individual has been convicted of illegal possession or use of drugs/narcotics on more than one occasion within five years from the date access to TAFB is requested.

4. Individual has been convicted of an offense involving drug trafficking, possession with intent to sell or drug distribution within 10 years from the date access to TAFB is requested.

4. Individual has been convicted of a felony involving violence against a person, arson, robbery or burglary within 5 years from the date access to TAFB is requested.

4. Individual is required to register as a sex offender under federal law or the applicable state law.

4. Individual has been convicted of any crime involving indecent acts with a minor or a felony that is sexual in nature.

1. Individuals adjudicated and cleared under the disqualifying criteria are considered cleared for installation access for a two-year period. Thereafter, renewal is required.
